Safety of COVID-19 vaccination and acute neurological events: A self-controlled case series in England using the OpenSAFELY platform.

INTRODUCTION: We investigated the potential association of COVID-19 vaccination with three acute neurological events: Guillain-Barré syndrome (GBS), transverse myelitis and Bell's palsy. METHODS: With the approval of NHS England we analysed primary care data from >17 million patients in England linked to emergency care, hospital admission and mortality records in the OpenSAFELY platform. Separately for each vaccine brand, we used a self-controlled case series design to estimate the incidence rate ratio for each outcome in the period following vaccination (4-42 days for GBS, 4-28 days for tran

Recurrent demyelinating myelitis associated with hepatitis C viral infection.

We report a 46-year-old-man who developed recurrent myelitis associated with hepatitis C viral (HCV) infection. Spinal cord biopsy showed acute demyelination without evidence of vasculitis. Antibodies to HCV were present in the CSF; HCV RNA was not detected in the CSF. Neither HCV antigens nor RNA were detected in the spinal cord biopsy, whereas they were found in the liver biopsy. Evaluation for other infectious or autoimmune causes was unrevealing. These observations suggest that recurrent myelitis in this patient is etiologically related to HCV infection, possibly via an immune-mediated mec

Nervous system affections caused by the mumps virus.

Hippocrates probably first described mumps parotitis, but not until 1758 was affection of the central nervous system reported in this disease. Mumps meningitis, encephalitis, myelitis, polyradiculitis and cranial neuritis are now well known, and may occur without clinical parotitis. Meningitis occurs most commonly, encephalitis, cranial neuritis and polyradiculitis less often, and myelitis rarely. They may present individually or in combination. A patient is described who first developed acute mumps meningoencephalitis, without clinical parotitis. Transverse myelitis occurred two weeks later,

Myelitis in the course of systemic lupus erythematosus: review.

Myelopathy manifested clinically as acute longitudinal or transverse myelitis constitutes one of the most severe and rare neuropsychiatric manifestations of systemic lupus erythematosus (SLE) (1-3% of patients). Myelitis has been observed less commonly in other connective tissue diseases, mostly in antiphospholipid syndrome, and rarely in Sjögren's syndrome, Behçet's disease and mixed connective tissue disease. Acute transverse myelitis (ATM) may also be present in diseases of various etiology, including multiple sclerosis, sarcoidosis, infectious diseases and malignancies. Myelitis in SLE is

Evolution of neuroimaging features in children with acute flaccid myelitis compared to other forms of childhood myelitis.

BACKGROUND: Acute flaccid myelitis (AFM) is characterized by acute onset of flaccid limb weakness, MRI abnormalities in the spinal cord grey matter and CSF pleocytosis often associated with enterovirus infections. MOG-associated diseases (MOGAD) can manifest with similar neuroradilogical features. OBJECTIVE: To analyze MRI findings in patients with AFM at baseline and follow-up in comparison to other forms of childhood transverse myelitis such as MOGAD. PATIENTS AND METHODS: Children from 11 European centers who fulfilled the clinical criteria of AFM were included. Brain and spinal MRI was p

Anti-myelin Oligodendrocyte Glycoprotein Antibody-positive Myelitis after Coronavirus Disease 2019.

We herein report a case of anti-myelin oligodendrocyte glycoprotein (MOG) antibody-related myelitis caused by coronavirus disease (COVID-19) infection in 2021. A 22-year-old man with no history of any related illness contracted COVID-19. Eight days later, he developed bladder problems, paraplegia and sensory disturbances. Cervical spinal cord magnetic resonance imaging revealed extensive hyperintensity at T2 and spinal cord lesions extending from C4 to Th1. The patient was diagnosed with transverse myelitis and started on intravenous methylprednisolone, plasma exchange and intravenous immunogl

Clinical and Radiologic Features Among Children With Myelin Oligodendrocyte Glycoprotein Antibody-Associated Myelitis.

