Addressing inequities in maternal health through postpartum nurse home visiting: a mixed methods evaluation.

BACKGROUND: The First Five Yolo Welcome Baby (WB) program, initiated in 2021, is a nurse home visiting initiative funded by local and federal dollars. It aims to address postpartum health in low-income, uninsured, or Medi-Cal insured families in Yolo County, California. Amid persisting inequities in maternal health, especially in low-income and historically marginalized communities, WB was designed to overcome barriers to postpartum care access and provide essential support for social, emotional, and physical wellbeing for mothers and their families. The purpose of this study was to evaluate t…

Maternal Health Infrastructure Investments in Zambia: Leveraging Geospatial Evidence to Inform Planning and Prioritization.

In many low- and middle-income countries (LMICs), inequities in maternal health outcomes are linked to disparities in spatial access to essential health services. In Zambia, significant portions of the rural population reside beyond 5 kilometers from the nearest health facility. This contributes to the "three delays" of maternal mortality: delay to seek care, delay to reach a health facility, and delay to receive care from a skilled health provider. Consequently, at 187 per 100,000 live births, Zambia's maternal mortality ratio is above the global target of fewer than 70. This paper describes …

Nutrition education and common maternal health and dietary outcomes during pregnancy: A scoping review.

This review aimed to summarise the nutrition education programs and interventions that have sought to improve maternal health outcomes. Pregnancy is often considered a "teachable moment" when mothers may be motivated to adopt positive behavioural changes, including improving their nutrition habits. Pregnancy nutrition education is the provision of information and guidance on optimal nutritional practices that aim to support a healthy pregnancy. This scoping review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Eight electronic databases wer…

L-Citrulline in Maternal-Fetal and Neonatal Health: Metabolic Mechanisms and Emerging Therapeutic Applications.

L-citrulline is a non-protein amino acid with critical roles in perinatal and neonatal physiology through the intestinal-renal arginine-citrulline axis and nitric oxide (NO) production. During pregnancy, L-citrulline supports placental angiogenesis, vascular adaptation, and fetal growth through augmentation of arginine availability and endothelial NO production. In neonates, particularly preterm infants, developmental immaturity of citrulline and arginine synthesis contributes to hypoargininemia and may increase susceptibility to necrotizing enterocolitis, bronchopulmonary dysplasia with pulmo

Heat and its effects on maternal and neonatal health care: Evidence from Burkina Faso.

OBJECTIVES: Climate change has a wide range of adverse consequences for human health. The effects of extreme weather on maternal and newborn health, as well as the availability and quality of health services, remain under-researched in the sub-Saharan region. The aim of this study is to examine the effects of heat on pregnant women's use of health services, the health facility working environment and the impact of heat on the quality of care in Burkina Faso, a limited resource country. STUDY DESIGN: We adopted a qualitative approach to address the research objectives. METHODS: This study was

Perinatal depression, maternal thyroid status and fetus/infant health and development: A systematic review.

BACKGROUND: Thyroid hormones are known to influence both maternal depression and child developmental outcomes, while maternal depression independently affects child outcomes. The potential interaction between thyroid dysfunction and depression in shaping child development remains insufficiently explored. The present study addresses such interplay. METHODS: Following PRISMA 2020 and JBI guidelines, three databases were searched through December 2025 for primary studies on maternal thyroid status, perinatal depression, and child development. Risk of bias (RoB) was assessed using validated tools

Maternal supraphysiological hypercholesterolemia and its adverse impact on transgenerational cardiometabolic health: a literature review.

AIMS: Among the risk factors leading to cardiovascular disease (CVD), hypercholesterolemia stands out as a key driver of vascular dysfunction and the development of atherosclerotic CVD. This review is aimed to highlight the emergent evidence showing that maternal supraphysiological hypercholesterolemia (MSPH) is a key risk factor for transgenerational CVD risk and to advocate for the development of strategies for the early prediction and prevention of MSPH. DATA SYNTHESIS: Increasing evidence suggests that an individual's lifetime CVD risk may be modified by in utero exposure. However, the co

Correlation between maternal infections and neonatal health: Evidence from Haji Adam Malik Hospital, Medan.

Maternal infections remain a critical but under-addressed determinant of neonatal health in Indonesia, yet hospital-based evidence is limited. This retrospective study reviewed 206 maternal records at Haji Adam Malik Hospital, Medan (2015-2018) to examine the prevalence of HIV, hepatitis B, and syphilis and their associations with neonatal outcomes. HIV prevalence was 6.8% (14/206), substantially higher than national estimates, while hepatitis B was detected in 2.4% (5/206) and no syphilis cases were identified. Overall, one-third of infants (33%) were born with low birth weight and 9.2% had s

Preterm Delivery and Postpartum Substance Use: Implications for Maternal Mental Health in Obstetric Care.

Introduction Preterm delivery is an obstetric complication linked to an increased risk of postpartum depression (PPD), which often goes undiagnosed. Individuals with PPD may self-medicate to manage depressive symptoms. While postpartum substance use is well-documented, few studies have assessed whether preterm delivery is associated with new-onset substance use. A greater understanding of birth trauma as a factor for new-onset substance use in postpartum women can broaden our understanding of postpartum mood disorders and propel greater inquiry into mental health care in peri- and postpartum w

Addressing Maternal Health Disparities: Building a Novel Two-Generation Approach to Comprehensive Postpartum Care.

The United States is facing a maternal health crisis with increasing rates of severe maternal morbidity and mortality. To improve maternal health and promote health equity, the authors developed a novel 2-generation model of postpartum and pediatric care. This article describes the Two-Generation Clinic (Two-Gen) and model of care. The model combines a dyadic strategy for simultaneous maternal and pediatric care with the collaborative care model in which seamless primary and behavioral health care are delivered to address the physical health, behavioral health, and social service needs of fami…
