The patients of the ENT clinic at the Gabriel Ture hospital (Bomako, Mali) suffering from otitis media purulenta (96 patients under 70) were examined for pathogenetic agents in their purulent exudate. The infection was found in 96.9% of the samples. Monoflora dominated in 74%, mixed flora in 26% of the examinees. The patients received antibiotics. The antibioticograms proved high effectiveness of gentamycin, colistin, sisomicin and cefatoxin in the treatment of acute and chronic otitis media purulenta.

A randomized double-blind study, in order to evaluate effectiveness and tolerance of albendazole, was carried out in 186 african subjects presenting intestinal nematode infections in Mali and Senegal. The drug, well tolerated, given as a single dose of 0.4 g (adolescents and adults) or 0.2 g (children) is curative in more than 90% for ascariasis. For ancylostomiasis and trichuriasis, the effectiveness is less satisfactory in children, probably due to insufficient dosage in this age group.

Leprosy is generally revealed by cutaneous lesions often associated to nerve impairment. Rarely, it may be revealed by polyarthritis. The diagnosis, often delayed in the cutaneous-nevritic form because of the low prevalence of the disease in metropolitan France, is very difficult in case of rheumatic presentation. We report the case of a 28 year-old woman from Mali, who was diagnosed with lepromatous borderline leprosy with reversal reaction occurring in the postpartum as she presented with polyarthritis and skin lesions.

We describe a Schistosoma haematobium infection with asymptomatic eosinophilia, persistently negative urine microscopy, and late seroconversion (7.5 months) in a traveler returning from Mali. After initial negative parasitological tests, travel history led to diagnostic cystoscopy, allowing final diagnosis with urine microscopy after the bladder biopsy. The patient was successfully treated with praziquantel. Difficulties in making the diagnosis of schistosomiasis in asymptomatic returning travelers are discussed; we propose a trial treatment in these cases.

We identified a family in Mali with two sisters affected by spastic paraplegia. In addition to spasticity and weakness of the lower limbs, the patients had marked atrophy of the distal upper extremities. Homozygosity mapping using single nucleotide polymorphism arrays showed that the sisters shared a region of extended homozygosity at chromosome 19p13.11-q12 that was not shared by controls. These findings indicate a clinically and genetically distinct form of hereditary spastic paraplegia with amyotrophy, designated SPG43.

Leptosphaeria tompkinsii is a dematiaceous fungus which is rarely reported as an agent of black-grain mycetoma. We present a case involving a mycetoma of the hand of a former farmer from Mali, West Africa, who has been a resident in France for 27 years. The patient was successfully treated with surgery and the use of oral itraconazole for 6 months. Species identification was based on sexual reproductive structures observed on potato-carrot agar media and the use of internal transcribed spacer sequencing.

Urinary schistosomiasis is an infection caused by parasite, Schistosoma haematobium. Squistosomiasis is an endemic disease in Africa and Middle East. We are presenting a case of a young immigrant male from Mali that came to our clinic with hematuria and miccional irritative syndrome during a year. Parasitological study reported Schimosoma's eggs and ecography showed a possible vesical newformation. After RTU, anatomopatological study confirms the presence of a vesical esquistosomiasis. Now pacient is asyntomatic after he was treated with Praziquantel.

A brucelian sacro-iliitis is reported in a 33-year-old West African man recently arrived from Mali. Thus, clinical, biological, radiological, and isotopic aspects could be precisely observed, and were found very similar to brucelian sacroiliitis in Europeans. Major information is required about the interest of technetium scintigraphy. The treatment included Rifampicin, in association with Doxycyclin and the theoretical bases of this choice are explained. Tolerance was good and a complete recovery was obtained after three months.

Detailed epidemiological mapping of three isolated foci of hyperendemic blinding onchocerciasis was undertaken in three West African countries as part of community trials of ivermectin, a new microfilaricide for the treatment of human onchocerciasis. The results show that the geographical distribution of the prevalence and intensity of onchocerciasis infection in the community can be very different from what was expected on the basis of demographic and entomological information.
