Motorcycle crashes and upper extremity trauma.

OBJECTIVES: Upper extremity injuries following motorcycle crashes (MCC) incur increased healthcare costs and rehabilitation needs. We aim to characterize the epidemiology of MCC upper extremity injuries and identify factors that influence the severity of and cost of care for upper extremity injuries. METHODS: We performed a retrospective cohort analysis of 571 patients with upper extremity injuries after MCC at a level 1 trauma center from 2002 to 2013. We collected data pertaining to demographics, helmet use, toxicology, bony injury, Injury Severity Score (ISS), Glasgow Coma Scale (GCS), hos

Iatrogenic trigeminal post-traumatic neuropathy: a retrospective two-year cohort study.

With the growing demand for dental work, trigeminal nerve injuries are increasingly common. This retrospective cohort study examined 53 cases of iatrogenic trigeminal nerve injury seen at the Department of Oral and Maxillofacial Surgery, University Hospitals of Leuven between 2013 and 2014 (0.6% among 8845 new patient visits). Patient records were screened for post-traumatic trigeminal nerve neuropathy caused by nerve injury incurred during implant surgery, endodontic treatment, local anaesthesia, tooth extraction, or specifically third molar removal. The patients ranged in age from 15 to 80ye

Subcutaneous Emphysema and Pneumomediastinum Triggered by Nasotracheal Intubation: A Case Report.

Nasotracheal intubation (NTI) is frequently chosen in oral and maxillofacial surgery to provide unobstructive access to the surgical field. Despite being rare, subcutaneous emphysema (SE) and pneumomediastinum (PM) caused by nasopharyngeal wall injury are serious complications. In this report, we present the case of a 74-year-old woman who developed right cervical SE and PM during anesthesia induction for oral surgery under general anesthesia associated with nasopharyngeal wall injury due to NTI. She had no remarkable medical history or abnormal preoperative findings. During intubation, swelli

Aetiology and incidence of maxillofacial trauma in Amsterdam: a retrospective analysis of 579 patients.

INTRODUCTION: The incidence of maxillofacial fractures varies widely between different countries. The large variability in reported incidence and aetiology is due to a variety of contributing factors, including environmental, cultural and socioeconomic factors. This retrospective report presents a study investigating the aetiology and incidence of patients with maxillofacial fractures in Amsterdam over a period of 10 years. RESULTS: The study population consisted of 408 males and 171 females with a mean age of 35.9 (SD: ±16.3) years. The age group 20-29 years accounted for the largest subgrou

Minimal Invasive Navigation-Assisted Removal of Penetrating Metallic Foreign Bodies in the Craniomaxillofacial Region: A Case Report.

Removal of deeply embedded foreign bodies (FBs) in the craniomaxillofacial (CMF) region remains a significant surgical challenge because of the complex anatomy, proximity of critical neurovascular structures, and the limited accuracy of conventional imaging modalities for precise intraoperative localization. Inaccurate localization may increase operative time and the risk of iatrogenic injury. In recent years, there has been a considerable increase in the use of three-dimensional image-guided navigation systems (3D-IGNS) in various maxillofacial procedures, especially the extraction of penetra

Electric scooters: a quick way to get to the emergency department?

Introduction Electric scooters (e-scooters) are rife but are yet to be legalised in the UK. The aim of this paper was to investigate whether there had been an increase in the number of e-scooter injuries referred to the oral and maxillofacial surgery (OMFS) service at our unit. We present here what is, to our knowledge, the largest dataset regarding e-scooter-related injuries in the UK.Method A double cohort study in which details of patients sustaining e-scooter-related injuries that were referred to the OMFS department were collected, prospectively, for a 16-week period in 2020 (investigatio

Posttraumatic carotid-cavernous sinus fistula.

BACKGROUND AND OBJECTIVES: Posttraumatic carotid-cavernous sinus fistula is a rare complication of maxillofacial trauma and is seldom discussed in the literature. Motor vehicle accidents, falls, and other crush injuries contribute to the incidence of basilar skull fractures and the formation of fistulae. When injuries occur in the vessel wall, the carotid artery has the potential to fill the low-pressure cavernous sinus. The symptoms include chemosis, proptosis, pulsating exophthalmos, diplopia, ophthalmoplegia, orbital pain, audible bruits, and blindness. METHODS AND MATERIALS: The conventio

Subcranial management of 395 combined frontobasal-midface fractures.

In severe craniofacial injuries, the involvement of the skull base with concomitant major dural tears is significantly high. Our methods and treatment plan are controversial compared with the conventional procedure: primary urgent neurosurgical exploration and repair with deferral of maxillofacial reconstruction. To avoid the disadvantages of the transfrontal intracranial management of the skull base, we modified the transethmoidal approach so as to enable the subcranial exposure of all the anterior fossa planes, including the sellar-sphenoidal region. The advantages rendered by this method ar

Posttraumatic missile injuries of the orofacial region.

Iraq became the world's battlefield for terrorist attack to the victims by different types of weapons of missile including explosive cars, explosive belt, fragments, rifle bullets, and handgun bullets. This situation in Iraq has been present for the last 3 years. As surgeons, we cannot influence the surge of this violence, but we are surely called upon to care for its victims. Missile injuries to the orofacial region have special features that provide the surgeon with multiple medical and surgical challenges when dealing with these injuries. This study include 140 patients who were treated in

Temporal bone fractures: a review for the oral and maxillofacial surgeon.

Fracture of the temporal bone is, by definition, a fracture of the skull base. Even though the oral and maxillofacial surgeon (OMS) may not provide definitive management of temporal bone fractures or their sequelae, a working knowledge of this area is important for any surgeon participating in the care of patients with craniomaxillofacial trauma, because temporal bone fractures are often associated with injuries to other areas of the craniomaxillofacial skeleton and because these fractures are relatively frequent. In many centers, particularly community hospitals, the OMS may be the primary pr

Skin Necrosis Following Extravasation Injury: A 5-Year Experience in a Tertiary Paediatric Centre.

BACKGROUND: Extravasation is the leakage of intravenous drugs, chemicals, or fluids into the extravascular compartment and is common in paediatric patients. These injuries can cause ulceration with tissue loss. This study investigates whether a washout procedure can reduce the incidence of partial- or full-thickness skin loss following extravasation. METHODS: All extravasation injuries referred to the Plastic and Maxillofacial Department at the Royal Children's Hospital, Melbourne, from June 2018 to June 2023 were prospectively identified and reviewed. Data collected included patient demograp
