BACKGROUND: The Texas/Chihuahua (US/Mexico) border is a medically underserved region with many reported barriers for health care access. Although Hispanic ethnicity is associated with health disparities for many different diseases, the population-based estimates of incidence and survival for patients with blood cancer along the border are unknown. The authors hypothesized that Hispanic ethnicity and border proximity is associated with poor blood cancer outcomes.
METHODS: Data from the Texas Cancer Registry (1995-2016) were used to investigate the primary exposures of patient ethnicity (Hispan

Adults with neurodevelopmental disorders (ANDDs), including intellectual disability and autism spectrum disorder, are largely underserved by dedicated specialty centers in the United States. Many ANDDs also have seizures and sensorimotor deficits. To fill the gaps in care for ANDDs, we developed the Clinical Center for Adults with Neurodevelopmental Disorders (CCAND) to provide a multidisciplinary care program. Funding for CCAND staff salaries was solicited from and allocated by the Maryland General Assembly and Governor after our fair market salary analysis determined that physician professio

OBJECTIVE: To investigate disparities in the utilization of genetic sequencing among children with Infantile Epileptic Spasms Syndrome (IESS), a severe early-onset epilepsy where timely diagnosis and treatment are crucial for improving neurodevelopmental outcomes, previous studies have highlighted disparities in the evaluation and management of IESS. Genetic sequencing has emerged as a crucial tool in diagnosing unexplained epilepsies, offering precise etiological insights that can guide management. Despite guidelines recommending genetic sequencing for all unexplained epilepsies, little is kn

BACKGROUND: Telehealth has emerged as an essential health care tool, particularly during the COVID-19 pandemic, when in-person medical services were significantly restricted. While telehealth adoption surged during the pandemic, disparities in its access and use have been observed, especially among vulnerable populations. Understanding these trends and identifying barriers is crucial for promoting equitable health care delivery.
OBJECTIVE: This study aims to assess disparities in telehealth use across Virginia, focusing on demographic, socioeconomic, and geographic factors influencing access.

BACKGROUND AND OBJECTIVES: Emergency medical services (EMS) clinicians play a critical role in early identification and management of neurologic emergencies yet may encounter sparse neurology-specific education. With interprofessional collaboration, we developed a multimodal, active-learning, and experiential curriculum for teaching prehospital care in a neurologically underserved setting. We evaluated curriculum feasibility and impact on prehospital neurologic assessment. The objectives of this study were to (1) recognize signs and symptoms of neurologic emergencies, aiding in the formulation

BACKGROUND: Patients with cancer seen in rural and underserved areas disproportionately face barriers to access genetic services. Genetic testing is critical to inform treatment decisions, for early detection of another cancer, and to identify at-risk family members who may benefit from screening and prevention.
OBJECTIVE: To examine medical oncologists' genetic testing ordering trends for patients with cancer.
DESIGN, SETTING, AND PARTICIPANTS: This prospective quality improvement study was performed in 2 phases over 6 months between August 1, 2020, and January 31, 2021, at a community netw

BACKGROUND: Psoriasis is a chronic immune-mediated disease associated with multiple systemic comorbidities. Biologic therapies have transformed the management of moderate-to-severe psoriasis; however, their high cost remains a major barrier for uninsured and socioeconomically disadvantaged individuals. The Psoriasis Biologics Center for Indigent Patients at Jackson Memorial Hospital provides a structured dermatology access model for underserved populations.
METHODS: We conducted a descriptive retrospective cohort study of patients with moderate-to-severe psoriasis receiving biologic therapy t

INTRODUCTION: 22q11 deletion syndrome (22q11DS) is the most common microdeletion in humans, leading to a wide range of variable clinical manifestations that can affect any part of the body. Most individuals with 22q11DS are diagnosed during childhood and the responsibility of care falls on their primary caregivers, usually their parents, who need to balance the needs of all family members including themselves. This affects parental wellbeing. This study investigates the perspective of healthcare professionals (HCPs), in Europe and Canada, on the parents' struggle with time invested in caring f

INTRODUCTION: Tobacco use is a major contributor to the burden of chronic obstructive pulmonary disease (COPD) and other non-communicable diseases in China. People at high risk for COPD who smoke, particularly those with pre-existing chronic conditions, often remain underserved by conventional smoking cessation programmes. Population medicine offers a promising framework for proactively identifying high-burden diseases, managing multimorbidity and prioritising interventions for vulnerable populations.

