The relationship of the medial roof and the posterior wall of the maxillary sinus to the sphenoid sinus: a radiologic study.

INTRODUCTION: The medial maxillary sinus roof is a ridge formed by the superior margin of the maxillary sinus antrostomy. The posterior wall of the maxillary sinus is always included in operative fields. OBJECTIVE: To perform a radiologic study assessing the utility of the medial maxillary sinus roof and the posterior wall of the maxillary sinus as fixed landmarks for providing a safe route of entry into the sphenoid sinus. METHODS: We reviewed 115 consecutive paranasal sinus Computed Tomographic scans (230 sides) of Korean adult patients performed from January 2014 to December 2014. Using t…

Prevalence of maxillary sinus pathology in patients considered for sinus augmentation procedures for dental implants.

PURPOSE: To determine the prevalence of maxillary sinus pathology in patients presenting for implant rehabilitation involving sinus augmentation procedures. MATERIALS AND METHODS: Three-dimensional images of 275 patients were evaluated. Age and gender were recorded to see if they had any relationship to the prevalence of pathology. Scans were classified into 1 of the 5 categories based on the type of sinus pathology detected: healthy, mucosal thickening > 5mm, polypoidal mucosal thickening, partial opacification and/or air fluid level, and complete opacification. RESULTS: Overall, 54.9% scan…

Choice of graft material in relation to maxillary sinus width in internal sinus floor augmentation.

PURPOSE: When grafts with solely osteoinductive or osteoconductive effects are used in the maxillary sinus, the medial wall of the sinus membrane should be reflected to receive the vascular supply and the osteoconductive effect. Because the internal sinus floor augmentation is a blind technique, it is impossible to directly verify whether the medial sinus wall is reflected in the operation. The purpose of this study is to describe the modality of choice among the grafting materials and techniques in relation to the width of the maxillary sinus through preoperative planning and measuring in com…

A retrospective study of the effects on sinus complications of exposing dental implants to the maxillary sinus cavity.

OBJECTIVE: The aim of this study was to investigate whether dental implant exposure to the maxillary sinus cavity increased the risk of maxillary sinus complications. STUDY DESIGN: Nine patients with 23 implants that had been inserted into the maxillary sinus more than 4 mm without lifting the sinus mucous membranes were evaluated for sinus complications 6 to 10 months after implant insertion, using a questionnaire and computerized tomography (CT). RESULTS: There were no clinical signs of sinusitis in any patient. However, CT scans showed postoperative sinus mucous thickening around 14 of th…

Translacrimal Antrostomy: An Augmented Endoscopic Approach to the Maxillary Sinus.

Conventional middle meatal antrostomy has limitations in surgical accessibility and may be associated with postoperative stenosis. Alternative approaches that improve access to the maxillary sinus while maintaining long-term patency are therefore needed. This study aimed to evaluate the safety and efficacy of a novel technique, translacrimal antrostomy (TLA). Unlike standard middle meatal antrostomy (MMA), TLA involves the removal of the inferior lacrimal bone to facilitate access to the anterior maxillary recesses. A retrospective review was conducted of 64 patients (81 sinuses) who underwent…

Leiomyosarcoma of the maxillary sinus presenting with epiphora: a case report.

BACKGROUND: Leiomyosarcoma is an invasive malignancy that emerges primarily from smooth muscle and accounts for a small percentage of head and neck sarcomas. Its occurrence in the sinonasal region is extremely infrequent, and few cases have been documented in the literature. Owing to vague symptoms and common imaging characteristics, diagnosis can often be delayed or misinterpreted. CASE PRESENTATION: We report a 57-year-old female patient who first visited the ophthalmology clinic with a complaint of epiphora and was misdiagnosed with a vascular tumor in the left maxillary sinus based on CT …

Undifferentiated pleomorphic sarcoma in the maxillary sinus: a case report.

