The aim of the study was to conduct a rapid assessment of breastfeeding knowledge amongst health workers in an area of high HIV prevalence. A cross-sectional survey using semi-structured questionnaires and problem-based scenarios was carried out. Responses were compared to those recommended in the World Health Organization (WHO) Breastfeeding Counselling Course. The setting was a rural area of KwaZulu Natal, with a population of 220 000 people. At the time of the study approximately 36 per cent of pregnant women were HIV-infected and no programme to prevent mother-to-child transmission was in

The aim of this research study was to map nursing diagnoses and interventions documented and observed in a neonatal ICU to the Clinical Care Classification system and to validate the translation of these mapped diagnoses and interventions into Persian. This descriptive research used directed content analysis to map diagnoses and interventions to the Clinical Care Classification. Documentation reports of nursing care were extracted from a paper-based documentation system for infants hospitalized in April, May, and June 2011. Observations of care were conducted and compared with documentation to

Alagille syndrome is a rare and complex pleiotropic multisystem disorder caused by an autosomal dominant genetic mutation of JAG1 (90%) and NOTCH2 (1%-2%) genes located on the short arm of chromosome 20. This case is reported as per the CA se RE ports (CARE) guidelines (2013). A 14-year-old boy who is a known case of chronic cholestatic liver disease of neonatal onset, was diagnosed with Alagille syndrome as evident from a NOTCH 2 mutation in genetic analysis and paucity of intrahepatic bile ducts on biopsy. He presented with portal hypertension, growth failure, and persistent hyperbilirubinem

This article examines how the Frontier Nursing Service (FNS) utilized nurse-midwives to respond to antepartum emergencies such as preterm birth, eclampsia, malpresentation, and hemorrhage in the women of Appalachia in the years 1925 to 1939. Particular attention is given to the preparation that nurse-midwives received during their midwifery education to prevent and respond to emergencies. Using traditional historical research methods and primary source material from the FNS papers in the Special Collections, University of Kentucky Libraries, Lexington, Kentucky, this article describes the nurs

BACKGROUND: Indwelling venous catheters, including peripheral intravenous catheters (PIVCs), are vital in pediatric care for delivering medications and fluids. However, catheter fractures, though rare (incidence 0%-2.1%), pose serious risks such as pulmonary embolism or cardiac arrhythmias if fragments migrate. While central venous catheter fractures are well-documented, PIVC fractures are underreported despite their widespread use.
CASE PRESENTATION: This report details two pediatric cases of PIVC fractures. In the first, a 1-day-old female neonate experienced a fractured left axillary cathe

The appropriate pain management in neonates and children is lacking. Factors that prevent the execution of proper pain relief vary from center to center. We studied the factors responsible for it in a surgical unit. We conducted a survey at a tertiary-level institute among the resident doctors and nursing staff by means of an informal questionnaire analyzing their basic knowledge. The questions pertained mainly to pain assessment, analgesic usage, role of opioids, and formal training, and the responses so obtained were analyzed under these four headings. Seventy-three percent (22/30) of the re

BACKGROUND: Empathy fatigue represents a significant concern among healthcare professionals working in critical care settings, including intensive care units, emergency departments and neonatal units. It is an established fact that constant exposure to emotionally intense situations may lead to a state of emotional exhaustion, a reduction in job satisfaction and a compromise in the quality of patient care. Despite the prevalence of this phenomenon, there remains a paucity of profession-specific tools with which to assess empathy fatigue in these high-stress environments.
AIM: The objective of

Fetal head impaction (IFH) occurs when the presenting part becomes wedged in the maternal pelvis during a cesarean section, requiring additional maneuvers to complete birth. As cesarean rates have risen, so has IFH, estimated to occur in 1.5% of all births annually in the United States. Maternal complications include uterine extension, hemorrhage, bladder injury, and intensive care unit admission. Neonatal complications include low cord pH, skull fracture, intracranial hemorrhage, hypoxic-ischemic encephalopathy, and death. These outcomes are not inevitable. Management technique, clinician edu

The earlier a child with biliary atresia undergoes surgery after diagnosis, the better the prognosis. However, newborns often present with additional symptoms, most commonly pneumonia, which complicate in stabilizing the child's internal environment preoperatively, challenges anaesthetic management during surgery and hampers postoperative recovery. In current clinical practice, nursing care tends to focus more on symptomatic treatment rather than on a comprehensive pre- and postoperative assessment. This paper aimed to summarize the perioperative nursing experience of a neonate with biliary at