BACKGROUND: To address the regional heterogeneity of Alzheimer's disease, a large-scale epidemiological study of 12,421 elderly individuals was conducted in southwestern China to depict its unique risk characteristics.
METHODS: A total of 12,421 subjects were selected via cluster sampling from southwestern China after low quality data were filtered out. On the basis of investigations and medical imaging examinations, three groups were distinguished: the AD, mild cognitive impairment (MCI), and normal control groups. The risk factors for AD and MCI were analysed via a multivariate logistic reg

Introduction: Clinical trials investigating the safety and efficacy of experimental drugs and devices are the cornerstone of medicinal advancement. Enrolling sufficient participants in these trials is vital to ensure adequate statistical power and generalizability. Clinical trial participation is particularly low among certain populations, including medically underserved communities (i.e., rural areas) and Black, Indigenous, and People of Color (BIPOC). Methods: A retrospective study design was used to understand patient outcomes and access/barriers to clinical trial participation in the rural

Babies born to severe acute respiratory syndrome corona virus-2 (SARS-CoV-2)-infected mothers are at greater risk for perinatal morbidity and more likely to receive a neurodevelopmental diagnosis in the first year of life. However, the effect of maternal infection on placental function and neonatal outcomes varies depending upon the patient population. We set out to test our hypothesis that maternal SARS-CoV-2 infection in our underserved, socioeconomically disadvantaged, mostly unvaccinated, predominantly African American and Latina population in the Bronx, NY would have effects evident at bi

An overview of health and social issues is presented here regarding Native Hawaiian transgenders. Perhaps due to relatively greater tolerance of gender diversity among Polynesian cultures, approximately 70% of all male-to-female transgenders in Hawai'i are Native Hawaiian. However, the overall climate is one of discrimination and harassment such that transgenders--who tend to be under-educated, under-employed, and medically underserved--may be the most severely impacted of all Native Hawaiians. Lei Anuenue, human immunodeficiency virus (HIV) prevention program for Native Hawaiians, has provide

INTRODUCTION: The USA exhibits the highest rate of maternal mortality among developed high-income countries. These deaths most commonly occur during the postpartum year and are largely preventable. Individuals that live in medically underserved areas and lack access to transitional care following childbirth are at higher risk for maternal mortality, severe maternal morbidity and adverse pregnancy outcomes. National US organisations have proposed implementation of innovative models, including coordinated postpartum service delivery across clinical and community-based organisations (CBOs), to he…

Female genital schistosomiasis (FGS) is a neglected but highly prevalent disease in sub-Saharan Africa, caused by Schistosoma haematobium egg-induced inflammation in the pelvic region. FGS is characterized by four mucosal lesion types in the lower female genital tract: grainy sandy patches (GSP), homogeneous yellow sandy patches, abnormal blood vessels, and rubbery papules. This study focuses on the segmentation of cervical GSP lesions using a deep-learning convolutional neural network. A total of 583 cervical images from women in a S. haematobium endemic region of Madagascar, all exhibiting F

INTRODUCTION: The MySteth is an intelligent medical tool designed for cardiac disease screening, utilizing either a stethoscope or smartphone to record heart sounds. Normal heart sounds in healthy individuals consist of "lub" and "dub" noises, while murmurs-additional sounds during heartbeats-can indicate cardiac anomalies such as valve dysfunctions and rapid blood flow, categorized as systolic or diastolic.
METHOD: MySteth was developed and tested using heart sounds recorded via smartphone and digital stethoscope. For ensuring the clinical validity of the data, all heart sound samples were m

BACKGROUND: The COVID-19 pandemic has disproportionately impacted rural and under-resourced urban communities in Kansas. The state's response to COVID-19 has relied on a highly decentralized and underfunded public health system, with 100 local health departments in the state, few of which had prior experience engaging local community coalitions in a coordinated response to a public health crisis.
METHODS: To improve the capacity for local community-driven responses to COVID-19 and other public health needs, the University of Kansas Medical Center, in partnership with the Kansas Department of

INTRODUCTION: People who use drugs (PWUD) face a multitude of barriers to accessing healthcare and other services. Mobile health clinics (MHC) are an innovative, cost-effective health care delivery approach that increases healthcare access to vulnerable populations and medically underserved areas. There is limited understanding, however, of how PWUD perceive and experience MHCs.
METHODS: Semi-structured interviews were conducted with 31 PWUD - 16 who had received care (clients) on an MHC (The Spot) and 15 who had not (non-clients) - to explore their perceptions and utilization of an MHC partn…
