Clinical application of ipsilateral C7 nerve root transfer for treatment of C5 and C6 avulsion of brachial plexus.

We applied a nerve transfer, using the ipsilateral C7 nerve root to treat the C5 and C6 root avulsion of the brachial plexus. Four patients with C5 and C6 preganglionic injury were operated on with this new technique from 1998-2000. Transfer of the spinal accessory nerve to the suprascapular nerve was simultaneously done in 2 these patients. After a follow-up of 1-2.5 years, the muscle strength of elbow flexors recovered to M4 (Lovett) in all cases, shoulder abduction of >90 degrees with external rotation of 30-40 degrees was gained in two cases, and that of 15-45 degrees with no external rota…

Permanent hand motor deficit and failure of elbow flexion recovery following Oberlin nerve transfer. A retrospective case series.

INTRODUCTION: The Oberlin nerve transfer is widely used to restore elbow flexion after brachial plexus injury. However, donor-site morbidity, particularly permanent motor deficit, has been rarely reported. This retrospective case series describes permanent donor-site morbidity following the Oberlin procedure and explores its association with poor elbow flexion outcomes. METHODS: Nine patients (mean age, 38 years) with upper-type brachial plexus paralysis, including seven C5-C8 injuries, were referred to a tertiary centre after undergoing a primary Oberlin transfer at external institutions. Al…

Precision Oral Medicine: A DPR Segmentation and Transfer Learning Approach for Detecting Third Molar Compress Inferior Alveolar Nerve.

Extraction of the third molar of the mandible is one of the most common oral surgical procedures. Preoperative monitoring and assessment are crucial to mitigate neurological risks. Identifying whether the third molar in the mandible compresses the inferior alveolar nerve still relies on dental professionals, a task that is repetitive and time-consuming. Thus, the primary objective is to utilize dental panoramic radiography for image processing and classify whether the third molar compresses the inferior alveolar nerve, aiming to reduce the demand for CT images in symptom diagnosis and mitigate

Automated assessment and detection of third molar and inferior alveolar nerve relations using UNet and transfer learning models.

Panoramic radiographs (PRs) are widely used in assessing the relationship between the mandibular third molar (MM3) and the inferior alveolar nerve (IAN). The relationship of MM3 and IAN is a critical consideration in oral and maxillofacial surgery due to the risk of nerve injury that can lead to anesthesia in the lower lip or chin. This study aimed to evaluate the performance of different transfer learning models in classifying these relations into four classes after detecting and segmenting the MM3 and the IAN using a new method with the UNet model. 714 PRs from more than 500 patients, repres

Wrist Extension Reconstruction Using Distal Anterior Interosseous to Extensor Carpi Radialis Brevis Nerve Transfer in Brachial Plexus Injuries.

PURPOSE: Reconstruction of wrist extension in C5-C8 root brachial plexus injuries (BPIs) is challenging, because of limited nerve and tendon donors. The purpose of this study was to report outcomes relating to the pronator quadratus branch of the anterior interosseous nerve (AIN) to extensor carpi radialis brevis (ECRB) nerve transfer from three BP surgeons. METHODS: Ten patients with C5-C8 BPI underwent AIN to ECRB nerve transfer. The mean age of the patients was 30.5 ± 15.9 years. All patients were men. The mean time to nerve surgery following initial injury was 4.5 ± 1.2 months. In all pat…

Supercharge End-to-Side Anterior Interosseous-to-Ulnar Motor Nerve Transfer Restores Intrinsic Function in Cubital Tunnel Syndrome.

BACKGROUND: The supercharge end-to-side anterior interosseous nerve-to-ulnar motor nerve transfer offers a viable option to enhance recovery of intrinsic function following ulnar nerve injury. However, in the setting of chronic ulnar nerve compression where the timing of onset of axonal loss is unclear, there is a deficit in the literature on outcomes after supercharge end-to-side anterior interosseous nerve-to-ulnar motor nerve transfer. METHODS: A retrospective study of patients who underwent supercharge end-to-side anterior interosseous nerve-to-ulnar motor nerve transfer for severe cubita…

A clinical and radiographic approach for establishing proper tibial tubercle transfer when using quad active femoral nerve stimulation.

BACKGROUND: The tibial tubercle-trochlear groove measurement (TT-TG), which measures the lateral offset of the tibial tubercle relative to the trochlear groove of the femur, has been utilized as an intraoperative tool to help establish maximum patellofemoral congruency in patients who suffer from patellar instability. We have previously published our approach of establishing how far to transfer the tibial tubercle using intraoperative femoral nerve stimulation in order to achieve congruency from 0-30° of flexion. The technique and clinical outcomes have previously been published in this journa

Long head of the triceps branch transfer to axillary nerve in C5, C6 brachial plexus injuries: anterior approach.

UNLABELLED: Shoulder abduction is a very complex movement and quite important for upper limb function, as more distal functions depend on a stable shoulder, especially in C5, C6 brachial plexus injuries. Various studies in the literature have emphasized the importance of improved functional outcome and shoulder reanimation with concomitant neurotization of suprascapular nerve and axillary nerve in C5, C6 brachial plexus injuries. A number of approaches to axillary nerve transfer in brachial plexus injuries have been reported. The author describes an innovative anterior deltopectoral approach f…

Minimally Invasive Corneal Neurotization: 10-Year Update in Technique Including Novel Donor Transfer of the Great Auricular Nerve.

Corneal anesthesia, caused by lack of corneal innervation, is a rare but devastating condition that can lead to neurotrophic keratopathy, corneal ulceration, scarring, and blindness. Minimally invasive corneal neurotization enables transfer of regional donor sensory nerves to the cornea to provide sensation and ocular protection. The authors provide an update on technical advances and modifications that have refined the surgery over the past 10 years, as well as step-by-step intraoperative videos of corneal neurotization, noting its critical steps, pitfalls, and caveats. The videos illustrate
