Eight-and-a-half syndrome: a rare presentation.

Eight-and-a-half syndrome is a rare entity characterised by conjugate horizontal gaze palsy, ipsilateral internuclear ophthalmoplegia and ipsilateral lower motor neuron type facial palsy. It is due to a lesion affecting median longitudinal fasciculus, paramedian pontine reticular formation and facial nerve fascicle on the same side at the level of pons. The diagnosis is easily missed as it needs detailed ocular movement examination. It is mainly caused due to infarction or demyelinating conditions. We are reporting an interesting case of a 54-year-old man with right-side eight-and-a-half syndr

A model-based study of internuclear ophthalmoparesis and ocular-motor fatigue in multiple sclerosis.

Internuclear ophthalmoparesis (INO) in multiple sclerosis (MS) is due to demyelination of the medial longitudinal fasciculus (MLF). INO is typically modeled as an increased peak-velocity and peak-acceleration ratio of abducting to adducting eye (pulse-size ratio, PSR). PSR can be affected by fatigue during prolonged ocular-motor tasks (ocular-motor fatigue). We propose that an important component of horizontal disconjugacy in INO is due to a delayed delivery of the saccadic pulse to the adducting eye (pulse-time delay, PTD). We expanded a control-system model to account for both abnormal PSR a

[Parinaud syndrome caused by corpus pineale tumour in a 14-year-old boy].

Parinaud syndrome (PS) can manifest in a pineal tumour. Major components of PS include restriction of gazing upwards, light-near dissociation and convergence retraction nystagmus. A 14-year-old boy presented with diplopia and restricted ability to gaze upwards. The objective examination revealed signs, which were compatible with the major manifestations of PS. A magnetic resonance scan (MRI) of cerebrum indicated pinealoma, and a pathological examination identified the lesion as a germ cell carcinoma. The patient received chemotherapy and stereotactic radiosurgery. The ophthalmic symptoms impr

Early ophthalmologic features of Parkinson's disease: a review of preceding clinical and diagnostic markers.

Non-motor symptoms in Parkinson's disease are an important cause of morbidity and may even precede the onset of the motor features of the disease. Visual abnormalities are among the most frequent non-motor symptoms observed during the early stages of the disease. Some of the visual symptoms of Parkinson's disease can likely be explained by the presence of dopaminergic neurons within the retina, where the progressive loss of dopamine and the accumulation of α-synuclein within the retinal layers leads to visual dysfunction, while some are caused by abnormalities in cortical visual processing. Ma

Nystagmus Does Not Limit Reading Ability in Albinism.

PURPOSE: Subjects with albinism usually suffer from nystagmus and reduced visual acuity, which may impair reading performance. The contribution of nystagmus to decreased reading ability is not known. Low vision and nystagmus may have an additive effect. We aimed to address this question by motion compensation of the nystagmus in affected subjects and by simulating nystagmus in healthy controls. METHODS: Reading speed and eye movements were assessed in 9 subjects with nystagmus associated with albinism and in 12 healthy controls. We compared the reading ability with steady word presentation an

A case of double depressor palsy followed by pursuit deficit due to sequential infarction in bilateral thalamus and right medial superior temporal area.

BACKGROUND: We present a unique case of a patient who suffered two rare events affecting the supranuclear control, first of the vertical and second of the horizontal eye movements. The first event involved bilateral thalamic infarcts that resulted in double depressor palsy. The second event occurred 1 year later and it involved supranuclear control of horizontal eye movements creating pursuit deficit. CASE PRESENTATION: A 47-year-old male presented with complaints of diplopia upon awakening. He had atrial fibrillation, mitral valve regurgitation, aortic valve regurgitation, and a history of s

Unilateral Eye Blinking Arising From the Ictal Ipsilateral Occipital Area.

We report on an 18-month-old boy with unilateral left eye blinking as a single ictal manifestation without facial twitching. The clinical onset of this phenomenon was first recorded (as an occasional event) at age 3 months, and it was overlooked. By age 6 months, the child's blinking increased to almost daily occurrence in clusters: during blinking the infant showed intact awareness and occasional jerks in the upper limbs and right leg. A video-electroencephalography (video-EEG) documented clinical correlation with a focal pattern arising from the left occipital region, and brain magnetic reso

Alternating skew deviation due to hemorrhage in the cerebellar vermis.

A 76-year-old Japanese woman with essential hypertension and diabetes mellitus abruptly presented with nausea, dizziness, an occipital headache, truncal ataxia, gaze-evoked nystagmus and alternating skew deviation (ASD) with abducting eye hypertropia. Cranial computed tomography demonstrated hemorrhage in the cerebellar vermis and its vicinity. These symptoms gradually resolved within three weeks. This is the first reported case of ASD secondary to cerebellar hemorrhage without hydrocephalus. The vertical misalignment of the eyes during the right-sided gaze was consistently larger than during

Unusual monocular pendular nystagmus in multiple sclerosis.

