[Severe anemia-induced type 2 myocardial infarction].

Acute bleeding can cause severe anemia-induced type 2 myocardial infarction in those with significant prior atherosclerotic coronary disease, but not plaque rupture. The authors present four cases where coronarography is presented together with the alarming ECG abnormalities. With their cases, they draw attention that suspected anemia in combination with extensive ST depression may cause type 2 myocardial infarction. In such cases, it is recommended to wait with the otherwise justified double antiplatelet or heparin treatment. In the case of severe anemia (hemoglobin <80 g/L) and extensive ST-…

Repeat CRP measurements and incident myocardial infarction.

BACKGROUND: Inflammation is a causal risk factor for cardiovascular disease. While individuals with elevated levels of inflammation may benefit from targeted therapy, population-level stability of inflammation over time is poorly defined, as is the relationship between repeated measures of inflammatory markers and incident myocardial infarction. C-reactive protein (CRP) is among the best-established inflammatory markers. OBJECTIVES: To examine (i) the stability of CRP over repeat measurements and (ii) the association between patterns of repeated CRP and rates of first-time myocardial infarcti…

Hemorrhagic Myocardial Infarction Implicating Left Ventricular Thrombus.

BACKGROUND: Left ventricular thrombus formation following ST-segment elevation myocardial infarction is typically attributed to blood stasis in an akinetic apex. However, thrombus can form even in the presence of anticoagulation, suggesting alternative mechanisms may be at play. CASE SUMMARY: A 44-year-old man with an anterior ST-segment elevation myocardial infarction underwent primary percutaneous coronary intervention with successful restoration of TIMI flow grade 3. Despite receiving 19,000 U of heparin, a mobile apical thrombus formed. Cardiac magnetic resonance revealed a large intramyo…

Cytomegalovirus Latency Fuels Inflammation Following Myocardial Infarction.

BACKGROUND: Epidemiological studies have consistently associated cytomegalovirus (CMV) seropositivity with adverse cardiovascular outcomes. However, the mechanisms by which cytomegalovirus infection impacts pathophysiology in the heart remain poorly understood. In this study, we sought to dissect how latent murine cytomegalovirus infection impacts cardiac immune cell dynamics at steady-state and during postmyocardial infarction (MI) repair. METHODS: Experimental MI studies were conducted in C57BL/6J mice previously infected with murine cytomegalovirus. In situ inflammatory responses were char…

Cardioprotection During Myocardial Infarction in Diabetic Cardiomyopathy.

UNLABELLED: Patients with diabetes are at an increased risk of diabetic cardiomyopathy (DCM) and acute myocardial infarction (AMI). Protecting the heart against AMI is more challenging in DCM than in nondiabetic hearts. We investigated whether intravenous (i.v.) atorvastatin administration during AMI exerts cardioprotection in DCM as seen in nondiabetic hearts. Sprague-Dawley rats were divided into streptozotocin-induced DCM and normoglycemic control groups. Our model of DCM rats exhibited interstitial fibrosis and cardiac dysfunction at 5 weeks. At this time point, all animals underwent AMI i…

Fever in Myocardial Infarction-Related Cardiogenic Shock.

BACKGROUND: The role of fever in acute myocardial infarction-related cardiogenic shock (AMI-CS) in the contemporary era is not well described. OBJECTIVES: The aim of the study was to describe the epidemiology of fever in AMI-CS and its impact on clinical outcomes. METHODS: Among all AMI-CS patients in the Northwell-Shock registry (n = 1,372), fever was identified via ≥2 temperatures ≥38.0 °C or one ≥38.3 °C. Characteristics, management, and outcomes were compared between patients with and without fever. RESULTS: Fever occurred in 40% of patients at a median of 2 days [0-6] after admission. …

Glucocorticoids impair T lymphopoiesis after myocardial infarction.

The thymus, where T lymphocytes develop and mature, is sensitive to insults such as tissue ischemia or injury. The insults can cause thymic atrophy and compromise T-cell development, potentially impairing adaptive immunity. The objective of this study was to investigate whether myocardial infarction (MI) induces thymic injury to impair T lymphopoiesis and to uncover the underlying mechanisms. When compared with sham controls, MI mice at day 7 post-MI exhibited smaller thymus, lower cellularity, as well as less thymocytes at different developmental stages, indicative of T-lymphopoiesis impairme…

Mortality After Procedural or Spontaneous Myocardial Infarction.

BACKGROUND: It remains unclear whether procedural myocardial infarction (pMI) and spontaneous myocardial infarction (spMI) have a similar impact on prognosis. OBJECTIVES: The aim of this study was to assess mortality after pMI and spMI. METHODS: Patients with chronic coronary syndrome (CCS) and baseline troponin ≤1× the upper reference level (URL) or with acute spMI who underwent percutaneous coronary intervention (PCI) were included. PMI was defined as post-PCI troponin increase >1× URL in patients with CCS. SpMI comprised any acute coronary syndrome with elevated troponin. The 1-year risk …
