Optimising investigative pathways in military medicine: operational impact of a military cardiopulmonary exercise testing clinic.

INTRODUCTION: Abnormal cardiorespiratory symptoms and investigative findings in service personnel typically result in prolonged investigation and occupational restriction. This analysis aimed to assess the impact of the Oxford Military Cardiopulmonary Exercise Testing Clinic (OMEC), which investigates such symptoms and findings, on occupational recommendations. METHODS: A service evaluation was conducted on all OMEC attendances over a 5-year period. Referral indication and occupational grading, demographics, exercise testing parameters, clinical diagnosis and occupational recommendation were

Modeling Zika Vaccination Combined With Vector Interventions in DoD Populations.

INTRODUCTION: Zika virus (ZIKV) is a mild febrile illness generally transmitted via the bite of infected Aedes species mosquitoes, including Aedes aegypti, with the potential to cause neurological complications. Nearly 200 U.S. military installations are located within areas where Aedes mosquitos are found, putting thousands of personnel at risk for infection with ZIKV. This analysis aims to quantify the benefits of interventions, including vaccination, to decrease the risk of ZIKV on U.S. military installations. METHODS: The authors developed a dynamic transmission model to test the "effecti

Identity adjustment among Afghanistan and Iraq war veterans with reintegration difficulty.

OBJECTIVE: To examine perceptions of identity adjustment in a diverse, national sample of U.S. veterans of the wars in Afghanistan and Iraq. METHOD: The authors conducted a planned thematic analysis of text written by Afghanistan and Iraq war veterans when they were asked to describe their reintegration difficulties as part of a randomized controlled trial (RCT) of online expressive writing (Sayer et al., 2015). Participants were 100 randomly selected veterans from the larger study (42 women and 58 men, 60 active duty and 38 reserves or National Guard). RESULTS: Nearly 2/3s of participants w

Child Abuse Experiences and Perceived Need for Care and Mental Health Service Use among Members of the Canadian Armed Forces.

OBJECTIVE: Child abuse is associated with poor mental health outcomes in adulthood. However, little is known about how a history of child abuse may be related to perceived need for care (PNC) and mental health service use (MHSU) among Canadian military personnel. The objectives of this study were to determine 1) the relationship between child abuse history and PNC and 2) the relationship between child abuse history and MHSU in the Canadian military. METHOD: Data were drawn from the 2013 Canadian Forces Mental Health Survey ( n = 6692 Regular Force personnel between the ages of 18 and 60 years

From Sensing to Sense-Making: A Framework for On-Person Intelligence with Wearable Biosensors and Edge LLMs.

Wearable biosensors increasingly stream multi-channel physiological and behavioral data outside the laboratory, yet most deployments still end in dashboards or threshold alarms that leave interpretation open to the user. In high-stakes domains, such as military, emergency response, aviation, industry, and elite sport, the constraint is rarely data availability but the cognitive effort required to convert noisy signals into timely, actionable decisions. We argue for on-person cognitive co-pilots: systems that integrate multimodal sensing, compute probabilistic state estimates on devices, synthe

Personal protective equipment solution for UK military medical personnel working in an Ebola virus disease treatment unit in Sierra Leone.

The combination of personal protective equipment (PPE) together with donning and doffing protocols was designed to protect British and Canadian military medical personnel in the Kerry Town Ebola Treatment Unit (ETU) in Sierra Leone. The PPE solution was selected to protect medical staff from infectious risks, notably Ebola virus, and chemical (hypochlorite) exposure. PPE maximized dexterity, enabled personnel to work in hot temperatures for periods of up to 2h, protected mucosal membranes when doffing outer layers, and minimized potential contamination of the doffing area with infectious mater

Norwalk-like virus and bacterial pathogens associated with cases of gastroenteritis onboard a US Navy ship.

