Feasibility and Acceptability of a Palliative Care Intervention among Older Adults with Advanced CKD and Their Caregivers.

BACKGROUND: In non-nephrology settings, specialty palliative care (PC) improves decision making, patient's quality of life (QoL), advance care planning, and certain indicators of the quality of end-of-life (EoL) care. This pilot randomized control trial (RCT) explored the feasibility and acceptability of a PC intervention, CKD-EDU, for adults aged 75 years and older with eGFR ≤25 ml/min and their caregivers. METHODS: Participants randomized to the control group received standard nephrology care and routine kidney therapy education, whereas those randomized to CKD-EDU received a decision aid a

Pain management during the withholding and withdrawal of life support in critically ill patients at the end-of-life: a systematic review and meta-analysis.

PURPOSE: To review and summarize the most frequent medications and dosages used during withholding and withdrawal of life-prolonging measures in critically ill patients in the intensive care unit. METHODS: We searched PubMed, EMBASE, the Cochrane Database of Systematic Reviews, and the Virtual Health Library from inception through March 2019. We considered any study evaluating pharmaceutical interventions for pain management during the withholding or withdrawing of life support in adult critically ill patients at the end-of-life. Two independent investigators performed the screening and data

Ethical Dilemmas in End-of-Life Care: A Psychological and Anesthetic Perspective.

End-of-life care presents some of the most complex and emotionally challenging ethical dilemmas in modern medicine. For anesthetists and clinical psychologists, these dilemmas intersect at the critical juncture where physiological support meets psychological suffering. This editorial advances the novel argument that the psychological distress inherent to the dying process constitutes a specific clinical entity (existential suffering), which requires interdisciplinary management distinct from standard symptom control. It explores the major ethical challenges encountered in end-of-life care, inc

A short tool to screen self-care preparedness: cross-sectional study in general practice.

BACKGROUND: Self-care is crucial in the prevention and treatment of chronic diseases. It is important to identify patients who need support with self-care. OBJECTIVES: This study introduces a self-care preparedness index (SCPI) and examines its associations with health-related quality of life (HRQoL) and other outcomes. METHODS: A cross-sectional study of adults (n = 301) with hypertension, coronary artery disease, or diabetes in primary health care. Based on the self-care questionnaire, SCPI was formed. A higher SCPI value indicated better self-care preparedness. We examined correlations an

Not Just for the Dying: The Benefits of Routine Palliative Medicine Consultation in ECMO Care.

IntroductionPalliative medicine (PM) aims to enhance the quality of life of patients and families facing serious illness. Currently, there are no national guidelines for involving PM in the care of patients on extracorporeal membrane oxygenation (ECMO), a form of advanced life support used in cardiac and/or respiratory failure.MethodsWe conducted a retrospective cohort analysis of adult ECMO patients hospitalized in 2023 at a tertiary academic medical center. During this time, a protocolized approach to PM was piloted-a default PM consult was embedded in the initial ECMO order set, and provide

Adherence to prophylactic haemophilic treatment in young patients transitioning to adult care: A qualitative review.

Recombinant prophylactic treatment (PTX) has greatly improved morbidity, mortality and health-related quality of life (HRQoL) in patients with severe haemophilia. Yet, treatment adherence appears suboptimal in adolescents and young adults with haemophilia (YWH). Young patients experience major biopsychosocial changes challenging their adherence through the transition from parental to self-care, from paediatric to adult care. In clinical practice, a systematic approach to transition is rarely used and there is little evidence on best practices. This qualitative review was based on a systematic

Effect of early integration of supportive care in addition to standard care in patients with breast cancer: A randomised controlled trial.

PURPOSE: Breast cancer is the most common malignancy among women in India, with patients often experiencing high symptom burden and compromised quality of life (QoL). Despite evidence supporting early supportive care integration, significant implementation gaps persist in low- and middle-income countries (LMICs). This study addresses the lack of feasible intervention models for resource-constrained settings and inadequate characterization of mechanisms through which early supportive care improves outcomes-particularly symptom-specific contributions and financial burden reduction. METHODS: Thi

Economic Decision Model for First-Time Traumatic Patellar Dislocations in Adolescents.

BACKGROUND: The surgical management of traumatic patellar dislocations in adolescents is associated with a lower rate of recurrent dislocations compared with nonoperative care. However, the attendant cost of surgery and the quality-of-life benefit of a surgical treatment strategy are unclear. PURPOSE: To compare the cost-utility of 3 management strategies for acute first-time patellar dislocations in adolescents: (1) nonoperative treatment only, (2) initial nonoperative treatment with surgery only for recurrent dislocations, and (3) immediate surgery. STUDY DESIGN: Economic and decision anal

Is it time to address survivorship in advanced breast cancer? A review article.

The outcome of advanced breast cancer has significantly improved over recent decades. As a consequence, the complex needs of patients living with the disease and their care-givers should be addressed not only in terms of supportive and palliative care but also of "survivorship" requirements. The multidisciplinary approach to advanced breast cancer should encompass - early in the history of the disease - not only physical but also functional, social, psychological and spiritual domains. It is important to clearly define the disease context with patients and families ("chronic" preferred to "inc