BACKGROUND: Myelin oligodendrocyte glycoprotein (MOG) antibody-associated disease (MOGAD) often manifests as optic neuritis, transverse myelitis(TM), and acute disseminated encephalomyelitis. Patients with a TM phenotype are at high risk for neurological sequelae, so recognizing the characteristics of MOG-IgG myelitis is essential for early, accurate diagnosis and treatment. METHODS: This was a single-center retrospective study. Pediatric MOG antibody-associated disease patients who had clinical myelitis were recruited for this study. Data on clinical and radiologic features and outcomes were

[Recurrent multifocal lesions of the nervous system after herpes zoster].

The authors report a case of zoster with signs of transverse myelitis and peripheral involvement of the facial nerve. The fact that tranverse myelitis developed late after zoster and was followed by facial nerve palsy is stressed. The involvement of these nervous structures occurred in a way suggesting exacerbations. In the light of literature data and the described case the authors state that zoster with complications in the form of multifocal nervous system involvement occurring at different time intervals is infrequent.

[Central nervous system infection caused by mumps virus].

Central nervous system infection caused by mumps virus is reviewed. Aseptic meningitis, acute encephalitis, chronic encephalitis, hydrocephalus, ataxia, transverse myelitis, Guillain-Barré syndrome and deafness are noted as central nervous system complications of mumps. Among these complications, acute and chronic encephalitis, hydrocephalus, transverse myelitis and deafness occasionally result in severe sequelae. Aseptic meningitis due to mumps vaccine strain increased significantly after the introduction of trivalent measles, mumps and rubella vaccination in Japan. Recently, a differentiatio

Longitudinally extensive myelitis: a rare parainfectious neurological complication of dengue.

Dengue fever, transmitted by Aedes aegypti, can cause neurological complications in up to 5 % of cases. Longitudinally extensive myelitis is a rare complication that affects three or more spinal segments and may occur in para-infectious or post-infectious phases through direct viral invasion or immune-mediated mechanisms. We report a 34-year-old man with dengue who, on the fifth day of illness, developed paraparesis with urinary and fecal retention. Magnetic resonance imaging showed lesions from T2 to T12, and cerebrospinal fluid analysis revealed lymphocytic pleocytosis with no other infectio

Myelopathy due to intracranial dural arteriovenous fistula: a potential diagnostic pitfall. Case report.

Intracranial dural arteriovenous fistula (DAVF) is rare and potentially life-threatening disease often presenting as vascular myelopathy. The early and proper diagnosis is challenging because the clinical manifestations are related to the distribution of the draining vein, not the fistula site, and imaging findings are similar to demyelinating disease of the spinal cord. The authors present the case of a 45-year-old man who developed acute progressive quadriplegia and respiratory difficulty with an enhancing, longitudinally extensive cervical cord lesion. These symptoms were highly suspicious

[Neurosyphilis or not - a case of a differential diagnostic challenge].

We report the case of a 42-year-old woman with paraparesis associated with transverse myelitis. For differential diagnostics detailed microbiological, cerebrospinal fluid (CSF) and neuroimaging examinations were performed. Syphilis was confirmed, but diagnosis of neurosyphilis was only probable based on the CSF microbiological test results. The beneficial treatment response to application of the therapeutic protocol for syphilis supported the supposed diagnosis of syphilis-associated myelitis in our case. In this case report we reviewed the differential diagnostic tools of myelopathies/myeliti

Myelin oligodendrocyte glycoprotein antibody-positive optic neuritis with considerable white blood cell elevation in cerebrospinal fluid.

We present a case with an atypical presentation of myelin oligodendrocyte glycoprotein associated disease (MOGAD) presenting with optic neuritis, short-segment transverse myelitis, and significant lymphocytic pleocytosis. The degree of lymphocytic pleocytosis in this case was unusually high and prompted extensive workup. Broad infectious and neoplastic work-up was unremarkable and raised the concern for aseptic meningitis. A literature review was performed of similar cases with documented CSF labs (see Table 1), which demonstrates the unique and significant pleocytosis noted in this case.

