Follow-up of Congenital Diaphragmatic Hernia: Need for Routinary Assessment of Acid Gastroesophageal Reflux with pH-metry.

INTRODUCTION: We aim to assess gastroesophageal reflux (GER) in patients treated for congenital diaphragmatic hernia (CDH) and to determine whether a pH-metry investigation should be routinely performed in follow-up. MATERIALS AND METHODS: Twenty-four-hour pH-metry at 1 year was performed in all patients treated for CDH between January 2014 and April 2015 (Group 1). We compared pH-metry results to those of two other groups: children treated for esophageal atresia (EA) (Group 2) and normal babies presenting with typical symptoms (Group 3). All the pH-metric findings were analyzed and compared.

Tools to assess psychological trauma & its correlates in child sexual abuse: A review & current needs in Asia.

Absence of visible physical symptoms and limited capacity to express trauma directly, pose significant challenges in assessment of its exact nature of trauma and its correlates in child sexual abuse. There are numerous assessment tools however, deciding upon the appropriateness is often challenging in Asian socio-cultural and health care set up. A review would provide a ready reference to the practioner regarding the exact clinically utility of the tools and also would guide them in the direction of culture specific modifications. Computerized databases namely Medline, PsycINFO, Health and Psy

Is dynamic gadolinium enhancement needed in MR imaging for the preoperative assessment of scaphoidal viability in patients with scaphoid nonunion?

PURPOSE: To compare the accuracy of dynamic gadolinium-enhanced magnetic resonance (MR) imaging with that of standard MR imaging for assessing the viability of the proximal pole of the scaphoid in patients with nonunion. MATERIALS AND METHODS: The study protocol was submitted to the institutional review board, and the need to obtain additional approval was waived for this retrospective study. Twenty-eight patients (mean age ± standard deviation, 24.3 years ± 6.4) with nonunion of a scaphoid fracture underwent dynamic gadolinium-enhanced MR imaging of the wrist 28 days ± 19 before surgery. Assessing the needs of older gay, lesbian, bisexual, and transgender people: a service-learning and agency partnership approach.

A county agency and a social work research class partnered to conduct a state-mandated needs assessment of older gay, lesbian, bisexual, and transgendered people (GLBT). A survey design with purposive sampling of GLBT people over 60 yielded 38 participants. Findings included that the Internet was a viable means to reach this population and student awareness increased. Areas of greatest unmet need were GLBT-oriented/friendly legal advice, social events, grief and loss counseling, social workers, and assisted living. Some participants perceived existing senior services as unfriendly or hostile t…

Irritant contact dermatitis due to 1-bromo-3-chloro-5,5-dimethylhydantoin in a hydrotherapy pool. Risk assessments: the need for continuous evidence-based assessments.

A physiotherapist working in hydrotherapy presented to occupational health with irritant contact dermatitis. Subsequent investigation revealed that the likely causative agent was 1-bromo 3-chloro 5,5 dimethylhydantoin which was used to disinfect the hydrotherapy pool. A COSHH risk assessment had been performed which failed to take full account of current knowledge and this agent had been introduced into the workplace. The development of adverse health effects among staff and other pool users lead to a review of this risk assessment and eventually a return to less hazardous chlorine-based disin

Standard Nutritional Assessment Tools Are Unable to Predict Loss of Muscle Mass in Patients Due to Undergo Pancreatico-Duodenectomy: Highlighting the Need for Detailed Nutritional Assessment.

BACKGROUND AND METHODS: Pancreatico-duodenectomy (PD) carries significant morbidity and mortality, with very few modifiable risk factors. Radiological evidence of sarcopenia is associated with poor outcomes. This retrospective study aimed to analyse the relationship between easy-to-use bedside nutritional assessment techniques and radiological markers of muscle loss to identify those patients most likely to benefit from prehabilitation. RESULTS: Data were available in 184 consecutive patients undergoing PD. Malnutrition was present in 33-71%, and 48% had a high visceral fat-to-skeletal muscle

Development of an obstetric anesthesia needs assessment framework to inform quality improvement: a pilot evaluation in a high-volume maternity hospital in Vietnam.

BACKGROUND: Maternal mortality rates in Vietnam and other low- and middle-income countries have remained stagnant over the past decade, suggesting opportunity for improvement in maternal care. Obstetric anesthesiologists are uniquely poised to enhance maternal outcomes. This study aimed to develop a needs assessment framework to identify and guide priorities for future quality improvement interventions at a high-volume maternal hospital in Hanoi, Vietnam. METHODS: An obstetric anesthesia needs assessment quality improvement framework was developed through an iterative and collaborative proces…

Impact of a Standardised Screening on the Assessment and Addressing of Spiritual Needs in Inpatient Palliative Care in Germany: A Mixed-Methods Study.

Spiritual Care is a recognised dimension of Palliative Care, yet its provision is often inadequate. This study aimed to improve the identification of patients' spiritual needs using a standardised screening, address these more effectively in everyday Palliative Care, and evaluate potential benefits of this intervention. We conducted a monocentric, mixed-methods, quasi-experimental, pre-post study. For a six-month period (12/2023-05/2024), patients attended by an inpatient Palliative Care consultation service in a tertiary hospital were screened for spiritual needs using the Spiritual Needs Scr

Structural vulnerability as a conceptual framework for transgender health research: findings from a community needs assessment of transgender women of colour in Detroit.

The concept of structural vulnerability explains how systems of oppression drive health inequities by reducing access to survival resources (e.g. food, housing) for marginalised populations. Indicators of structural vulnerability such as housing instability, violent victimisation and poverty are often interconnected and result from intersectional oppression. We sought to demonstrate the utility of the structural vulnerability framework for transgender health research by examining patterns of structural vulnerability indicators among transgender women of colour in Detroit. We conducted latent c
