Mandibular neuralgia due to anatomical variations.

In our large collection of macerated human adult skulls and disarticulated skulls of young individuals we found cases with an extremely larg … ession. The result is possible pain, especially during chewing, and may finally create a trigeminal neuralgia. Similar symptoms could be provoked by a foramen pterygospinale or ovalis canal replacing the foramen ovale.

[Amyotrophic neuralgia: review of 37 cases].

INTRODUCTION: Amyotrophic neuralgia is characterized by pain of acute or subacute onset, accompanied by weakness and occasionally by atrophy of the brachial muscles, of unknown origin. We present our experience over the past 20 years. PATIENTS AND METHODS: We made a retrospective review of 37 patients with the above diagnosis, following the criteria of other series of such cases published in the literature. RESULTS: Twenty four of the patients were men and thirteen were women. The average age was 38 (11 to 71). A relevant clinical history was recorded in 9 cases; infection (5), surgery (4), …

Postherpetic neuralgia in immunocompetent elderly people.

The most menacing complication of herpes zoster in immunocompetent elderly people is chronic pain or postherpetic neuralgia (PHN). The cardinal epidemiological feature of PHN is its striking relationship to aging. Among zoster patients over 60 years old, estimates of the occurrence of PHN, defined as pain 1 month after rash onset, vary from 27 to 68%. The pathogenesis of PHN is incompletely understood but seems to involve varicella-zoster virus (VZV)-induced damage of peripheral afferent neurons and resultant changes in central afferent neurons and efferent pain-modulating neurons. PHN improve…

Postherpetic neuralgia: irritable nociceptors and deafferentation.

Postherpetic neuralgia (PHN) is a common and often devastatingly painful condition. It is also one of the most extensively investigated of the neuropathic pains. Patients with PHN have been studied using quantitative testing of primary afferent function, skin biopsies, and controlled treatment trials. Together with insights drawn from an extensive and growing literature on experimental models of neuropathic pain these patient studies have provided a preliminary glimpse of the pain-generating mechanisms in PHN. It is clear that both peripheral and central pathophysiological mechanisms contribut…

Gamma knife radiosurgery for trigeminal neuralgia.

Radiosurgery is one of the surgical treatments of trigeminal neuralgia. Through precise irradiation of the proximal trigeminal nerve identified on high-resolution imaging, pain relief can be achieved after a short latency interval. This image-guided approach has been useful for both patients with persistent pain after other surgeries, and as a primary surgical option. A minimum dose of 70 Gy delivered with the gamma knife has been associated with low risk for facial numbness, and no other morbidity. Management of trigeminal neuralgia without an incision, transfacial needle placement or nerve s…

Cardiac arrest caused by trigeminal neuralgia.

A 67-year-old man with a 12-year history of trigeminal neuralgia experienced multiple fainting episodes preceded by right facial pain. One episode resulted in cardiac arrest with successful resuscitation. Pacemaker insertion prevented further episodes of syncope despite the occurrence of pain. The fainting episodes and cardiac arrest are believed to be unusual manifestations of trigeminal neuralgia.

Occipital neuralgia manifesting as orofacial pain.

This is a case report and brief review of the literature on occipital neuralgia presenting as dental pain. A patient with a chief complaint of long-standing pain in the maxillary right posterior quadrant was evaluated. Dental examination demonstrated the pain was not of odontogenic origin. The patient was referred to a neurologist who was a chronic pain specialist and was diagnosed with a rare neurologic disorder, occipital neuralgia referring to the facial region. After conservative treatment, local nerve blocks, and physical therapy, the patient reported a dramatic improvement of symptoms an…

Topical lidocaine gel relieves postherpetic neuralgia.

Postherpetic neuralgia (PHN) following herpes zoster is a common and disabling neuropathic pain syndrome. In a double-blind, three-session study, 5% lidocaine gel or vehicle was applied simultaneously to both the area of pain and to the contralateral mirror-image unaffected skin. In the local session, lidocaine gel was applied to the painful skin area. In the remote session, lidocaine gel was applied to mirror-image skin. In the placebo session, vehicle was applied bilaterally. For cranial PHN, gel was applied without occlusion for 8 hours. For limb or torso PHN, gel was applied under occlusio…

Acupuncture vs carbamazepine in trigeminal neuralgia.

UNLABELLED: Type 2 diabetes mellitus (T2DM) is a metabolic disorder characterised by impaired insulin signalling, chronic hyperglycaemia, and associated metabolic dysfunction. Plant-derived bioactive compounds are increasingly explored as potential therapeutic agents for its management. To investigate the anti-diabetic effect and underlying mechanism of Myrcene, in streptozotocin (STZ) + high-fat diet (HFD) induced diabetes in mice, male C57BL/6 mice were administered HFD for eight consecutive weeks, followed by low-dose STZ injections (60 mg/kg) during the succeeding week to develop diabetes.

Altered cutaneous sensation in trigeminal neuralgia.

Noninvasive quantitative somatosensory tests were used to measure sensory perception thresholds on the faces of 26 patients with idiopathic trigeminal neuralgia. The affected divisions were determined by the presence of trigger zones. Elevations of thresholds of sensations subserved by both large- and small-diameter fibers were found both in the affected and in the unaffected adjacent divisions when compared with the healthy side. The findings are interpreted as further evidence of combined peripheral and central pathologic conditions in idiopathic trigeminal neuralgia.

Etiologies of tic douloureux: trigeminal neuralgia.

Tic doloreaux, commonly known as trigeminal neuralgia, has bewildered researchers for hundreds of years as to the exact etiology of its pain mechanisms. Due in large part to the intense nature of the tic-like pain, this disease has drawn a lot of attention. Two divergent viewpoints, central verses peripheral, provide insight into possible mechanisms. This paper provides an overview of the different theories postulated as the causative factors so that the practitioner may better be able to diagnosis and treat the syndrome.