Impact of the hematocrit value of the blood prime unit on patient hematocrit values after therapeutic plasma exchange in children weighing 10 kg or less.

We report the impact of measuring the hematocrit (HCT) of blood prime units (BPUs) on postprocedure patient HCT values in a small child with transverse myelitis undergoing therapeutic plasma exchange (TPE). Initially, the BPU HCT values were not measured, according to our apheresis policy of using our blood center's estimated HCT value. This approach resulted in unexpected increasing elevations of our patient's post-TPE HCT after the first two TPE procedures. Subsequent measurement of the BPU HCT prior to use stabilized the patient's post-TPE HCT. To our knowledge, this is the first case repor

Neuromyelitis optica (Devic's syndrome) as first manifestation of systemic lupus erythematosus.

Neurologic symptoms rarely occur as presenting feature of systemic lupus erythematosus (SLE). We describe a 37-year old woman who presented with several episodes of transverse myelitis and optic neuritis. Clinical, radiologic and laboratory findings were compatible with neuromyelitis optica (NMO). Seven years after disease onset clinical and laboratory findings were diagnostic for SLE. This case illustrates that NMO may represent a first manifestation of SLE for many years.

Plasma exchange for steroid-refractory relapses in multiple sclerosis: an observational, MRI pilot study.

BACKGROUND: Numerous studies have shown that plasma exchange (PE) is effective as second-line treatment of severe exacerbations of multiple sclerosis (MS) or other idiopathic inflammatory demyelinating diseases of the central nervous system that are nonresponsive to steroid therapy. OBJECTIVE: The goal of this study was to analyze the effect of PE on clinically active radiologic lesions in steroid-refractory relapses of MS and idiopathic inflammatory demyelinating diseases of the central nervous system. METHODS: This was a prospective, observational pilot study in which the primary end point

Case report: "an unexpected origin of coma in a young adult".

We report a peculiar case of acute non-traumatic coma due to neuromuscular hypoventilation syndrome caused by a non-traumatic spinal cord injury (NTSCI). A 21-year-old patient presented to the emergency room complaining of sudden onset weakness in his lower limbs and shortness of breath. While in the ER, he briefly became comatose and labs revealed an acute respiratory acidosis. Detailed neurologic examination ultimately revealed upper motor neuron signs and quadriplegia. He was ultimately diagnosed with a non-traumatic spinal cord injury, in particular, a cervical transverse myelitis which ha

Acute paraparesis in HIV-infected patient after initiation of highly active antiretroviral therapy.

Neurological syndromes occur in around 40-70% of HIV-infected people. Direct central nervous system involvement by the virus usually manifests as HIV encephalitis, HIV leucoencephalopathy, vacuolar leucoencephalopathy or vacuolar myelopathy. Indirect involvement is usually associated with neurotropic opportunistic infections which include tuberculosis, toxoplasmosis, cryptococcosis and viral encephalitis such as herpes simplex, varicella-zoster, cytomegalovirus and Human polyomavirus 2. We report a case of transverse myelitis in a recently diagnosed HIV patient who was otherwise asymptomatic i

Full recovered meningoencephalomyelitis caused by mumps virus.

Meningitis is the most common central nervous system complication of mumps virus, whereas encephalitis, cranial nerve palsies, poliradiculoneuritis are less common and myelitis caused by mumps virus is very rare [Nussinovitch et al., Acta Pediatrica (1992) 81, 183]. Lesions in young age as a result of severe acute transverse myelitis [shown on a magnetic resonance imaging (MRI) procedure] often has a viral cause. The case of a young male afflicted by meningoencephalomyelitis because of mumps virus is discussed.
