Cross-sectional echocardiographic visualization of abnormal systolic motion of the left ventricle in mitral valve prolapse.

In 11 of 34 patients with typical mitral valve prolapse on the M-mode echocardiogram, cross-sectional echocardiography in the apical four-chamber view revealed a characteristic abnormality not previously described. In mid- to late-systole the ventricular septum contracted vigorously, forming a bend or sharp convexity toward the left ventricle, while motion of the posterolateral basal left ventricle was diminished and delayed. This finding was not encountered in 35 other patients with innocent systolic murmurs, or in 15 patients with mitral regurgitation of etiologies other than mitral valve pr

[Early diagnosis of circulatory insufficiency in thyrotoxicosis].

On the basis of the results of examination of 256 patients with various forms of thyrotoxicosis the author distinguished criteria for early diagnosis of circulatory insufficiency of thyrotoxic origin. The following were referred to their number: a) prolongation of the ejection time with normal or decreased expulsion values; b) increase of the minute heart volume chiefly or exclusively on account of increased frequency of cardiac contractions; c) disproportion between the indices of heart work and the rate of cardiac contraction; d) the presence of hypodynamic reaction to the altering volume lo

Adulthood adiposity affects cardiac structure and function in later life.

BACKGROUND AND AIMS: Excess adiposity is associated with poorer cardiac function and adverse left ventricular (LV) remodelling. However, its … E/e', e', and left atrial volume indexed to body surface area; systolic: ejection fraction, S', and myocardial contraction fraction) was investigated using multivariable linear regression models. RESULTS: Increased BMI from age 20 years onwards was associated with greater LVM and LVIDd independent of confounders. Associations remained independent of current BMI for LVI …

Regional myocardial hemodynamic and metabolic effects of ionic and nonionic contrast media in normal and ischemic states.

The effects of intracoronary injection of two nonionic contrast media (iohexol and metrizamide) on myocardial contraction and chemical composition of coronary sinus (CS) blood were compared with those caused by the standard ionic contrast material for coronary angiography, sodium meglumine diatrizoate (R76), in 14 anesthetized dogs. The effects of each agent on regional contractility were compared in the normal state and in the presence of a critical coronary artery stenosis. The three contrast media produced equivalent decreases in hematocrit and sodium (both NS), but R76 caused a greater inc…

Circulating excitation-contraction coupling proteins and incident cardiovascular outcomes: associations and incremental predictive performance in the UK Biobank.

INTRODUCTION: Excitation-contraction coupling is central to cardiovascular physiology, but the prospective relevance of circulating proteins involved in this pathway remains uncertain. METHODS: We evaluated a prespecified seven-protein excitation-contraction coupling panel (EC7: NEXN, SLMAP, ITPR1, CACNB1, HRC, RYR1, and CACNA1C) in the UK Biobank proteomics cohort in relation to incident stroke, heart failure, acute myocardial infarction, and angina recorded during the available observation period. Outcome-specific logistic-regression models estimated odds ratios per 1-standard-deviation hig

Evaluation of Left Ventricle Function by Regional Fractional Area Change (RFAC) in a Mouse Model of Myocardial Infarction Secondary to Valsartan Treatment.

AIM: Left ventricle (LV) regional fractional area change (RFAC) measured by cardiac magnetic resonance (CMR) allows the non-invasive localization and quantification of the degree of myocardial infarction (MI), and could be applied to assess the effectiveness of pharmacological or regenerative therapies. Here we investigate the ability of RFAC to identify regional dysfunction and discriminate the effect of pharmacological treatment with valsartan, a selective antagonist of angiotensin II type 1 receptor, in a model of MI. METHODS AND RESULTS: C57BL/6N mice, undergoing coronary artery ligation,

[A study of 36 cases of mitral valve prolapse by isotopic ventricular tomography].

Left and right ventricular wall motion was studied in mitral valve prolapse with or without ventricular arrhythmias. Regional and global ventricular wall motion was evaluated by isotopic methods, based in ejection fraction and Fourier phase analysis representing the progression of wall contraction. The synchronisation of the ventricles was characterized by the difference of the mean phase of each ventricle. The heterogeneity of contraction of each ventricle was defined by the dispersion around the mean (standard deviations of the phases). Fifteen of the 36 patients had complex ventricular arrh

Congenital absence of the left circumflex coronary artery in the systolic click syndrome.

The anatomy of the coronary artery circulation was examined by means of selective coronary arteriography in 19 patients, evaluated because of disabling chest pain and ECG abnormalities, with typical clinical findings of the systolic click syndrome (SCS). In 17 (89.5%), the elft circumflex coronary artery (LCCA) was absent; a single marginal branch arose from the left main vessel, but no vessel was present in or near the atrioventricular (A-V) groove. In contrast, the LCCA was identified in 74 of 78 control patients (94.9%) considered to have representative normal distribution of coronary arter

A rare cause of dilated cardiomyopathy: hypocalcemia.

