Left atrial appendage and atrial septal occlusion in elderly patients with atrial septal defect and atrial fibrillation.

BACKGROUND: Elderly patients with atrial septal defect (ASD) often present with chronic atrial fibrillation and large left to right shunt. This study reports the experience of left atrial appendage (LAA) and ASD closure in patients with significant ASD and chronic atrial fibrillation. METHODS: We report six consecutive elderly patients with chronic atrial fibrillation and significant ASD who underwent LAA and fenestrated ASD closure from January 1, 2014 until December 31, 2019. All periprocedural and long-term (>1 year) outcomes were reported. RESULTS: Six patients (male: 33.3%; mean age: 66

Patients with Gastrointestinal Bleeding and Atrial Fibrillation: Potential Ideal Target for Epicardial Appendage Occlusion.

BACKGROUND: Gastrointestinal bleeding in patients with atrial fibrillation is an indication for left appendage occlusion. All endovascular devices mandate antithrombotic therapies: rebleeding risk remains an issue. To date, there are no reports on gastrointestinal rebleeding and stroke prevention by left appendage occlusion without any antithrombotic therapy in this category of patients. METHODS: A total of 129 patients (male 85, mean age 76.6 ± 7.1, CHA2DS2Vasc 3.8 ± 1.5, HASBLED 3.3 ± 1.0; upper GI bleeding 10%, lower GI bleeding 86%, obscure occult 4.6%, on NOACS full dose 77.5%, NOACs red

Three-dimensional CT-fluoroscopy fusion imaging improves the efficiency of left atrial appendage occlusion: A comparative study with a real-time guidance planning system.

Real-time three-dimensional (3D) fusion of cardiac computed tomography (CT) with fluoroscopy may enhance procedural guidance in left atrial appendage closure (LAAC), yet comparative data with conventional planning systems are limited METHODS: In this retrospective comparative study, 109 consecutive patients undergoing LAAC were assigned to guidance using either 3D CT-fluoroscopy fusion (n = 51) or the real-time operation guidance planning system (ROGPS, n = 58). Procedural efficiency, radiation exposure, contrast use, and clinical outcomes were analyzed RESULTS: The 3D fusion group demonstrate…

Effectiveness and safety of left atrial appendage electrical isolation in managing persistent atrial fibrillation: an updated meta-analysis of 3425 patients.

BACKGROUND AND AIM: Atrial fibrillation (AF) is a substantial public health disorder associated with heightened morbidity and mortality. Pulmonary vein isolation (PVI) has limited efficacy in persistent AF (PAF), indicating additional strategies such as left atrial appendage electrical isolation (LAAEI). This study evaluates the efficacy and safety of LAAEI in managing PAF. METHODS: An extensive search of the literature was carried out on PubMed, Scopus, Web of Science, and Cochrane Library. We included all the eligible studies comparing LAAEI plus PVI to PVI alone in managing PAF. Our primar

Totally Thoracoscopic Ablation for Atrial Fibrillation: All-Box Clamping.

BACKGROUND: Epicardial surgical ablation is an effective strategy to treat non-paroxysmal forms of atrial fibrillation. Current thoracoscopic epicardial surgical strategies are complex, and are therefore often avoided. With slight modifications to the thoracoscopic maze procedure, totally thoracoscopic all-box clamping may facilitate the performance of epicardial thoracoscopic ablation, while maintaining good results. METHODS: Between December 2023 and December 2024, 42 patients underwent thoracoscopic all-box clamping at a single center. All-box clamping uses commercially available bipolar r

The Deceptive Shadow: Inverted Left Atrial Appendage Mimicking Thrombus After Infant Open Heart Surgery.

Inversion of the left atrial appendage (LAA) is a rare postoperative complication following cardiac surgery, often misdiagnosed as thrombus or vegetation on imaging, potentially leading to inappropriate management. We report a case of a five-month-old female infant who underwent elective surgical closure of a perimembranous ventricular septal defect via a right atrial approach under cardiopulmonary bypass. On postoperative day 2, transthoracic echocardiography (apical four-chamber view) revealed a 1 × 0.9 cm echogenic mass attached to the anterior mitral leaflet in the left atrium. The mass ca

Right atrial appendage rupture and cardiac tamponade secondary to blunt trauma.