Two unusual cases of monocular pendular nystagmus in patients with multiple sclerosis are reported. One patient showed regular horizontal oscillations of the right eye in abduction, associated with right abduction paresis. The second patient had a similar abnormal eye movement of the left eye in adduction, with partial left internuclear ophthalmoplegia. Such eye position-dependent monocular pendular nystagmus provides new insights into pathogenic mechanism for acquired pendular nystagmus. Different mechanisms are discussed such as the combination of paresis and commonly accepted hypothesis of

Immunological mechanisms in opsoclonus-myoclonus associated neuroblastoma.

Opsoclonus-myoclonus syndrome is a rare paraneoplastic disorder, often associated with malignancies including neuroblastoma (NB), the most common solid tumor of childhood derived from the sympathetic nervous system. The pathogenesis of this syndrome is still undefined but is suspected to be the result of an autoimmune response. In this respect, different autoantibodies binding to neurons or cerebellar Purkinje cells have been detected in OMS-associated NB. In addition, immunohistochemical analysis of NB affecting children with OMS demonstrated the presence of interstitial or perivascular lymph

Eye movement involvement in Parry-Romberg Syndrome: a clinicopathologic case report.

We report the case of a 38-year-old woman who developed a progressive bilateral disease in which the eye motility disorder-diplopia-is the outstanding feature over a period of 12 years. The muscle biopsy of the medial rectus muscle did not show any trace of striated muscle. To the best of our knowledge, this is the first pathological report in an affected extraocular muscle of a patient with Parry-Romberg syndrome (PRS). Previous rare reports of diplopia in PRS have been attributed to enophthalmos, progressive atrophy of the orbit, ocular motor nerve dysfunction, or mechanical restrictions.

Treatment of the jaw-winking syndrome.

BACKGROUND: Marcus Gunn described for the first time a syndrome consisting of unilateral, congenital ptosis, and rapid exaggerated elevation of the ptotic eyelid during movement of the mandible to the contralateral side. METHODS: Here, clinical findings from the management of 20 patients with jaw-winking syndrome have been reviewed. Preoperative measurement of the ptotic degree and the levator function by Berke's method, and marginal limbal distance were all evaluated. Moreover, the amount of winking found in the upper eyelid on primary gaze was graded on a scale from I to III. The operation

Unilateral reverse ocular bobbing caused by tuberous sclerosis.

A female aged 12 months with developmental delay and left-sided reverse ocular bobbing (rapid deviation of the eye upward and a slow return to the horizontal position) was observed. At birth, an upper left gingival mass, pathologically diagnosed as a benign granular cell tumour, was removed. On computed tomography (CT), left middle cerebral pedincular and midpontine lesions were seen. Magnetic resonance imaging revealed additional white matter hamartomas, corticosubcortical tubers, and subependymal nodules. The patient was re-examined at 36 months. Her general developmental quotient was equiva

Three-dimensional eye movement analysis of superior oblique myokymia.

PURPOSE: To confirm the idea that sole contraction of the superior oblique muscle causes attacks of superior oblique myokymia (SOM). DESIGN: Observational case report. METHODS: A 43-year-old man presented with episodic monocular oscillatory eye movements. Three-dimensional eye movements were recorded using the magnetic search coil system and analyzed as rotation vector. The attacks of SOM consisted of intorsional, depressive, and abductive fast phases and their exponential decaying slow phases. RESULTS: Average direction of rotation axis for the fast phases while fixating straight ahead was

Fixation dysfunction with intermittent saccadic intrusions managed by yoked prisms: a case report.

BACKGROUND: A 44-year-old woman came to us with a chief symptom of "jumping letters side-to-side, which is most noticeable while reading." The onset occurred after she had experienced a closed head traumatic brain injury 3 years earlier. Several neuro-ophthalmologists diagnosed a fixational instability secondary to saccadic intrusions and prescribed Gabapentin, which provided minimal relief. METHODS: The term saccadic intrusions refers to an inappropriate saccade with a disrupting effect on fixation. Our examination revealed a myopic anisometropia. Motility testing confirmed saccadic intrusio

Opsoclonus in a patient with cerebellar dysfunction.

After two days of malaise, headache, nausea, and vomiting, a 26-year-old man suddenly developed opsoclonus and stance and gait ataxia, without myoclonus. Having excluded a paraneoplastic etiology, we assumed that the disorder was probably related to a viral infection. Spontaneous resolution occurred in about two months. Opsoclonus became flutter dysmetria and then resolved. Saccadic eye movement recording disclosed the occurrence of hypermetria, increased velocity, and delayed latency, which also resolved. In this patient, the correspondence between clinical and ocular motor abnormality course
