Using GPS Technologies to Examine Community Mobility in Older Adults.

BACKGROUND: Objective measures of community mobility are advantageous for capturing movement outside the home. Compared with subjective, self-reported techniques, global positioning system (GPS) technologies leverage passive, real-time location data to reduce recall bias and increase measurement precision. We developed methods to quantify community mobility among community-dwelling older adults and assessed how GPS-derived indicators relate to clinical measures of physical and cognitive performance. METHODS: Participants (n = 149; M ± standard deviation [SD] = 77.1 ± 6.5 years) from the progr

Assessment of the Value of a Carriage System to Organize and Elevate Intravenous Tubing.

Caregiver/patient fall injury risks increase when medical tubing drapes on floors. The objective of this research was to examine the value of a novel carriage system that organizes and elevates medical and intravenous (IV) tubing. Using a prospective, multicenter, cohort design, value of the IV carriage system was assessed using a valid, reliable survey that provided the total score and scores of 3 involvement factors: personal relevance, attitude, and importance. The survey was scored on a 0-100 scale, and questions about tubing elevation, patient mobility, and ease of use were rated on 0-10

[The Lübeck Scale of Basic Mobility : Assessment of geriatric patients with severely impaired mobility].

BACKGROUND: In the geriatric assessment of mobility, the timed up and go (TUG) test is often used; however, many inpatients are unable to master this test. The Lübeck Scale of Basic Mobility (LSBM) was developed as a performance test for this target group. OBJECTIVE: The study investigated the properties of the 7‑task LSBM, which has a scaling at item level based on the 5‑level assessment of impairments according to the ICF. MATERIAL AND METHODS: In 77 patients who had not mastered the TUG test at acute geriatric hospital admission, the LSBM was completed at intervals of 7-18 days (t0, t1),

Effect of a physical activity and behaviour maintenance programme on functional mobility decline in older adults: the REACT (Retirement in Action) randomised controlled trial.

BACKGROUND: Mobility limitations in old age can greatly reduce quality of life, generate substantial health and social care costs, and increase mortality. Through the Retirement in Action (REACT) trial, we aimed to establish whether a community-based active ageing intervention could prevent decline in lower limb physical functioning in older adults already at increased risk of mobility limitation. METHODS: In this pragmatic, multicentre, two-arm, single-blind, parallel-group, randomised, controlled trial, we recruited older adults (aged 65 years or older and who are not in full-time employmen…

Effects of an Electronic Health Record Tool on Team Communication and Patient Mobility: A 2-Year Follow-up Study.

BACKGROUND: Intensive care unit early mobility programs improve patients' functional status and outcomes. An electronic health record-based communication tool improved interprofessional communication within an early mobility program. Long-term sustainability of this communication tool has not been evaluated. LOCAL PROBLEM: Ineffective interprofessional communication was perceived as a barrier to success of an intensive care unit early mobility program at the project institution. An electronic health record-based communication tool was successfully implemented to improve communication. Sustain

Rehabilitation Following Fracture in Dystrophinopathy, A Case Series.

BACKGROUND: Boys with dystrophinopathies (DMD) are at increased risk of low bone mineral density and fracture. Femoral fracture is the most common extremity fracture and is accompanied by significant risk of functional loss. Care considerations for DMD have stressed that aggressive management may be needed to maintain ambulation and that surgical fixation allows early mobilization. OBJECTIVES: Describe 5 cases of femoral fracture in ambulatory boys with DMD and the course of care undertaken to optimize function. PATIENTS: Five boys with DMD median age 15y (12-16) who were independently ambul

The Role of Architecture and Design in the Management of Parkinson's Disease: A Systematic Review.

BACKGROUND: Parkinson's disease (PD) is a neurological condition characterized by the development of daily disabling symptoms. Although the architecture and design of a PD patient's environment can hinder or facilitate full participation in daily activities, their putative role in the management of these patients has received little attention to date. OBJECTIVE: We conducted a systematic review to evaluate the evidence of architectural and design features in the management of people with PD. METHODS: An electronic database search of observational and experimental studies was conducted in MED

Pilot Study of Cadence, a Novel Shoe for Patients with Foot Drop.

Foot Drop is a mobility disorder that limits ankle dorsiflexion, complicating the swing phase of gait and balance. It is a common result of a neurological injury or disease such as stroke, cerebral palsy or multiple sclerosis. Here we present Cadence, a low-cost assistive shoe designed to passively improve the biomechanics and rhythmicity of gait for people with foot drop. The shoe reduces the magnitude of scuffing forces when dragging the foot forward across the ground by using regions of low-friction material that can retract into the shoe to restore friction during stance phase. We report t

[Principles and challenges of mobilisation in intensive care].

The harmful consequences of bed rest and inactivity in patients in intensive care have been widely described. The point at which these patients should be mobilised and the methods used however still remain unclear. It is nevertheless important that the mobilisation is implemented early and often, adapted to the condition of the patient and overseen by a cross-disciplinary team.

Renal function overestimation in underweight and/or non-ambulatory patients.

Creatinine clearance estimation is widely used to evaluate the renal function of the patients in order to initiate or adjust the drugs dosage. However serum creatinine, as a muscle metabolism by-product, may not be a reliable parameter in underweight and/or non-ambulatory patients, such as geriatric, acquired immunodeficiency syndrome patients and bed-confined and cachexic cases. To avoid overestimation of the renal function in those patients, serum cystatin C can be considered as a sensitive and accurate alternative for serum creatinine.
