Intraluminal mobile thrombus of the descending aorta are rare disorders. They are at high risk for peripheral embolism and therefore indication for treatment is mandatory. We report on a 54-year-old patient with peripheral arterial embolization who was treated by surgical thrombus removement by thoracotomy and staged peripheral bypass grafting. New diagnostic tools are presented, therapy and prognosis are discussed.

Jejunogastric intussusception is a rare complication of gastric surgery. It usually presents with abdominal pain, nausea, vomiting, and hematemesis. A history of gastric surgery can help in making an accurate diagnosis. An early diagnosis and urgent surgical intervention is mandatory. We herein report two cases of patients with jejunogastric intussusception who presented with acute abdomen and hematemesis.

The literature of infections after colo-rectal surgery is reviewed, including the influence of mechanical preparation and the prophylactic use of various antibiotic combinations, orally or parenterally. Is is concluded that appropriate prophylaxis is mandatory. Third generation cephalosporins singly or in combination seem to give the best results in the majority of reports. They are economically justified as prophylaxis with antibiotics decreases hospital costs.

The condition is probably more common than previously reported and embryology and clinical implications are briefly discussed. Nodules consisting of aberrant adrenal tissue should be removed whenever incidentally detected, but a routine search for them is not mandatory.

Unicondylar fractures of the lower end of the femur are uncommon injuries that usually occur in the saggital plane 2. Coronal (tangential) plane fractures, first described by Hoffa in 1904, are unusual.(1,2, 3,4) We report our experience with three cases and the mechanism of injury is discussed. OBJECTIVE: We report three cases of a rare form of cavernoma developed within the wall of the common bile duct.
CONCLUSION: To our knowledge, this kind of portal cavernoma has not been described in the literature. Because the cavernoma may be easily confused with other causes of bile duct wall thickening, color Doppler sonography is mandatory for a correct diagnosis.

An unusual postoperative complication of percutaneous thermocoagulation of the gasserian ganglion is reported. Computed tomography and magnetic resonance imaging of the brain disclosed an intracerebral lesion following the surgical procedure. Some rare abnormalities of the skull base could increase the risk of such complications. A meticulous surgical technique with fluoroscopic and neurophysiological control is mandatory in any percutaneous procedure on the trigeminal ganglion.

Lyme disease (LD) is rare in British Columbia (BC) and, despite being a reportable condition since 1994, may be underreported. Here we review all provincial laboratory and clinical databases to determine the number of LD cases reported in BC from 1997 to 2008. Myasthenia gravis (MG) is a well-known acquired autoimmune neuromuscular disorder. Patients with MG have a higher incidence of autoimmune disease than the normal population. MG is frequently associated with autoimmune thyroid disease, the most common of which is thyrotoxicosis. Associated hypothyroidism is not common, and the central (pituitary) origin, to our knowledge, has not yet been reported. We report an MG patient with thymoma that coexisted with central hypothyroidism, the correction of which is mandatory and significant to achieve remission.
