[The diagnosis of tubercular lymphadenitis in the case records of the Institute of Pathological Anatomy of Siena University in 1970-1981].

A study of the diagnosis of the tubercular lymphadenitis was carried out. No significant difference of incidence was observed through the years in the considered period. Tubercular lymphadenitis was more frequent in males under 15 years and in females over 51 years. The mixed reactive and caseous type of tubercular lesion was the most frequent. Non reactive caseous lymphadenitis was significantly more frequent in the youngest males. Tuberculosis was clinically suspected only in 49.5% of the cases.

Clinical and Laboratory Characteristics Distinguishing Node-First Kawasaki Disease from Bacterial Cervical Lymphadenitis: A Systematic Review.

Node-first Kawasaki disease (NFKD) presents with fever and cervical lymphadenopathy as the sole initial manifestations, mimicking bacterial cervical lymphadenitis (BCL). Delayed diagnosis may increase coronary artery lesion risk. This review synthesizes evidence differentiating NFKD from BCL. We conducted a PRISMA 2020-compliant systematic review registered with OSF. PubMed, Web of Science Core Collection, and Cochrane CENTRAL were searched, supplemented by handsearching and citation tracking. Eligible studies compared clinical and laboratory findings between children with NFKD and BCL. Two re…

Mycobacterium mageritense-associated refractory cutaneous infection and lymphadenitis in an immunocompetent adult: insights from genomic sequencing.

BACKGROUND: Nontuberculous mycobacteria are increasingly recognized as causes of chronic and refractory skin and soft tissue infections, eve … hy man presented with a slowly enlarging cutaneous lesion on his lower leg and ipsilateral inguinal lymphadenitis. Empirical antibacterial therapy with β-lactams and macrolides was ineffective. Wound cultures subsequently grew M. mageritense, confirmed by whole-genome sequencing. Several antimicrobial regimens were attempted, and the final successful therapy co …

Paediatric cervicofacial lymphadenitis caused by non-tuberculous mycobacteria: nation-wide overview in the period 2000-2020.

PURPOSE: Cervicofacial lymphadenitis caused by non-tuberculous mycobacteria (NTM) is a rare but increasing infection in children worldwide. The purpose of this study was to analyse and evaluate all microbiologically proven cases of NTM lymphadenitis in children under the age 14 years in Slovenia. METHODS: Between 2000 and 2020, we retrospectively reviewed laboratory and medical records for basic demographic and microbiological data. Different clinical samples were collected in medical centres and regional hospitals from all over Slovenia. RESULTS: In the period before mandatory BCG vaccinati…

Study of Diagnostic Yield of Nucleic Acid Amplification Test among Tuberculous Cervical Lymphadenitis in Immunocompetent Patients.

BACKGROUND: The most common form of extrapulmonary tuberculosis (TB) is tuberculous lymphadenitis, which constitutes about 30-40% of all extrapulmonary TB cases. A new diagnostic method like the nucleic acid amplification test (NAAT) is a very sensitive and rapid test for diagnosing tuberculous cervical lymphadenopathy. It also detects rifampicin sensitivity among positive patients. OBJECTIVES: (1) To evaluate the diagnostic yield of TrueNAT for detecting Mycobacterium tuberculosis bacteria in the fine-needle aspirated samples of cervical lymph nodes compared with Ziehl-Neelsen (ZN) staining;…

Successful long-term management for postoperative sternal infection with multiple disseminated lymphadenitis caused by Mycobacterium abscessus.

BACKGROUND: Postoperative sternal infection caused by Mycobacterium abscessus (M. abscessus) is rare, but associated with a high 2-year mortality rate of 40%. Decision-making around treatment strategy is challenging. Here, we present a successfully treated case of postoperative M. abscessus sternal infection with multiple disseminated lymphadenitis. CASE PRESENTATION: The patient, an 80-year-old woman with anterior mediastinal tumor and myasthenia gravis, underwent extended thymectomy under median sternotomy. Redness appeared around the scar two months after the operation. Sternal wires were …

Case Report of Kikuchi-Fujimoto Disease from Sub-Saharan Africa: An Important Mimic of Tuberculous Lymphadenitis.

