Reframing TB Care: A Perspective on Multimorbidity-Centered Care for People with TB.

Tuberculosis (TB) rarely occurs in isolation; most people with TB experience multiple coexisting conditions, including HIV, diabetes, undernutrition, depression, and substance use disorders, which worsen disease severity and compromise treatment outcomes. Although the World Health Organization has issued disease-specific guidance for managing key comorbidities, TB care remains largely siloed and poorly equipped to address the growing burden of multimorbidity, particularly in African health systems. In this perspective article, we propose a phased framework for multimorbidity-centered TB care.

Promoting Diversity in Cardiac Rehabilitation Programs at Community Hospitals.

BACKGROUND: Although comprehensive cardiac rehabilitation (CR) is an effective treatment for cardiovascular diseases, its implementation in Japan remains insufficient. Following the 2022 Basic Act on Cardiovascular Disease Prevention, each prefecture has established specific targets. METHODS AND RESULTS: We report our experience implementing and managing CR programs across hospitals of different sizes in Yamaguchi Prefecture, Japan, including Tokuyama Medical Association Hospital (initiated in 2019), Mitajiri Hospital (initiated in 2022), and Yamaguchi Prefectural Grand Medical Center. Assessing the Link between Maternal Transport Modes and Childhood Mortality in Nigeria.

OBJECTIVES: The study explored the association between maternal transport modes and childhood mortalities in Nigeria. METHOD: Utilizing data and definitions from the 2018 Nigeria Demographic and Health Survey report, the ten-year early mortality rates of the five childhood mortalities and the percentage of live births in the 5 years before the survey, transported by eight identified means of transportation, were statistically correlated for each of Nigeria's 36 states and the federal capital territory (FCT) in the R environment at a significance level of α < 0.05. RESULTS: In the spatial dis

Prevalence and risk factors for extended-spectrum β-lactamase or AmpC-producing Escherichia coli in organic dairy herds in the Netherlands.

Extended-spectrum β-lactamase and AmpC-producing Escherichia coli (ESBL/AmpC) are an emerging problem and are hypothesized to be associated with antimicrobial use (AMU), and more specifically with the use of third- and fourth-generation cephalosporins. Whether ESBL/AmpC also occur in organic dairy herds, which have restricted AMU, is not known. Additionally, it is unknown whether, in addition to restricted AMU, other factors in organic herd management are associated with ESBL/AmpC herd status. The aim of this study was to estimate the prevalence of ESBL/AmpC in organic dairy herds in the Nethe

Consilient Care: When Physical and Mental Health Issues Coexist.

The intricate relationship between physical and mental health presents a significant challenge in modern healthcare. This editorial introduces the concept of consilient care, an integrated, interdisciplinary approach that bridges mental and physical health services to improve patient outcomes. It explores the bidirectional impact of mental disorders on physical health and vice versa, emphasizing the need for holistic, patient-centered strategies. The discussion spans emergency and internal medicine, highlighting the diagnostic complexities of overlapping symptoms and the importance of standard

Coordination of Care for Expiratory Central Airway Collapse: A Structured Process for a Multifaceted Disease.

Common respiratory symptoms, including dyspnea, cough, sputum production, and recurrent infections, frequently remain without a clear cause and may be the result of expiratory central airway collapse (ECAC). Establishing the diagnosis and appropriate treatment plan for patients with ECAC is challenging and benefits from a multidisciplinary approach. A coordinator role is crucial in this process to ensure optimal patient-centered outcomes. We describe the coordination of care in the process of diagnosing and treating ECAC. The coordinator leads the organization of the multiple services involved

[Work-related musculoskeletal disorders].

Work-related musculoskeletal disorders. Upper-limb musculoskeletal disorders are painful conditions related to the overuse of periarticular soft tissues. The main musculoskeletal disorders are rotator cuff tendinopathy, epicondylalgia, carpal tunnel syndrome and non-specific pain. Musculoskeletal disorders affect millions of European workers and represent the first problem of health at work in the European Union. They testify to the intensification of working conditions affecting a growing number of workers in the industry and services sectors. Their impact in terms of pain and disruption of c

Problem substance use among depressed patients in managed primary care.

This study identifies characteristics associated with problem substance use among 1,187 patients with either depressive symptoms (44%) or depressive disorders (56%) in primary care clinics of six managed care organizations. Sedative misuse (reported by 14% of all patients) was associated with greater wealth, social phobia, and misuse of prescription opioids. Cannabis use (11%) was associated with younger age, male gender, single marital status, white ethnicity, less education, recurrent depression, agoraphobia, and hazardous alcohol use. Hazardous drinking (11%) was significantly associated wi

Geriatric referrals to a psychiatric consultation-liaison service.

Geriatric patients constitute a significant proportion of referrals to a psychiatric consultation-liaison service, accounting for 29% of referrals in the present study. Organic mental disorders, the most common psychiatric diagnosis, were identified in 51% of geriatric referrals. The next most common psychiatric conditions were affective disorders and adjustment disorders, each of which were diagnosed in 17% of geriatric referrals. Transfers to in-patient psychiatry were unusual and the most common recommendations were for psychotropic medications, further medical investigations, and for psych

Reframing spinal cord injury rehabilitation in fragile contexts: a multi-level systems framework for Lebanon.

