Obstructive sleep apnea and severe maternal-infant morbidity/mortality in the United States, 1998-2009.

STUDY OBJECTIVES: A recent trend in increasing rates of severe maternal morbidity and mortality despite quality improvements has been noted. The goal of this study is to estimate the national prevalence of obstructive sleep apnea (OSA) in pregnant women and examine associations between OSA and pregnancy-related morbidities, including in-hospital maternal mortality. DESIGN: A retrospective, cross-sectional analysis. SETTING: A nationally representative sample of maternal hospital discharges from 1998-2009. PATIENTS OR PARTICIPANTS: The analytic sample included 55,781,965 pregnancy-related in…

Predictive factors of perinatal mortality in transfused fetuses due to maternal alloimmunization: what really matters?

INTRODUCTION: Alloimmunization is the main cause of fetal anemia. There are not many consistent analyses associating antenatal parameters to perinatal mortality in transfused fetuses due to maternal alloimmunization. The study aimed to determine the prognostic variables related to perinatal death. MATERIAL AND METHODS: A cohort study analyzed 128 fetuses treated with intrauterine transfusion (IUT), until the early neonatal period. Perinatal mortality was associated with prognostic conditions related to prematurity, severity of fetal anemia and IUT procedure by univariated logistic regression.

[Safe illegal abortion would make an important contribution to decreasing maternal mortality and female suffering].

Unsafe abortion is a major public health concern in many developing countries, contributing to a substantial proportion of maternal deaths. Increased legal access to abortion services is associated with improvement in mortality and morbidity. Safe illegal abortion by means of drugs is another possible way to prevent this unnecessary harm to women. The Dutch government, however, is now proposing legal changes that will diminish the access to medical abortion by general practitioners.

Maternal mortality rates in the last eight years: A university hospital-based study from Turkey.

OBJECTIVE: To analyze the cause of changing maternal mortality ratios (MMRs) in a tertiary women's health center in Turkey in the last eight years. MATERIALS AND METHODS: Charts of patients seen between 1998 and 2005 were retrospectively reviewed. Statistical analysis was performed using the Chi-square test. The results were accepted to be significant when the p value was <0.05. RESULTS: During this period, 27 pregnancy-related deaths were identified via hospital death records. The MMR was found to have decreased in rate by approximately 50% from 822.2/100,000 live births in the previous rep…

Maternal mortality at B P Koirala Institute of Health Sciences, Nepal: review of 6 years.

This study aimed to find out the maternal mortality rate (MMR) in mothers dying after admission to the gynaecology and obstetrics ward of BP Koirala Institute of Health Sciences, Nepal, and to analyse the most common causative factor. There was a total of 58 mortality cases in 6 years. The MMR in 1997--1998 was 12/100,000 live births, in 1998--1999 144/100,000, in 1999--2000 294/100,000, in 2000--2001 450/100,000, in 2001--2002 546/100,000 and in 2002--2003 400/100,000. The most common cause of death was septic-induced abortion and its complications, followed by eclampsia and puerperal sepsis.

Neonatal morbidity and mortality associated with maternal haemolysis elevated liver enzymes and low platelets syndrome.

UNLABELLED: To compare the impact of maternal haemolysis, elevated liver enzymes and low platelets (HELLP) syndrome, uncomplicated hypertension in pregnancy (HIP), and no hypertension (controls) on neonatal morbidity and mortality, 108 infants were matched with respect to gestational age, date of birth, and gender. The HELLP group infants had more grade 3 and 4 respiratory distress syndromes (36%) than the HIP group (19%) or controls (11%). Cardiovascular instability (arterial hypotension, volume resuscitation) was significantly more common in HELLP neonates (20% and 31%) than in HIP infants (

Putting the "M" back in maternal-fetal medicine: A 5-year report card on a collaborative effort to address maternal morbidity and mortality in the United States.

The Centers for Disease Control and Prevention have demonstrated continuous increased risk for maternal mortality and severe morbidity with racial disparities among non-Hispanic black women an important contributing factor. More than 50,000 women experienced severe maternal morbidity in 2014, with a mortality rate of 18.0 per 100,000, higher than in many other developed countries. In 2012, the first "Putting the 'M' back in Maternal-Fetal Medicine" session was held at the Society for Maternal-Fetal Medicine's (SMFM) Annual Meeting. With the realization that rising risk for severe maternal morb…

Tracking the impact of maternal health service coverage on maternal mortality across 137 low- and middle-income countries in the progress toward Sustainable Development Goals.