BACKGROUND: Right atrial appendage rupture from blunt trauma is exceedingly rare, even more so when no other chest wall injuries are found. Very few cases have been documented with respect to survival from such an injury. PURPOSE: To highlight the optimal management of such cases, namely through timely and safe transport to a trauma centre, maintaining a high degree of clinical suspicion for tamponade, early diagnostic ultrasound use, pericardial decompression, haemorrhage control and situational control. CASE PRESENTATION: A case report delineating the diagnostic and therapeutic approach to

Transient Cortical Blindness and Hemi-Spatial Neglect Following Watchman Device Implantation.

A 66-year-old male with prior right middle cerebral artery stroke status post thrombectomy-as well as right internal carotid artery (ICA) stenting and chronic occlusion, paroxysmal atrial fibrillation, and hypertension-underwent left atrial appendage occlusion with a Watchman device for stroke prevention. Immediately following the procedure, he developed acute bilateral vision loss accompanied by headache and worsening left-sided weakness. Emergent computed tomography (CT) and CT angiography demonstrated no hemorrhagic stroke or new large-vessel occlusion and confirmed the previously known chr

Isolated Chylopericardium: An Unusual Cause of Late Cardiac Tamponade After Mitral Valve Repair.

Isolated chylopericardium after cardiac surgery is extremely rare, but potentially fatal. We present an unusual case of late postoperative chylopericardium causing cardiac tamponade 6 weeks after mitral valve repair, tricuspid annuloplasty, and left atrial appendage closure through median sternotomy. Emergent pericardiocentesis was performed. Microscopic analysis confirmed the chyle. The patient was successfully managed conservatively with oral dietary manipulation and intravenous octreotide.

Epicardial approach for cardiac electrophysiology procedures.

Since its introduction over two decades ago, percutaneous epicardial procedures have become well-adopted by cardiac electrophysiologists, most commonly for catheter ablation of cardiac arrhythmias as well as left atrial appendage closure. The percutaneous epicardial approach has also been utilized for cardiac pacing and drug delivery. But still, its most common usage is for the treatment of intramural and subepicardial substrates that give rise to ventricular tachycardia, particularly in patients with nonischemic cardiomyopathy. In fact, subxiphoid, percutaneous epicardial mapping and ablation…

Early Discontinuation of Long-Term Anticoagulation After Surgical Left Atrial Appendage Occlusion.

BACKGROUND: Surgical left atrial appendage occlusion (sLAAO) is known to prevent strokes in patients with atrial fibrillation (AF). However, for AF patients on long-term anticoagulation (AC), little is known about the safety of AC discontinuation after sLAAO. METHODS: The study included Medicare patients with AF on AC undergoing coronary artery bypass grafting or valve surgery from 2016 to 2019. By propensity score matching, patients who did not continue AC more than 90 days after surgery were compared with patients continuing AC. RESULTS: Of 10,407 patients undergoing sLAAO, 2470 (14.0%) ex

Large hemopericardium and pericardial tamponade from presumed erosion of a Watchman 2.5 device 1-year postimplantation.

Percutaneous left atrial appendage closure has allowed patients with atrial fibrillation who are at high risk of bleeding to safely discontinue their anticoagulant therapy shortly after device implantation. The procedure, however, comes with a small risk of complications, including pericardial effusion and tamponade. The complications pertaining to pericardial effusion occur mainly perioperatively. We present an 82-year-old man with a 24 mm Watchman 2.5 device who developed hemopericardium resulting in tamponade and shock from presumed erosion of the device into the pericardium 1 year after im…

Left atrial appendage occlusion with Watchman device after incomplete surgical ligation.

Non-valvular atrial fibrillation (NVAF) is the most common cause of cardioembolic stroke. The left atrial appendage (LAA) is the major source of cardiac emboli in patients with NVAF. Anticoagulation (AC) is the standard of care for stroke prevention in atrial fibrillation (AF), but many patients are intolerant of AC. Surgical exclusion of the LAA may result in incomplete closure and is associated with an increased risk of embolism. We report a case of a woman in her 50s with a history of persistent AF, mitral valve prolapse s/p repair with surgical LAA exclusion, and multifocal haemorrhagic st
