Metastatic squamous cell carcinoma of the tonsil presenting as otorrhea: a case report.

We describe the case of a 52-year-old man with a history of squamous cell carcinoma (SCC) of the tonsil who presented with right subacute otalgia and otorrhea. Dedicated computed tomography of the temporal bones showed opacification within the mastoid process with destruction of bony mastoid septations consistent with coalescent mastoiditis. Preoperative imaging showed no destruction or expansion of the bony eustachian tube that would indicate that a direct spread had occurred. An urgent cortical mastoidectomy was performed. Intraoperatively, a friable white mass surrounded with purulence and …

[Carcinoid of the middle ear. A case report].

The case of a 47-year-old patient is described with a carcinoid of the middle ear. Initial symptoms were hearing impairment, feeling of pressure and dizziness. Mastoidectomy was carried out for mastoid shadowing with space encroachment in the auditory canal. The histological examination initially revealed an atypical cholesteatoma and the subsequent immunohistochemical investigation revealed a carcinoid. A radical excavation with complete excision of the tumor and tympanoplasty was carried out. A carcinoid of the middle ear is definitely a rare finding and the primary treatment is complete sur…

Hypertrophic chronic pachymeningitis associated with chronic otitis media and mastoiditis.

We describe the case history of a 70-year-old female patient presenting with bilateral hearing disturbance, facial paralysis, and vertigo. Radiological tests of temporal bone revealed soft tissue in the mastoid and tympanic cavities, and T1 weighted MRI revealed prominent Gd enhancement of the middle skull basal meninges. Middle ear inflammation appeared to induce pachymeningitis and to exacerbate associated symptoms, leading to a decline in the patient's overall condition. Bilateral mastoidectomies were effective in improving her general condition. Her hearing improved only on the right side

Tympanoplasty in children. The Boston Children's Hospital experience.

Considerable controversy surrounds the subject of tympanoplasty in children. Conflicting opinions about the indications, patient selection, timing, and technique of surgery are supported by various published series of cases. The records of 64 consecutive tympanoplasty procedures performed at the Boston (Mass) Children's Hospital over a recent 6-year period were reviewed. The study was limited to cases of repair of uncomplicated perforation of pars tensa that did not require ossiculoplasty or mastoidectomy. Surgery was successful in 73% of cases. A number of factors that are postulated to affec…

[Anatomic characteristics of the mastoid in children. Pneumatization of the subfacial region].

Three specific features of the mastoid in the pediatric population appeared to us to be important in view of our per-operative findings on mastoidectomy for chronic suppurative otitis, cholesteatoma or middle ear effusion with chronic persistent mastoiditis in the older child. These included the position of the facial nerve, the superficial nature of its third part being well known, but also the existence of the Körner septum and especially the pneumatization of the sub-facial region which explains the similarity between certain infectious lesions of the retro-tympanic structures and infectiou…

Comparative study of cholesteatoma in paediatric and adult patients.

BACKGROUND: The aggressiveness of paediatric cholesteatoma has long been a matter of debate. While much of the evidence is substantiated by data from the Western world, it is further limited by the retrospective nature of most studies. Therefore, this paper presents a comparative analysis of various characteristics of cholesteatoma between paediatric and adult populations seen at our centre. METHODS: A total of 50 patients (25 adults and 25 paediatric) with clinical diagnosis of chronic suppurative otitis media with cholesteatoma underwent canal wall down mastoidectomy over a period of two ye…

Spontaneous Congenital Perilabyrinthine Cerebrospinal Fluid Fistulas.

OBJECTIVES: To report a recalcitrant spontaneous cerebrospinal fluid (CSF) fistula arising from multiple, anatomically-linked lateral skull base defects, and to review the available literature to determine optimal techniques for operative repair of congenital CSF fistulae. METHODS: A patient with recurrent episodes of otologic meningitis was found to have a patent tympanomeningeal fissure, also known as a Hyrtl's fissure, and internal auditory canal (IAC) diverticulum that communicated with the jugular bulb. A systematic review of the literature characterized all reports of spontaneous congen

Brain abscess following acute otitis media in a nasopharyngeal carcinoma survivor: a case report.

INTRODUCTION: Although rare, brain abscesses secondary to acute otitis media (AOM) may pose serious risks, particularly among adolescent and young adult (AYA) survivors of head and neck cancer. PRESENTATION OF CASE: A 29-year-old woman who survived AYA nasopharyngeal carcinoma following chemoradiotherapy developed an AOM complicated by a left temporal lobe brain abscess and motor aphasia. After undergoing neurosurgical drainage and receiving intravenous antibiotics, she achieved full recovery without mastoidectomy. DISCUSSION: Eustachian-tube dysfunction resulting from prior radiotherapy and…

[Expansion of an ceruminous adenoma into the middle ear].

A 37-year-old female presented for surgery with central perforation of the eardrum with granulation. Mastoidectomy had been performed 18 years ago following chronic mastoiditis. As the clinical picture now suggested a suspected cholesteatoma, radiological imaging was performed. The CT scan revealed specification of the mastoid and the tympanic cavity. In addition, MRI scan showed signal enhancement in the same areas. However, the suspected cholesteatoma could not be confirmed intraoperatively. Pathohistology revealed a ceruminal gland adenoma. They are a rare phenomenon and should be distingui…

Tympanomastoidectomy for chronic otitis media without cholesteatoma.

A series of 221 ears with chronic suppurative otitis media without cholesteatoma is presented--84% of the cases were treated using one-stage tympanomastoidectomy and 15% underwent cortical mastoidectomy with planned second-stage tympanoplasty. Mean follow-up period was 6.3 years. Control of infection succeeded in 92% after the primary operation. Failures were most common in ears infected with Pseudomonas aeruginosa. Postoperative cholesteatoma developed in 5 ears (2.2%). Hearing results were unsatisfactory; a postoperative air-bone gap within 20 dB was achieved in only 62%. In revision operati…
