Open Sinus Lift Surgery and the Importance of Preoperative Cone-Beam Computed Tomography Scan: A Review.

Open sinus lift surgery is a form of pre-prosthetic surgery for increasing the quality and quantity of bone in the posterior region of the maxilla. Pre-operative assessment of the maxillary sinus is essential for the success of this surgery. PubMed search was carried out in English language literature for open sinus lift surgery and cone-beam computed tomography (CBCT). The results focused on anatomic variants, vascular anatomy, complications, osteotomy/ostectomy window dimensions and thickness of the Schneiderian Membrane. 59 articles were included in this review. Features other than the heig

Maxillary sinus floor augmentation using blood without graft material. Preliminary results in 10 patients.

PURPOSE: The maxillary sinus lift is recognized and stable, and there have been different innovations to optimize the technique. The aim of this study was to investigate the maxillary sinus lift technique with the use of a blood clot and without the use of a bone graft. MATERIALS AND METHODS: Ten patients were recruited for a unilateral sinus lift; patients without sinus pathology or other contraindication were selected. The maxillary sinus was accessed conventionally under local anesthesia followed by an osteotomy and a 1-cm(2) bony window access. The sinus membrane was detached and the wind

Severity and long-term complications of surgical site infections after orthognathic surgery: a retrospective study.

Surgical site infections (SSI) occur in 1.4% to 33.4% of cases after orthognathic surgery. This type of complication is a major concern to surgical teams, but there is no consensus for the prevention and treatment of SSI in orthognathic surgery. The purpose of this descriptive study was to evaluate the severity and the consequences of postoperative infections. The charts of all the patients operated on by the orthognathic surgery team between January 2015 and July 2017 were collected. All types of orthognathic procedures (Le Fort I maxillary osteotomy, bilateral sagittal split mandibular osteo…

Treatment of a class III patient with maxillary retrognathia and amelogenesis imperfecta using a surgery-only orthognathic approach.

Amelogenesis imperfecta (AI) is a rare disorder characterized by structural irregularities of enamel and may be associated with various oral-craniofacial features. The compromised surface of the teeth creates an unsuitable area for the application of orthodontic appliances. The aim of this study is to present the prosthetic rehabilitation of an 18-year-old male patient diagnosed with amelogenesis imperfecta and skeletal Class III malocclusion due to maxillary retrognathia, following a surgery-only approach (SOA) with virtually planned Le Fort I and bilateral sagittal split ramus osteotomies (B

Experience with bone morphogenetic protein-2 and interpositional grafting of edentulous maxillae: a comparison of Le Fort I downfracture to full-arch (horseshoe) segmental osteotomy done in conjunction with sinus floor grafting.

This paper is a retrospective report of the treatment of six patients with severely resorbed maxillae. Patients were treated, based on the amount of maxillary retrognathia, with either a Le Fort I downfracture or a "horseshoe" interpositional sandwich osteotomy, along with sinus elevation. Recombinant human bone morphogenetic protein-2 in an absorbable collagen sponge carrier was used for grafting in all patients, either alone or in combination with other grafting materials. Implants were placed and the patients were restored with fixed prostheses. Both grafting techniques are described, and t

Postero-lateral expansion osteotomy of maxilla. A case report.

The posterior-maxillary osteotomy introduced by Schuchardt was designed to close skeletal anterior open bite. Also, it can be used to reduce a posterior cross bite. But in this latter case, the most important problem is the difficulty in expanding the maxilla because of the relative inelasticity of the palatal mucosa. This paper reports on the result obtained by a surgical expansion technique without orthodontic treatment. The use of a local soft tissue flap is necessary; the anteroposterior part of the palatal incision is placed contralaterally to the bone incision. A palatal appliance is use…

Computer-Assisted Frontofacial Monobloc Advancement and Facial Bipartition for Pfeiffer Syndrome: Surgical Technique.

BACKGROUND: In patients with Pfeiffer syndrome, several corrections are required to correct facial retrusion, maxillary deficiency, or even hypertelorism. The frontofacial monobloc advancement (FFMA) and the facial bipartition (FB) are the gold standard surgeries. We present the correction of this deformity using a simultaneous computer-assisted FFMA and FB. METHODS: The 3-dimensional surgical planning defined the virtual correction and bone-cutting guide in view of the FFMA and FB. Coronal and intraoral approaches were combined to perform the osteotomies. Four internal distractors were also

Treatment of an adult patient with a periodontally compromised skeletal Class II malocclusion.