BACKGROUND: Undifferentiated pleomorphic sarcoma, previously called malignant fibrous histiocytoma, is a type of malignant mesenchymal tumor (sarcoma) of soft tissue and sometimes bone. It is uncommon in the oral cavity and very sporadic in the maxillary sinus. Microscopic diagnosis of this malignancy in the maxillary sinus can be very challenging, because there is a range of features that may overlap with other benign and malignant tumors. CASE PRESENTATION: In this paper, we report a case of undifferentiated pleomorphic sarcoma in the maxillary sinus of a 61-year-old Iranian man who was ref…

Maxillary Sinus Papillary Edema as a Predictor of Odontogenic Sinusitis.

OBJECTIVES: Odontogenic sinusitis (ODS) refers to purulent sinusitis caused by infectious maxillary dental pathologies, with > 90% occurring unilaterally. While various clinical features suggest ODS, diagnosing some ODS patients can be challenging. This study aimed to determine the value of maxillary sinus papillary edema (MSPE) in predicting ODS. METHODS: A retrospective cohort study was conducted on patients who underwent at least maxillary antrostomy for unilateral sinus disease (USD). Adequately powered samples of three USD cohorts were generated: ODS, infectious maxillary CRS without nas…

ANATOMY OF THE MAXILLARY SINUS: IMPLICATIONS FOR ODONTOGENIC SINUSITIS DEVELOPMENT.

UNLABELLED: This study investigates the anatomical prerequisites that could contribute to the development of this condition. MATERIAL AND METHODS: Using multi-slice computed tomography (MSCT), the study examined the structure and relationships of key anatomical features, including the alveolar process, sinus wall thickness, and the position of the teeth in relation to the maxillary sinus. RESULTS: The results revealed that the lower wall of the maxillary sinus is predominantly formed by the alveolar process, with significant variability in wall thickness, especially between the central and l…

A rare presentation of a mucocele in a separate compartment of maxillary sinus.

Maxillary sinus is frequently presented as a single space within each maxilla. It drains directly into the nose through the middle meatus. We are reporting the second rare case of a mucocele in a separate compartment of maxillary sinus in a 26-year-old patient without a history of facial trauma. The non-contrast computed tomography of paranasal sinuses showed an expansile right infraorbital cystic lesion developed in an upper lateral position of the maxillary sinus. An excision biopsy was performed, which showed histopathological findings of a maxillary sinus mucocele.

Bilateral cholesterol granulomas of the maxillary sinus with review of the literature.

Cholesterol granulomas are a common benign pathology classically found in the mastoid antrum and air cells of the temporal bone and less commonly found in the paranasal sinuses. We present a unique case of bilateral cholesterol granulomas of the maxillary sinus that is the second case to our knowledge reported in the literature. In an effort to provide an update about cholesterol granulomas of the paranasal sinuses, we examined the literature from January 2011 through 2021 in conjunction with a previous systematic review of the literature from 1970 to December 2010. This report reinforces that…

The Use of CBCT in Evaluating the Health and Pathology of the Maxillary Sinus.

The use of cone-beam computed tomography (CBCT) has been increasing in dental practice. This narrative review summarized the relevance and utilizations of CBCT to visualize anatomical structures of the maxillary sinus and common pathologies found in the maxillary sinus. The detection/visualization rate, the location and the morphometric characteristics were described. For sinus anatomy, the reviewed features included the posterior superior alveolar artery, sinus pneumatization, sinus hypoplasia, sinus septa, and primary and accessory sinus ostia. For pathology, the following items were reviewe…

Hemangioma of the maxillary sinus: A benign and rare cause of epistaxis.

Hemangioma of the facial sinuses is a rare pathology, and given the lack of clinical specificity, the differential diagnosis with a malignant lesion often arises. We report the case of a 32-year-old patient who consulted for recurrent epistaxis of moderate severity. The preoperative diagnosis of a hemangioma of the left maxillary sinus was based on computed tomography and magnetic resonance imaging data, confirmed by the anatomopathological study of the surgical specimen, preceded by an embolization that facilitated the endoscopic surgical excision.