BACKGROUND: Achieving Sustainable Development Goal (SDG) 3, particularly targets 3.1 (reducing the global maternal mortality ratio (MMR) to less than 70 per 100,000 live births) and 3.8 (universal health coverage), requires sustained reductions in maternal mortality and equitable access to essential maternal health services (MHS). Despite global progress, large disparities persist. Low- and middle-income countries (LMICs) account for more than 90% of maternal deaths worldwide, largely reflecting gaps in access to essential MHS. However, multi-country evidence examining how improvements in MHS …

Developing Tools to Report Racism in Maternal Health for the CDC Maternal Mortality Review Information Application (MMRIA): Findings from the MMRIA Racism & Discrimination Working Group.

PURPOSE: The purpose of this report from the field is to describe the process by which an multidisciplinary workgroup, selected by the CDC Foundation in partnership with maternal health experts, developed a definition of racism that would be specifically appropriate for inclusion on the Maternal Mortality Review Information Application (MMRIA) form. DESCRIPTION: In the United States Black women are nearly 4 times more likely to experience a pregnancy-related death. Recent evidence points to racism as a fundamental cause of this inequity. Furthermore, the CDC reports that 3 of 5 pregnancy rela…

Survival analysis of maternal, household, environmental, and child-level determinants of under-five mortality in Nigeria.

BACKGROUND: In Africa, Nigeria bears the highest burden of under-five mortality, with an estimated 850,000 under-five deaths from preventable causes reported in 2024. Existing evidence has not applied a time to event approach using recent nationwide data to examine factors associated with child mortality in this high-burden setting. The current study employed a time to event approach to examine factors affecting under-five mortality across a 59-month life course. METHODS: Retrospective data from the 2024 Nigeria Demographic and Health Survey was used. We analyzed data for 27,604 under-fives.

Maternal socioeconomic status and neonatal mortality in OECD countries: a systematic review and meta-analysis protocol.

INTRODUCTION: Socioeconomic inequalities in neonatal mortality are observed globally but gaps remain in the evidence from current reviews, specifically: a wider range of socioeconomic indicators at the individual, household and area level than previous reviews, and alternative time frames to define neonatal mortality. Thus, a comprehensive updated review of the literature is required, focusing on multiple measures of socioeconomic status and alternative time frames, to assess the relationship between maternal socioeconomic status and neonatal mortality in high-income countries. METHODS AND AN

Birth weight of term born offspring in relation to long-term maternal cardiovascular morbidity and mortality.

UNLABELLED: Previous studies have identified associations between offspring birth weight (BW) and future maternal cardiovascular health, but many studies lack sufficient adjustment for confounders such as pregnancy complications, gestational age, maternal body mass index (BMI), and smoking. This study aimed to assess whether the association between term offspring BW and maternal cardiovascular risk is independent of these factors. We used data from the Copenhagen Perinatal Cohort (1959–1961). After excluding women with diabetes, preeclampsia, preterm births, and twin pregnancies, 5,766 women r

Recurrent hypertensive disorders of pregnancy and subsequent maternal coronary heart disease, stroke, and all-cause mortality.

INTRODUCTION: Hypertensive disorders of pregnancy (HDP) complicate 5%-10% of pregancies, to maternal morbidity and mortality. HDP increases the risk of cardiovascular events later in life, but whether repeated HDP episodes confer cumulative risk remains unclear. We aimed to assess the association between recurrent HDP and subsequent maternal coronary heart disease (CHD), stroke, and all-cause mortality. METHODS: This retrospective cohort included women aged 12-49 with two or more singleton live births in South Carolina (2004-2016). Deliveries were evaluated for HDP (preeclampsia, eclampsia, g

Global Burden of Maternal Disorders and Mortality Among Female Individuals Aged 15-49 Years, 1990-2021.

OBJECTIVE: To quantify mortality trends among female individuals of childbearing age (15-49 years) across global regions from 1990 to 2021, systematically evaluating persistent geographic inequities in preventable deaths. METHODS: We analyzed data from the GBD (Global Burden of Diseases, Injuries, and Risk Factors Study) 2021, focusing on five leading causes of maternal mortality: hemorrhage, hypertensive disorders, sepsis and infections, obstructed labor and uterine rupture, and abortion and miscarriage. We calculated estimated annual percent change (APC) in age-standardized incidence rate (…

Preventing maternal morbidity and mortality from preeclampsia and eclampsia particularly in low- and middle-income countries.

Preeclampsia (PE) is a complex heterogeneous disorder with overlapping clinical phenotypes that complicate diagnosis and management. Although several pathophysiological mechanisms have been proposed, placental dysfunction due to inadequate remodelling of uterine spiral arteries leading to mal-perfusion and syncytiotrophoblast stress is recognized as the unifying characteristic of early-onset PE. Placental overgrowth and or premature senescence are probably the causes of late-onset PE. The frequency of PE has increased over the last few decades due to population-wide increases in risk factors v