Congenital diaphragmatic hernia (CDH) is a developmental defect in the diaphragm that allows abdominal viscera to herniate into the thoracic cavity. Pulmonary hypoplasia and pulmonary hypertension are consequences of this disease process. The incidence is approximately 2.4-4.1/10,000 births, and survival rate is estimated at 70-90 percent. To avoid potentially devastating delays in care, it is crucial that neonatal nurses and care providers in both tertiary and nontertiary care centers be familiar with the pathogenesis of CDH and the standard of care for initial stabilization of the neonate. BACKGROUND: Preparing families and preterm infants for discharge is relatively unstructured in many UK neonatal units (NNUs). Family-centred neonatal care and discharge planning are recommended but variable.
DESIGN AND PARTICIPANTS: Qualitative interviews with 37 parents of infants in NNUs, and 18 nursing staff and 5 neonatal consultants explored their views of discharge planning and perceptions of a planned family-centred discharge process (Train-to-Home). Train-to-Home facilitates communication between staff and parents throughout the neonatal stay, using a laminated train and parent bookle

INTRODUCTION: The rise in fentanyl use during pregnancy has created new challenges in caring for women with opioid use disorders (OUD) and their infants with neonatal abstinence syndrome or neonatal opioid withdrawal syndrome (NAS/NOWS). Despite complexities in treating opioid-affected dyads, little research exists on healthcare workers' perspectives regarding fentanyl's impact on perinatal and neonatal care.
OBJECTIVES: Explore dynamic challenges fentanyl has brought to the care of perinatal women with OUD and their infants experiencing NAS/NOWS from healthcare providers' perspectives.

OBJECTIVE: To determine if a policy change that limited the ability of parental caregivers to leave the postpartum unit during the COVID-19 pandemic influenced neonatal abstinence syndrome (NAS) scores, admissions to the NICU for NAS treatment, and length of stay (LOS) on the nursing unit.
DESIGN: Retrospective chart review.
SETTING/LOCAL PROBLEM: During the pandemic, a change in policy limited parental caregivers from leaving the nursing unit.
PARTICIPANTS: Neonates being screened for NAS during the period before the policy change from April 2, 2019, through April 1, 2020 (n = 44), and the

The CRYO-ROP study confirmed the success of treatment for ROP and made screening mandatory. National based screening has been influenced by the varied incidence of disease in developed and developing countries. Most ophthalmologists in developed countries screen infants born between 1000 and 1500 g and between 28 and 31 weeks gestation post menstrual age. The 1984 classification has been updated to highlight the importance of plus disease. The ETROP study findings have resulted in earlier treatment and elevated the importance of screening. Measures such as nesting may help to reduce infant dis

Congenital cardiac defects and the clinical symptoms they exhibit are affected intimately by the relation they have with pulmonary circulation. Cardiac lesions that increase pulmonary blood flow often occur clinically with signs and symptoms of congestive heart failure, including hepatomegaly, tachycardia, diaphoresis, and feeding difficulties. However, in the neonatal period, pulmonary vascular resistance often remains elevated, decreasing the pulmonary blood flow and, therefore, severity of symptoms. Cardiac lesions that reduce pulmonary blood flow often manifest early in life with clinical

We investigated the staff in our neonatal intensive care unit (NICU) for the presence of methicillin-resistant Staphylococcus aureus (MRSA) in the nasal cavity, and then applied intranasal povidone-iodine cream to the physicians and nurses working on the unit. Prior to the application of povidone-iodine cream, the isolation rate of S. aureus from the nasal cavity was 30% for the physicians and nurses in contact with NICU patients (contact group), not significantly different from the 33.3% rate for other hospital staff (control group). The isolation rate for the contact group decreased to 10.5%

Preliminary observations have shown that fentanyl citrate, a potent narcotic, is helpful during labor without undue side effects. This randomized prospective investigation compared the patient-controlled administration of fentanyl with that of administration by nurses on request. Eighty healthy women beginning active labor (cervical dilation 4 cm) at term were assigned to receive fentanyl intravenously by either patient-controlled administration (n=37) or nurse administration on demand (n=43). Pain intensity measurements during early and late labor revealed the degree of analgesia to be the sa