Acute gastroenteritis is a potential cause of substantial morbidity in U.S. military personnel during deployment. This study investigated the microbial causes of diarrhea in U.S. troops on exercises in Southeast Asia aboard the U.S.S. Germantown from March through May 1996. A total of 49 (7%) patients with diarrhea reported to sick call during a 3-month deployment involving 721 personnel. Diarrheal samples from 49 patients were subjected to bacterial and parasitologic examination, but sufficient samples from only 47 of 49 were available for analysis of the presence of Norwalk-like virus (NLV).

How to train small military medical teams.

Small military medical teams are increasingly required to deliver advanced clinical capability in remote and austere environments. They are structurally fragile because expertise is concentrated in a small number of individuals who cannot be replaced or rotated. Redundancy is minimal and the organisational depth that absorbs interpersonal dysfunction in larger systems is absent. When dysfunction occurs, the consequences may propagate directly into patient care and operational capability. Dysfunction in these teams is not primarily a product of technical deficit. It arises from a combination of

The ambivalence about accepting the prevalence somatic symptoms in PTSD: Is PTSD a somatic disorder?

This study examined the prevalence of somatic symptoms in post-traumatic stress disorder (PTSD) in a population-based military sample (N = 14,445). Descriptive statistics explored somatic symptom endorsement in the entire sample. A latent class analyses was conducted on participants with a posttraumatic stress checklist (PCL) score ≥29 (n = 2433), with class differentiated by somatic symptom endorsement.Multinomial logistic regression explored correlates of latent class. Somatic disorder was more prevalent in probable-PTSD (59.6%) and subsyndromal-PTSD (26.5%) than no-PTSD (5.0%) groups, suppo

A rare presentation of sexsomnia in a military service member.

Sexsomnias are unconscious sexual behaviors during sleep (parasomnias) that are garnering more attention as they become increasingly evident in forensic cases. Presentations of sexsomnia in active duty service members are seldom described, and often evaluation is clouded by intoxication with substances such as alcohol, sleep deprivation, untreated sleep disturbances, or criminal behavior masquerading as a medical disorder. We present a case of a 40-year-old male soldier evaluated in our sleep clinic for multiple episodes of sleep masturbation occurring over a period of 2 years. The patient was

Application of Genomics to Prevent and Combat Emerging Infectious Disease Threats in Latin America: Perspectives from an Overseas U.S. Navy Research Command.

Endemic and (re)emerging infectious diseases pose a major threat to public health and military readiness in Central and South America. The high biodiversity and the differing transmission pathways require the use of highly sensitive methods to track the emergence and spread of these diseases. High-throughput sequencing technologies have become a critical resource for the field of infectious disease monitoring, specifically for tracking pathogen evolution, antimicrobial resistance, and transmission. This review provides an overview of the implementation of genomics-informed infectious disease s

Laboratory Validation of the Sand Fly Fever Virus Antigen Assay.

Sandfly fever group viruses in the genus Phlebovirus (family Bunyaviridae) are widely distributed across the globe and are a cause of disease in military troops and indigenous peoples. We assessed the laboratory sensitivity and specificity of the Sand Fly Fever Virus Antigen Assay, a rapid dipstick assay designed to detect sandfly fever Naples virus (SFNV) and Toscana virus (TOSV) against a panel of phleboviruses. The assay detected SFNV and TOSV, as well as other phleboviruses including Aguacate, Anahanga, Arumowot, Chagres, and Punta Toro viruses. It did not detect sandfly fever Sicilian, He

Treating complex trauma among veterans: three stage-based treatment models.

This article addresses the issue of complex trauma in veterans and treatments for symptom presentations resulting from complex trauma exposure. While various definitions have been proposed for complex trauma, the clinical issues related to it are relevant for veterans as they are at risk for cumulative trauma exposures such as multiple combat deployments and military sexual trauma. Several treatments were either developed to address and/or implemented with complex trauma. This article discusses three of these treatments that share a stage-based approach, focusing on the present (e.g., skills t

Apparent exacerbation of Hansen's disease despite appropriate antimicrobial therapy.