Background: Spinal cord injury (SCI) requires long-term, multidisciplinary rehabilitation to optimize functioning and participation. In fragile health systems, however, rehabilitation services are often fragmented, under-resourced, and inconsistently accessible. This Current Debate paper examines SCI rehabilitation in Lebanon as an example of how health system fragility constrains rehabilitation delivery and disability inclusion. Methods: Using an International Classification of Functioning, Disability and Health (ICF)-informed approach, we developed a multi-level conceptual framework through s

World Organisation for Animal Health: strengthening Veterinary Services for effective One Health collaboration.

To effectively reduce health risks at the animal-human-ecosystems interface, a One Health strategy is crucially important to create strong national and regional animal health systems that are well coordinated with strong public health systems. Animal diseases, particularly those caused by new and emerging zoonotic pathogens, must be effectively controlled at their source to reduce their potentially devastating impact upon both animal and human health. As the international organisation responsible for developing standards, guidelines and recommendations for animal health, the World Organisation

How Community-Based Organizations View and Use Data: Practices, Challenges, and Opportunities.

This analysis aimed to define the data needs of community-based organizations (CBOs) after the COVID-19 pandemic. Through a mixed-methods explanatory sequential design during the summer/fall of 2023, 51 CBOs completed surveys and 24 participated in subsequent interviews. The CBOs provided social services to Black, Latinx, Asian, and/or low-income communities in the Chicago region. They varied in size, funding sources, and focus areas. Results showed that all used data, and 82.4% used population-level data. Data use expansion was limited by funding, staffing, data management systems, and skills

Implementation and fidelity assessment of the NAVIGATE treatment program for first episode psychosis in a multi-site study.

The NAVIGATE program was developed for the Recovery After Initial Schizophrenia Episode-Early Treatment Program (RAISE-ETP) study, which compared NAVIGATE to usual Community Care in a cluster randomized design involving 34 sites and 404 patients. This article describes the approach to training and implementing the NAVIGATE program at the 17 sites (including 134 practitioners) randomized to provide it, and to evaluating the fidelity of service delivery to the NAVIGATE model. Fidelity was evaluated to five different components of the program, all of which were standardized in manuals in advance

The Potential for Community-Directed Interventions: Reaching Underserved Populations in Africa.

Community-directed interventions (CDIs) have the potential for fulfilling the promise of primary health care by reaching underserved populations in various settings. CDI has been successfully tested by expanding access to additional health services like malaria case management through local effort in communities where ivermectin distribution is ongoing. The question remains whether the CDI approach has potential in communities that do not have a foundation of community-directed treatment with ivermectin. The UNICEF/UNDP/World Bank/WHO Special Program of Research and Training in Tropical Diseas

Rhizopus microsporus in wooden tongue depressors: a major threat or minor inconvenience?

The investigation and management of an apparent outbreak of Rhizopus spp. in a London paediatric referral centre between September 1995 and April 1996 is described. The organism was identified in microbiological surveillance samples from 23 patients nursed in four hospital areas. Investigations revealed the presence of the organism in spatulae from all ward areas investigated and from closed boxed containers held in the central hospital stores obtained from a new supplier. In contrast, culture of spatulae from the initial supplier failed to yield any fungal isolates. The incident was reported

Emergency medicine expeditions to the developing world: the Loma Linda University experience in Papua New Guinea.

Emergency physicians (EPs) are increasingly participating in international medicine in regions that are chronically medically underserved. In August 1994, a ten-member emergency medicine team from the Loma Linda University School of Medicine staffed a 70-bed bush hospital in the primitive highlands of Papua New Guinea, providing both outpatient and inpatient medical care. Typhoid fever, malaria, polio, and numerous other infectious diseases were encountered. Rampant local tribal warfare resulted in regular penetrating injuries from arrows, spears, and machetes. The expedition was judged highly

Health-Literate Healthcare Organization Attributes and Perceived Workload Among Nurses and Nursing Coordinators.

BACKGROUND: Health-literate healthcare organizations (HLHOs) have been associated with improved clinical outcomes and quality of care for both patients and healthcare services, through the implementation of the "Ten Attributes of HLHOs" which support individuals in navigating, understanding, and using health information and services. Nurses, in both clinical and managerial roles, are crucial in supporting HLHO initiatives. However, a high level of perceived workload may influence their engagement with health literacy-related initiatives. PURPOSE: To assess nurses' and nursing coordinators' pe

Transfusion in the Spanish Civil War: Supply and demand, the role of the "blood transfusion officer" and British planning for the outbreak of the Second World War.

During the First World War blood transfusion progressed from being a rarely used treatment to a major component of the resuscitation of exsanguinated casualties, relying on local donation and availability of medical expertise. In the 1920s and early 1930s, clinical use of transfusion gradually increased supported by growing civilian, often volunteer, blood donor systems. The Spanish Civil War (1936-1939) introduced the first systematic use of aerial bombardment to intimidate the civilian population and to destroy infrastructure; it also saw more mobile battlefronts, replacing the relatively st

[Discussions Concerning the Generalist-Specialist Combination Management Model of Chronic Kidney Disease].

In recent years, the effective management of patients with chronic kidney disease (CKD) is gaining growing attention. In 2014, our hospital established the CKD generalist-specialist combination management model, which incorporates a set of CKD management processes. The generalist component incorporates the following, general practitioners from 6 community health centers in the surrounding areas (with about 650 000 permanent residents in the region) joining hands, setting up a management team composed of doctors and nurses, and formulating management protocols for patient follow-up, patient rec