Kikuchi-Fujimoto disease (KFD) is a rare form of painful lymphadenopathy, usually cervical, which is more common in Southeast Asia and rarely reported from Africa. Symptoms are usually nonspecific (fever, night sweats, etc.), and can mimic more common diseases such as tuberculosis (TB) in endemic areas. We report a case of a 29-year-old black African woman who was admitted with headache, neck pain, fever, and lymphadenopathy. She was found to have aseptic meningitis, eventually attributed to TB based on cervical node biopsy, although further histology suggested KFD. Blood tests for systemic lu

Acute viral lymphadenitis mimicking low-grade peripheral T-cell lymphoma. A clinicopathological study of nine cases.

Acute viral lymphadenitis, especially infectious mononucleosis (IM), often shows the presence of Reed-Sternberg-like cells, resulting in confusion with Hodgkin's disease. However, acute viral lymphadenitis requiring differential diagnosis from non-Hodgkin's lymphoma is not widely recognized. We describe the clinicopathological and immunohistochemical features of lymph node lesions from nine such patients which pose serious problems of differential diagnosis from low-grade peripheral T-cell lymphoma. There were three males and six females with ages ranging from 21 to 44 years (median 25 years).…

Focal mycobacterial lymphadenitis following initiation of protease-inhibitor therapy in patients with advanced HIV-1 disease.

BACKGROUND: Inhibitors of HIV-1 protease produce a rapid decrease in plasma HIV-1 RNA, with concomitant increases in CD4 T-helper lymphocyte … for culture and pathology in four patients. FINDINGS: Lymph-node biopsy samples showed that focal lymphadenitis after initiation of indinavir resulted from unsuspected local or disseminated Mycobacterium avium complex (MAC) infection. The prominent inflammatory response to previously subclinical MAC infection was associated with leucocytosis in all patients a …

Recurrent Paradoxical Reactions in Cervical Tuberculous Lymphadenitis During and after Antitubercular Therapy in an Immunocompetent Patient.

Paradoxical reaction (PR) in lymph node tuberculosis (LNTB) is characterized by the worsening of existing lesions or the appearance of new lesions after an initial favourable response to antitubercular therapy (ATT). Although frequently reported in HIV-infected individuals, it can also occur in immunocompetent patients and may be mistaken for treatment failure or disease relapse. We report a 25-year-old immunocompetent woman with microbiologically confirmed cervical LNTB who developed recurrent lymph node enlargement during ATT and again after completing treatment. The diagnosis was establishe

Unilateral pulmonary miliary tuberculosis with abdominal tubercular lymphadenitis in an apparently immunocompetent adult: A case report.

Atypical forms of tuberculosis (TB) are gaining recognition, challenging traditional diagnostic criteria classically linked to chronic cough, lethargy, anorexia, evening pyrexia, and weight loss in immunocompromised patients. We present the case of a 23-year-old immunocompetent man from rural central India who developed unilateral pulmonary miliary TB with concurrent abdominal TB lymphadenitis. The patient presented with a two-month history of chronic cough and diffuse abdominal pain. Initial chest radiograph revealed unilateral, right-sided miliary opacities, while abdominal ultrasonography i…

A rare co-existence of histiocytic necrotizing lymphadenitis with metastatic papillary thyroid carcinoma and review of the literature.

Histiocytic necrotizing lymphadenitis (HNL) is a benign, self-limiting disease that is rare clinically. The coexistence of HNL and tumor is rarer. We report a male patient who was preoperatively diagnosed with papillary thyroid carcinoma with cervical lymph nodes metastasis, and the postoperative pathological examination showed histiocytic necrotizing lymphadenitis combined with metastatic papillary thyroid carcinoma in the same single lymph node. More interestingly, Epstein‒Barr virus was positive in these lymph nodes by in situ hybridization. This may suggest a trigger for the coexistence of…

Purulent Streptococcus intermedius Pericarditis in the Setting of Histoplasma Mediastinal Lymphadenitis: A Case Report and Literature Review.

Purulent pericarditis is a rare and potentially life-threatening condition characterized by infection of the pericardial space. We describe a case of purulent bacterial pericarditis in a 41-year-old male with no significant medical or surgical history who had concomitant pulmonary Histoplasma infection. Streptococcus intermedius was the bacteria directly responsible for the pericardial infection, though co-infection with histoplasmosis likely predisposed him to develop purulent pericarditis. We hypothesize histoplasmosis caused mediastinal lymphadenopathy, facilitating contact between a necrot
