Assessing carpal tunnel syndrome with magnetoneurography.

OBJECTIVE: To measure the neuromagnetic fields of carpal tunnel syndrome patients after electrical digital nerve stimulation and evaluate median nerve function with high spatial resolution. METHODS: A superconducting quantum interference device magnetometer system was used to record neuromagnetic fields at the carpal tunnel after electrical stimulation of the middle digital nerve in 10 hands of nine patients with carpal tunnel syndrome. The patients were diagnosed based on symptoms (numbness, tingling, and pain) supported by a positive Phalen or Tinel sign. A novel technique was applied to re

The Presence and Significance of A-Waves-A Retrospective Review of 679 Patients.

PURPOSE: A-waves are late responses that have been reported in healthy individuals and patients with neurologic conditions. The mechanism(s) responsible for their generation and their clinical significance are not fully understood. The aim was to better characterize A-waves. METHODS: A retrospective study was conducted in a high-volume Neurophysiology Department in a tertiary hospital in Melbourne, Australia. Consecutive neurophysiological tests including F-wave studies performed between July 2017 and September 2018 were reviewed to identify A-waves. Patients' characteristics and neurophysiol

Ultrasound Findings in Patients with Normal Nerve Conduction despite Clinical Signs and Symptoms Consistent with Carpal Tunnel Syndrome.

BACKGROUND: Confirmatory methods such as nerve conduction studies are often performed to support a clinical carpal tunnel syndrome diagnosis and to rule out other upper extremity pathologies. Ultrasonography provides another diagnostic option, especially when nerve conduction studies are discordant with history and physical examination. The authors explored the correlation of ultrasound findings with clinical carpal tunnel syndrome diagnosis in patients with normal nerve conduction study results. METHODS: A database of 220 patients with carpal tunnel syndrome was analyzed retrospectively to r

Acute Axonal Motor Neuropathy With Completely Reversible Conduction Failure-Is It Really Axonal?

We present the case of a 24-year-old man with a 3-day history of limb weakness and flaccid tetraparesis, hyporreflexia, and gait difficulties (Hughes grade 3) in the examination. Electromyography at presentation revealed severe amplitude reduction in distal compound muscle action potentials of several nerves without features of demyelination, fulfilling electrodiagnostic criteria for acute axonal motor neuropathy. The patient was treated with immunoglobulin and recovered completely 21 days after symptom onset. Electromyography at this timepoint showed normalization of compound muscle action po

Clinical and Electrophysiological Factors Predicting Prolonged Recovery in Children with Guillain-Barré Syndrome.

OBJECTIVE: To compare clinical and nerve conduction studies (NCS) parameters predictive of outcome in children with acute inflammatory demyelinating polyneuropathy (AIDP) and acute motor axonal neuropathy (AMAN). METHODS: In this prospective observational study, NCS was done on all children at admission and repeated before discharge. Functional status of patients was graded as per Hughes Disability score. These children were followed up till they achieved independent walking. Clinical and NCS criteria were compared between (a) AMAN and AIDP and (b) two subgroups of children with AMAN-those wh

Chronic inflammatory demyelinating polyneuropathy with hypertrophic nerves.

We describe the distinctive features (with images and video) of a case of chronic inflammatory demyelinating polyneuropathy (CIDP) with giant nerves. The main clinical findings were insidious onset, gait ataxia and sensory symptoms. Electrodiagnostic studies showed very slow nerve conduction velocities, multiple conduction blocks, distal CMAP duration increase and absence of F-waves. The protein level in the cerebrospinal fluid was very high. Nerve ultrasound showed swelling of all peripheral nerves outside entrapment sites, with significant variability within different segments of the same ne

Nerve conduction studies in Guillain-Barré syndrome: Influence of timing and value of repeated measurements.

OBJECTIVE: Nerve conduction studies (NCS) are essential to differentiate between demyelinating and axonal subtypes in Guillain-Barré syndrome (GBS). However, it is debated to which extent the delay of NCS after symptom onset and repeated measurements during the disease course influence the diagnostic accuracy. METHODS: We evaluated NCS in 93 patients with a classical GBS applying two widely used criteria (Hadden's and Rajabally's). The initial measurements after symptom onset were compared to follow-up studies where available (n = 43). We analyzed the influence of NCS timing after symptom ons

Differentiation of ulnar neuropathy at the wrist due to ganglion cyst from ulnar neuropathy at the elbow.

