Distribution of glomerular diseases in Taiwan: preliminary report of National Renal Biopsy Registry-publication on behalf of Taiwan Society of Nephrology.

BACKGROUND: Despite the development of biomarkers and noninvasive imaging tools, biopsy remains the only method for correctly diagnosing patients with unexplained hematuria, proteinuria and renal failure. Renal biopsy has been performed for several decades in Taiwan; however, a national data registry is still lacking until 2013. METHODS: The Renal Biopsy Registry Committee was established within the Taiwan Society of Nephrology in January 2013. A biopsy registry format, including basic demographic data, baseline clinical features, laboratory data, and clinical and pathological diagnosis was d…

Persistent Increase in Serum Ferritin Levels despite Converting to Permanent Vascular Access in Pediatric Hemodialysis Patients: Pediatric Nephrology Research Consortium Study.

Our objective was to examine serum ferritin trends after conversion to permanent vascular access (PVA) among children who started hemodialysis (HD) using tunneled cuffed catheters (TCC). Retrospective chart reviews were completed on 98 subjects from 20 pediatric HD centers. Serum ferritin levels were collected at the creation of PVA and for two years thereafter. There were 11 (11%) arteriovenous grafts (AVG) and 87 (89%) arteriovenous fistulae (AVF). Their mean TCC use was 10.4 ± 17.3 months. Serum ferritin at PVA creation was elevated at 562.64 ± 492.34 ng/mL, increased to 753.84 ± 561.54 ng/

Kidney Diseases and Pregnancy: A Multidisciplinary Approach for Improving Care by Involving Nephrology, Obstetrics, Neonatology, Urology, Diabetology, Bioethics, and Internal Medicine.

This multidisciplinary series is aimed at offering readers many opportunities to appreciate how a clinical and ethical approach to pregnancy has changed in patients with kidney diseases and with related conditions, including diabetes, hypertension, and immunologic diseases. Furthermore, this series aims to focus on the fact that many issues remain unreslved, that there are enormous gaps in knowledge, and that the bioethical approach needs to integrated in the clinical practice, which would allow for a deeper appreciation of different cultural and religious backgrounds. Much still needs to be d

Early integration of palliative care in outpatient nephrology: exploring healthcare professionals' knowledge, attitudes, and communication practices - an exploratory multi-methods study.

BACKGROUND: Patients with advanced kidney disease often experience high symptom burden and complex illness trajectories, highlighting the importance of integrating palliative care into nephrology. However, palliative care remains insufficiently integrated into routine care within nephrology outpatient centers. This study explored healthcare professionals' (HCPs) perspectives on palliative care integration, including knowledge and confidence in identifying patients with palliative care needs as well as perceived barriers and facilitators to implementation. METHODS: An explorative multi-methods…

Communication in living donor kidney transplantation across Italian nephrology and transplant settings: a qualitative descriptive interview study with nephrologists and nurses.

BACKGROUND: Communication is central to living donor kidney transplantation (LDKT). Patients must understand treatment options, donor risks, family implications, and the possibility of pre-emptive transplantation. However, how LDKT communication is perceived and managed by nephrology and transplant professionals across care settings remains insufficiently explored. METHODS: A qualitative descriptive interview study was conducted with nephrologists and nurses involved in communication with patients who may be LDKT candidates. Participants were purposively sampled from Italian low- (1-50 proced…

Nephrology review frequency, associated factors, and outcomes among people with chronic kidney disease G5: a population-based cohort study in England.

BACKGROUND: Nephrologists support decision-making and preparation for kidney replacement therapy (KRT) and conservative kidney management (CKM). However, the frequency of nephrology review among people with chronic kidney disease (CKD) G5 has not been well characterised at the population level. METHODS: We identified adults in England who reached CKDG5 between 2013-2018 using Clinical Practice Research Datalink Aurum, a primary care database covering 16.1% of the UK population, linked to Hospital Episode Statistics. Review frequency was defined as the annualised number of 90-day quarters with…

Histology for nephrology, from pre-implantation to post-transplant kidney biopsy. Lesson learned from ReBIrth (Renal BIopsy for Kidney Transplantation Therapy).

A meeting entitled Renal BIopsy for Kidney Transplantation Therapy (ReBIrth) took place on May 31st, 2022 in Bologna, Italy. The meeting drew together nephrologists, surgeons, and pathologists and recognized as experts in the field of kidney transplantation in Italy. In this paper, we present our experience working with kidney transplants in the current era of immunosuppression therapy. The primary aim is to report the histopathological characteristics of failed kidney allografts after a consensus of experts reviewed the cases on a wholeslide imaging digital platform. Regardless of the cases d

When the COVID-19 pandemic changed the follow-up landscape of chronic kidney disease: a survey of real-world nephrology practice.

BACKGROUND: Patients with chronic kidney disease (CKD) require specialized management. However, the current situation of CKD management is unclear during the coronavirus disease 2019 (COVID-19) pandemic. We aimed to investigate the influence of the COVID-19 on kidney patients' follow-ups. METHODS: In April 2020, we included patients who underwent kidney biopsy from January 2017 to December 2019 in a referral center of China, and then initiated a survey via telephone on different aspects of follow-up during the COVID-19 pandemic. We collected and analyzed demographic data, diagnoses, follow-up

Intermittent cholecalciferol supplementation in children and teenagers followed in pediatric nephrology: data from a prospective single-center single-arm open trial.

Vitamin D deficiency is frequent in pediatric nephrology. The 2017 European guidelines recommend keeping 25OH vitamin D (25-D) levels within the 75-120 nmol/L range, ideally with daily supplementation. Intermittent supplementation with D3 has also been proposed. We aimed to assess the influence of our local protocol of intermittent vitamin D supplementation on the evolution of 25-D levels between baseline and 2 months. VITATOL is a prospective single-center study performed in our tertiary unit in children and teenagers followed for chronic kidney disease (CKD), kidney transplantation, or stabl…

Improved cardiovascular risk factors in pediatric renal transplant recipients on steroid avoidance immunosuppression: A study of the Midwest Pediatric Nephrology Consortium.

Several centers have examined the implementation of immunosuppression protocols that minimize steroid exposure. This study retrospectively examined cardiovascular risk factors in 70 pediatric renal transplant recipients on steroid avoidance-based immunosuppression over three yr compared to matched pediatric patients maintained on chronic corticosteroids. Although higher rates of acute rejection were noted in the steroid-avoidant group (22% vs. 16%, p = 0.034), graft function was similar (67 + 10 mL/min/1.73 m(2) vs. 72 + 12 mL/min/1.73 m(2)) (p = 0.053). The steroid-avoidant group demonstrated

Green nephrology: water and waste - one year in a hemodialysis unit in Northeastern Brazil and reassessment of dialyzer reprocessing from an environmental perspective.

INTRODUCTION: Hemodialysis is one of the most water-intensive and waste-generating therapies in health care, yet its environmental impact remains poorly documented in low- and middle-income countries. This study aimed to estimate water consumption and solid waste generation in a hemodialysis unit in northeastern Brazil that reprocesses dialyzers, comparing the observed scenario with an estimated counterfactual single-use scenario. METHODS: This was a single-center, retrospective observational study. Aggregate institutional data on water consumption, supply procurement, and waste mass were ana
