Hormonal secretion and quality of life in Nelson syndrome and Cushing disease after long acting repeatable octreotide: a short series and update.

Clinical management of persistent adrenocorticotropin hormone (ACTH) excess in Nelson syndrome (NS) and Cushing disease (CD) remains a challenge. Somatostatin and its analogs as octreotide decrease ACTH secretion through somatostatin receptors of pituitary cells. To our knowledge, there are no reports on the effect of long-acting repeatable octreotide (oct-lar) on hormonal secretion and quality of life in patients with NS and CD who failed conventional therapy. Herein, we describe the effects of treatment with oct-lar (20 mg/month intramurally) in 1 woman with NS and 2 women with persistent CD…

Case report: octreotide as an adjunct to embolisation in the management of recurrent bleeding upper gastrointestinal metastases from primary renal cell cancer.

The recommended treatment for patients with severe bleeding from upper gastrointestinal (GI) metastases is embolisation. We report a case in which despite adequate embolisation major haemorrhage from renal cell carcinoma (RCC) gastric metastases continually recurred. During a severe bleed refractory to embolisation octreotide was used to control and prevent further bleeding. No further episodes of severe haemorrhage occurred over the following 23 months since starting treatment. Octreotide has been observed to be effective both in the acute management of persistent haemorrhage and in preventio…

The role of combined imaging in metastatic medullary thyroid carcinoma: 111In-DTPA-octreotide and 131I/123I-MIBG as predictors for radionuclide therapy.

PURPOSE: The medical treatment of metastatic medullary thyroid carcinoma (MTC) is still questionable. The aim of this study was to evaluate a combined imaging protocol using 111In-DTPA-octreotide and 131I/123I-MIBG to decide whether targeted radiotherapy would be useful, and which radiopharmaceutical (90Y-DOTATOC or 131I-MIBG) would be more effective. METHODS: Eight patients (four men, four women; mean age 61 years) with metastatic MTC were included. Treatments were performed with 3,330 MBq 90Y-DOTATOC at 6-week intervals, or 11.1 GBq 131I-MIBG with a minimum interval of 3 months. RESULTS: T…

Octreotide mitigates cisplatin-induced nephrotoxicity through coordinated modulation of the miR-34a/SIRT1 axis and downstream apoptotic, inflammatory, and fibrotic pathways.

AIMS: Cisplatin (CIS) is a widely used chemotherapeutic agent, but its clinical utility is limited by dose-dependent nephrotoxicity. Octreotide (OCT), a somatostatin analog, exhibits anti-inflammatory, antioxidant, and anti-apoptotic properties in several experimental models, yet its therapeutic potential against cisplatin-induced nephrotoxicity remains largely unexplored. This study, therefore, investigated the nephroprotective potential and the underlying molecular mechanisms of octreotide against cisplatin-induced nephrotoxicity, and asked whether protection depends on the timing of adminis…

Imaging Somatostatin Positive Tumors with Tyr3-Octreotate/Octreotide Conjugated to Desferrioxamine B Squaramide Radiolabeled with either Zirconium-89 or Gallium-68.

Radiolabeled derivatives of Tyr3-octreotide and Tyr3-octreotate, synthetic analogues of the peptide hormone somatostatin, can be used for positron emission tomography (PET) imaging of somatostatin receptor expression in neuroendocrine tumors. In this work, a squaramide ester derivative of desferrioxamine B (H3DFOSq) was used attach either Tyr3-octreotide or Tyr3-octreotate to the metal binding ligand to give H3DFOSq-TIDE and H3DFOSq-TATE. These new peptide-H3DFOSq conjugates form stable complexes with either of the positron-emitting radionuclides gallium-68 (t1/2 = 68 min) or zirconium-89 (t1/…

Pituitary apoplexy causing spontaneous remission of acromegaly following long-acting octreotide therapy: a rare drug side effect or just a coincidence.

Pituitary apoplexy is characterized by abrupt onset of haemorrhage or non-haemorrhagic infarction of a pituitary adenoma. The clinical features include acute onset severe headache, visual field defects, meningeal irritation, ophthalmoplegia and hypopituitarism. The pituitary apoplexy may be clinically silent in ∼25% of patients. We report a case of acromegaly due to pituitary macroadenoma. The patient was started on long-acting octreotide therapy. On 3-month follow-up, the patient showed clinical and biochemical remission and the magnetic resonance imaging (MRI) of the brain showed subclinical…

Correlation between a short-term intravenous octreotide suppression test and response to antrectomy in patients with type-1 gastric neuroendocrine tumours.

BACKGROUND: Type-1 gastric neuroendocrine tumours (NETs) arise in some patients with chronic hypergastrinaemia secondary to autoimmune atrophic gastritis. Patients with small tumours are usually managed conservatively, because their prognosis is very good. However, larger tumours may require surgical intervention. Many type-1 gastric NETs regress following antrectomy because this removes the source of hypergastrinaemia. However, some tumours do not regress following antrectomy and additional surgery may be required. An octreotide suppression test has been previously suggested as a means to ass…

Mesenteric Fibromatosis Mimicking Metastasis of Neuroendocrine Tumor on 18 F-AlF-NOTA-Octreotide PET/CT and Contrast- Enhanced MRI : A Case Report.

A 52-year-old man with a prior history of resected G2 pancreatic neuroendocrine tumor presented with a newly identified abdominal mass during 17-month follow-up. Contrast-enhanced MRI depicted a heterogeneously hyper-enhancing jejunal mesenteric lesion, highly suggestive of metastatic disease. ¹⁸F-AlF-NOTA-Octreotide PET/CT revealed mild tracer accumulation within the lesion, which was initially interpreted as NET metastasis. Surgical pathology ultimately confirmed mesenteric fibromatosis. This case emphasizes an important diagnostic trap, as benign fibromatosis can mimic NET metastasis on bot…

Somatostatin-receptor avidity of pancreatic neuroendocrine tumor thrombus in porto-caval venous systems on 99mTc-Octreotide and posttherapeutic 177Lu-DOTA-TATE scans.

A 62-year-old woman with a history of abdominal pain presented with multiple hepatic lesions and dilatation of portal, splenic and superior mesenteric veins on the magnetic resonance imaging referred for a 99mTc-octreotide scan. Accordingly, similar octreotide-avid lesions were found as well as an uptake in the epigastric region conforming to the anatomy of the portocaval venous system, compatible with a tumor thrombosis. Then, the patient underwent two cycles of therapy with 177Lu-DOTA-TATE, on that the same appearance was observed. The uptake in the tumor thrombus remained somewhat unchanged…