OBJECTIVE: Here, we aimed to describe the clinical, electrodiagnostic (EDx) and ultrasonographic (US) findings in a series of patients with ulnar neuropathy at the wrist (UNW) due to compression by a ganglion cyst. We also sought features that differentiate UNW from ulnar neuropathy at the elbow (UNE). METHODS: We reviewed electronic medical records of consecutive patients with UNW caused by ganglion cysts. We compared their clinical, EDx and US findings to findings in our previously reported prospective series of UNE patients. RESULTS: We identified 10 patients with UNW caused by ganglion c

Outcome of carpal tunnel release in patients with normal nerve conduction studies.

BACKGROUND: We evaluated outcome after carpal tunnel release (CTR) in patients with clinically diagnosed carpal tunnel syndrome (CTS) but normal results in nerve conduction studies (NCS), and compared these results with those from a prospective group of patients with NCS-verified CTS. METHODS: Over a 5-year period, we prospectively included 103 patients with clinical CTS. NCS were done at inclusion, with surgeon and patient being kept blind regarding the result. The patients underwent endoscopic CTR. QuickDASH and satisfaction score were recorded preoperatively and 4 months after surgery. 94

N-hexane exposure: a cause of small fiber neuropathy.

A 59-year-old woman presented with progressive paresthesias of all of her limbs for 4 years, associated with neuropathic pain, tingling in the tongue and allodynia, consistent with small fiber neuropathy (SFN). Several systemic symptoms and signs were found on clinical examination and laboratory work-up. Neurological investigations including neurophysiologic test and skin biopsy supported the diagnosis of SFN. Chronic exposure to N-hexane was then disclosed and suspected to be the cause of the disease. Following the discontinuation of chronic N-hexane exposure, the patient had a progressive im

Ionic mechanisms underlying history-dependence of conduction delay in an unmyelinated axon.

Axonal conduction velocity can change substantially during ongoing activity, thus modifying spike interval structures and, potentially, temporal coding. We used a biophysical model to unmask mechanisms underlying the history-dependence of conduction. The model replicates activity in the unmyelinated axon of the crustacean stomatogastric pyloric dilator neuron. At the timescale of a single burst, conduction delay has a non-monotonic relationship with instantaneous frequency, which depends on the gating rates of the fast voltage-gated Na+ current. At the slower timescale of minutes, the mean val

Neuromuscular ultrasound in the evaluation of inclusion body myositis.

This case report illustrates the role of using neuromuscular ultrasound to diagnose inclusion body myositis (IBM) in a patient who was previously diagnosed with polymyositis. Emerging studies have demonstrated the accuracy of MRI in detecting the selective involvement of the flexor digitorum profundus muscle in those with IBM. However, there have been only few reports on the use of ultrasound in diagnosing this condition. Our case demonstrates the benefit of using this ultrasonographic approach, which is simple, clear, inexpensive, painless and radiation-free, and provides another modality to

Sensory response following knee joint damage in rabbits.

BACKGROUND: Altered sensory information arising from damaged knee joint structures has been hypothesized as a contributing factor to persistent muscle dysfunction following injury. METHODS: Composite femoral nerve sensory signal was measured in 24 rabbits randomly allocated (8 per group) to receive surgical anterior cruciate ligament (ACL) transection with or without autograft reconstruction or nothing (control). Two-weeks after the intervention composite afferent signals were recorded from the femoral nerve. Side-to-side ratios (surgical side vs contralateral healthy side) for peak femoral n

[Neuropathy and unmyelinated epidermal nerve fibers].

Detection of unmyelinated epidermal nerve fibers allows objective, pathologic, and minimally invasive assessment of small myelinated A-delta and unmyelinated C primary afferent fibers. This procedure is especially valuable for diagnosing small fiber neuropathy; this condition is characterized by sensory symptoms and pain in the lower extremities but standard nerve conduction fails to show abnormalities in this condition. Detection of epidermal nerve fibers is useful not only for confirming the diagnosis of neuropathies affecting small-diameter sensory fibers, but also in advanced clinical care