Dilated cardiomyopathy (DCM) is characterized by systolic dysfunction and is usually idiopathic. A rare cause of reversible DCM is hypocalcemia. Calcium plays a key role in myocardial contraction. Hypocalcemia can lead to a decrease in contraction, left ventricular systolic dysfunction, and heart failure with reduced ejection fraction (EF). Hypocalcemia-related reversible DCM reports are rare. Herein, we present two cases with heart failure caused by hypocalcemia developed due to hypoparathyroidism. The first case presented with severe heart failure and an extremely low serum calcium level (4.…

Biorheology of occlusive thrombi formation under high shear: in vitro growth and shrinkage.

Occlusive thrombi formed under high flow shear rates develop very rapidly in arteries and may lead to myocardial infarction or stroke. Rapid platelet accumulation (RPA) and occlusion of platelet-rich thrombi and clot shrinkage have been studied after flow arrest. However, the influence of margination and shear rate on occlusive clot formation is not fully understood yet. In this study, the influence of flow on the growth and shrinkage of a clot is investigated. Whole blood (WB) and platelet-rich plasma (PRP) were perfused at high shear rates (> 3,000 s-1) through two microfluidic systems with

Improvement of afterload mismatch of left atrial booster pump function with positive inotropic agent.

OBJECTIVES: The objective of this study was to examine the hypothesis that a positive inotropic agent improves left ventricular (LV) filling during left atrial (LA) contraction in the presence of markedly elevated LV filling pressure. BACKGROUND: In patients with old myocardial infarction (MI), an increase in the operational LV chamber stiffness reduces LV filling during the LA contraction, resulting from an "afterload mismatch" of the LA booster pump function. METHODS: We investigated the effect of dobutamine infusion (3 microg/kg/min) on the LA pump function in the presence of elevated LV

Abnormal global left ventricular relaxation occurs early during the development of pharmacologically induced ischemia.

In animal and human models, left ventricular (LV) diastolic function has been observed to be highly sensitive to myocardial ischemia. The response of LV diastolic parameters to pharmacologically induced ischemia, however, has not been characterized and might be important in the interpretation of dobutamine stress echocardiography. Eight mongrel dogs, in which were inserted a high-fidelity micromanometer LV catheter, coronary sinus sampling catheter, and ultrasonic coronary artery flow probe, underwent intravenous dobutamine infusion at escalating doses both before (control protocol) and after

Pharmacology of beta-blockers: classical aspects and recent developments.

All clinically used beta-blockers share the common feature of being competitive antagonists at beta-adrenoceptors. They differ, however, in additional pharmacological properties, such as beta1/beta2-selectivity ratios, presence or absence of intrinsic sympathomimetic activity (ISA), and/or local anesthetic activity. Furthermore, beta-blockers differ widely in their pharmacokinetic properties. The mammalian beta1- and beta2-adrenoceptors are the products of different genes but the receptor proteins show a certain degree of homology. Both span the cell membrane seven times. The cytoplasmic part

Cerium chloride as a histochemical marker of hydrogen peroxide in reperfused ischemic hearts.

Hydrogen peroxide (H2O2) has been implicated in cardiac damage due to ischemia and reperfusion. We adapted an electron microscopic, histochemical method for demonstrating H2O2 produced by isolated cells to isolated, buffer-perfused rabbit hearts. The method involves formation of an electron-dense precipitate when H2O2 reacts with cerium chloride (CeCl3). We perfused hearts retrograde via the aorta with well-oxygenated bicarbonate-buffered solution, followed by one in which bicarbonate was replaced with imidazole (IPSS) to prevent precipitation of bicarbonate and CeCl3. Some hearts were made gl

The pathophysiologic role of myocardial depressant factor as a mediator of circulatory shock.

Myocardial Depressant Factor (MDF) is a small peptide circulating in the blood of all mammalian species tested in a variety of shock states including endotoxic, hemorrhagic, cardiogenic, bowel ischemic, acute pancreatitis, burn, and traumatic shock. MDF is produced by the action of proteolytic enzymes released by the ischemic pancreas. MDF acts to depress myocardial contractility, constrict the splanchnic arteries and impair reticuloendothelial system phagocytosis. Several pharmacologic agents prevent the formation of MDF including membrane stabilizers (e.g., glucocorticoids), protease inhibit

[Myocardial infarction in pregnancy].

We report two cases of pregnant women who developed an acute myocardial infarction. The proposed cause of massive myocardial anterior and interventricular wall infarction of the first patient appears to be bacterial intracoronary thrombus during the course of bacterial endocarditis. Acute insufficiency of the mitral valve caused by rupture chordae tendineae of the anterior mitral left was another complication. Labor contractions occurred on the 13th day of treatment. On the next day the patient gave birth to a healthy child by elective cesarean section. She was also qualified for an operation

Subclinical Cardiac Dysfunction in Children with Iron Deficiency Anemia Assessed by Tissue Doppler and Speckle-Tracking Echocardiography.

OBJECTIVE: Iron deficiency is the leading cause of anemia in childhood and may induce cardiovascular adaptations, including increased heart rate, elevated cardiac output, plasma volume expansion, and myocardial remodeling. Beyond hematologic consequences, reduced iron availability may also affect myocardial structure and function. This study aimed to assess biventricular function in children with iron deficiency anemia using two-dimensional speckle-tracking echocardiography. METHOD: This prospective case-control study included pre- and post-treatment evaluations. Forty children with iron defi