This article describes the treatment of an adult female with periodontally induced bone loss, excessive gingival display when smiling, vertical maxillary excess, a convex profile, a traumatic deep overbite, and severe lip incompetence. The therapy included four steps: (1) periodontal flap surgery and root planing with regular follow-ups; (2) leveling and aligning of the maxillary and mandibular arch with closure of all spaces, the extraction of mandibular first premolars, and retraction of the mandibular incisors; (3) LeFort I two-segment osteotomy with removal of the second premolars and a 5-

Reconstruction of the atrophic maxilla combining Le Fort I osteotomy, sinus augmentation and titanium mesh guided bone regeneration: a technical note.

Severe atrophy of the edentulous maxilla poses a significant surgical challenge. This technical note presents a novel approach combining Le Fort I osteotomy, bilateral sinus augmentation, and titanium mesh guided bone regeneration. Preoperative prosthetic planning allows accurate maxillary repositioning to restore a skeletal Class I relationship and enable an implant-supported prosthesis without prosthetic gingiva. The titanium mesh provides stable, three-dimensional support for uniform ridge reconstruction, enabling ideal implant placement. This technique offers a reproducible and less invasi

Modification of planned postoperative occlusion in orthognathic surgery, based on computer-aided design/computer-aided manufacturing-engineered preoperative surgical simulation.

In orthognathic surgery, it is important to have a planned postoperative occlusion. A 3-dimensional preoperative simulation, based on 3-dimensional optically scanned occlusion data, can predict how the planned postoperative occlusion will affect the maxilla-mandibular relationship that results from orthognathic surgery. In this study we modified the planned postoperative occlusion, based on computer-aided design/computer-aided manufacturing-engineered preoperative surgical simulations. This modification made it possible to resolve the facial asymmetry of the patient successfully with a simple

Long-term results after horseshoe sandwich osteotomy of the edentulous maxilla as a preprosthetic procedure.

The long-term results in 30 patients with maxillary atrophy who have been operated on using a horseshoe sandwich osteotomy and interpositional bone grafting are reported. The postoperative period ranges from 1-4 years. Lateral cephalometric X-rays, models and clinical investigations served to determine the resorption rate and the postoperative results. It could be shown that the vertical resorption rate amounts to 1.8 mm (20%) after 3 years. The sagittal resorption rate was about 1 mm with a wide individual range. Clinical investigation revealed satisfactory results in 28 patients.

Cephalometric study of posterior airway space in patients affected by Class II occlusion and treated with orthognathic surgery.

The posterior airway space (PAS) is delimited by hard and soft tissues with anomalies that may produce alterations in volume. In patients with severe hypoplasia of the middle and lower third of the face, a decrease in volume of the PAS is present, producing a polysyndromic condition that ranges from snoring to obstructive sleep apnea syndrome (OSAS). The aim of this report is to define PAS variations via a cephalometric study in patients affected by class II occlusion and treated with orthognathic surgery. The authors studied 44 patients affected by class II occlusion who underwent surgery to

Anterior maxillary distraction using a tooth-borne device for hypoplastic cleft maxillas-a pilot study.

PURPOSE: The management of maxillary retrusion in a patient with cleft lip and palate is performed using Le Fort I advancement or distraction osteogenesis using an external or an internal device. Distraction mostly involves movement of the entire maxilla at the Le Fort I level and is characterized by a higher relapse rate and a hampering of speech. Hence, distraction of the anterior maxilla was performed using a tooth-borne palatal distractor similar to the one proposed by Gunaseelan et al (J Oral Maxillofac Surg 65:1044, 2007). However, the technique was modified by placing the distractor pre

A Case of Successful Tracheal Tube Exchange With McGrath MAC for Tube Damage During Oral Surgery.

A patient undergoing a bilateral sagittal split and LeFort 1 maxillary osteotomy performed under general anesthesia required emergent intraoperative exchange of a potentially damaged nasotracheal tube. This exchange was smoothly performed under constant indirect visualization using the McGrath MAC video laryngoscopy system. After the exchange, ventilation of the patient dramatically improved. The removed endotracheal tube was torn 19 cm from the distal tip. The McGrath MAC was useful for visualizing the glottis and confirming the entire course of the tube exchange despite the patient's having …