In 1991, the World Health Organization and its member states resolved to reduce the global prevalence of Hansen's disease (leprosy) by at least 90%. Despite its waning prevalence, however, the disease remains endemic in parts of Africa, Asia, and Latin America. Moreover, imported cases are occasionally encountered innonendemic areas. This fact, coupled with the current tempo of overseas deployments, makes it imperative that military physicians familiarize themselves with both the presentation and potential complications of Hansen's disease. Here, we present the case of a soldier referred to Wa

Lessons Learned From a Nationwide Implementation of Airborne Hazards and Open Burn Pits Registry Clinical Examinations: A Qualitative Assessment.

INTRODUCTION: The Airborne Hazards and Open Burn Pits Registry (AHOBPR) examination was a service for Veterans with concerns about environmental exposures during military deployments. Interested Veterans enrolled in the AHOBPR online, completed a questionnaire, and indicated interest in a clinical examination by a Veterans Health Administration clinician. Several factors (e.g., COVID, unanticipated demand, and personnel turnover) resulted in a large number of pending examinations nationwide by 2022, prompting an evaluation and implementation effort to accelerate completion of high-quality exam…

Training, Deployment Preparation, and Behavioral Health of New York National Guard Personnel Deployed to Assist with COVID-19 Decedent Work.

INTRODUCTION: A small body of research conducted mostly among civilians has shown that adequate training and preparation can prevent or reduce the development of behavioral health problems in first responders. Several civilian studies have shown that social support is protective against behavioral health problems. However, very few studies have examined the impact of these factors on the behavioral health of military first responders. Military first responders, who serve in the aftermath of natural disasters and disease outbreaks such as the coronavirus disease 2019 (COVID-19) pandemic, are of

Prevalence, patterns and motives for hormonal contraceptive use among German servicewomen: results from a nationwide survey.

Hormonal contraceptive (HC) use is common among servicewomen and has been discussed in relation to health, performance, and operational readiness. No systematic data exist on HC prevalence among German servicewomen. This study analyzes contraceptive practices and motivations for contraceptive choice in German servicewomen. A nationwide, cross-sectional online survey was conducted among female members of the German Armed Forces. Recruitment occurred via military institutions and a digital flyer. The anonymous questionnaire (40 items, 10-15 min) assessed demographic characteristics and contracep

Hemostasis in a noncompressible hemorrhage model: an end-user evaluation of hemostatic agents in a proximal arterial injury.

OBJECTIVE: 1. Evaluate hemostatic bandages by the end user using subjective and objective criteria. 2. Determine if user training and education level impact overall hemostatic outcomes. 3. Our hypothesis was that prior medical training would be directly linked to improved hemostatic outcomes in noncompressible hemorrhage indepen- dent of dressing used. DESIGN: Military personnel were given standardized instruction on hemostatic dressings as part of a tactical combat casualty care course (TC3). Soldiers were randomized to a hemostatic dressing. Mediation and Moderation of the Relationship Between Combat Experiences and Post-Traumatic Stress Symptoms in Active Duty Military Personnel.

BACKGROUND: Post-traumatic stress disorder (PTSD) is a major health concern among the U.S. military population, affecting up to 12% to 24% of veterans returning from Iraq and Afghanistan. Sleep disturbances, neuroticism, and childhood trauma have all been associated with the development of PTSD in military populations, especially in relation to combat experiences. The effects of disrupted sleep and post-traumatic stress can affect the physical well-being of soldier and sailors in the field and impact them for years after deployment. This study aimed to evaluate the relationship between self-re

A QuEST for Nursing Clinical Activity Exposure: Comparison of the Military Treatment Facility and a Civilian Level I Trauma Center.

INTRODUCTION: Most military nurses work day-to-day in Military Treatment Facilities (MTFs), caring for a low volume of non-trauma patients until tasked to deploy. Military-civilian partnerships (MCPs) were established to increase opportunities for trauma exposure and high acuity patient care that are often lacking at MTFs but are essential for deployment readiness. In a near-peer conflict, deployed medical teams can expect to hold patients for days before evacuation and must be proficient in prolonged casualty care. Current Air Force nurse readiness checklists do not quantify expeditionary ski
