Paget's disease of bone is characterized by primary osteoclastic dysfunction and prolonged treatment with conventional medications including calcitonin and etidronate, results in a number of patients becoming refractory to treatment. We have evaluated the effectiveness of three dosage regimes of aminohydroxypropylidene bisphosphonate (pamidronate) in 15 patients with extensive Paget's disease who had become refractory to conventional therapy. Nine patients had pamidronate (intravenous infusion of 30 mg over 4-5 hours at monthly intervals) for 6 months. A further four patients received 30 mg of

Recent findings suggested a possible role for pinhole collimator scintigraphy to differentiate between metastases, infection and fractures of the vertebrae. The aim of this study was to verify these findings and to ascertain whether we could identify any other specific scintigraphic patterns. The study group consisted of 58 patients. A metastatic pattern was obtained in 18 patients, while 16 showed the compression fracture pattern, 3 the tuberculosis pattern and 1 the pyogenic infection pattern. The study was considered normal in 4 patients and showed a nonspecific pattern in 4. Sensitivity wa

The CT and magnetic resonance (MR) findings of Paget disease of the calvaria and facial bones are described and compared with one another. The sites of dense, woven bone, myeloid marrow, and background Paget matrix can be clearly identified. A rare case of Paget sarcoma (osteogenic sarcoma) of the facial bones is also presented. The distinction between this sarcoma and the Paget bone was clearer on CT than on MR. This presumably is because the bone is directly seen on CT and only indirectly imaged on MR.

Undemineralized methacrylate embedded bone biopsies and other bone specimens can be processed much more rapidly by application of acidified 2,2-dimethoxypropane (DMP) dehydration, which requires two hours, than by traditional graded ethanol dehydration, which requires at least four days. This shortened processing time is valuable when biopsy results are urgently needed to detect osteomalacia or to determine bone features prior to possible parathyroidectomy. We have processed over 200 bone specimens with DMP and have compared DMP dehydration to graded ethanol dehydration in 11 biopsies in which

We have presented three patients who had bony expansion due to metastatic carcinoma of the prostate demonstrated by roentgenography and bone scintigraphy. Bony expansion must be differentiated from the blooming phenomenon which may occur in areas of high radioactivity. Bony expansion is not specific for Paget's disease, but also occurs in many other benign and malignant conditions.

The alkaline phosphatases present in the human tissues liver, kidney, intestine, placenta and a serum from a patient with Paget's disease of bone have been purified to apparent homogeneity by affinity elution from a phosphonic acid-Sepharose derivative. Polyacrylamide gel electrophoresis in SDS gave subunit molecular weights ranging from 74 000 for the enzyme form placenta to 95 800 for the enzyme from kidney. The purified native and desialylated enzymes have been characterized by agar electrophoresis and isoelectric focusing. All five of the native enzymes behaved differently but the desialyl

The neurological features of 96 patients whose skull and spinal X-rays showed the features of Paget's disease of bone were reviewed. The clinical phenomena were compared with those in another group of patients, identified by having a normal skull X-ray. Cranial nerve lesions and spinal syndromes could be attributed to the Paget's disease in many cases but the evidence did not suggest that other clinical phenomena such as dementia and epilepsy were related to the bony changes. Amongst cranial nerve lesions deafness is certainly attributable to Paget's disease, hemifacial spasm when combined wit

The value of the estimation of liver and bone isoenzyme of alkaline phosphatase in a series of 500 admissions to a geriatric unit is described. A raised total alkaline phosphatase was found in 40 patients and this was due to raised levels of the bone isoenzyme in about two-thirds of these. Its value in diagnosis of treatable bone disease is emphasized.

Juvenile Paget's disease (JPD) is a rare, autosomal recessive osteopathy. Although it has phenotypic overlap with Paget's disease of bone (PDB), it is probably a distinct entity. Because of its rarity, optimal disease management has not yet been established by randomized controlled trials. However, clinical, biochemical, and radiographic improvement has been reported after treatment with antiresorptive agents, including calcitonin and bisphosphonates (BPs). Compared with other BPs, zoledronic acid (ZOL) has a higher affinity to bone mineral and is a stronger inhibitor of the enzyme farnesyl py

Hyperuricemia and gout can be seen in patients with Paget's disease of bone (PD). This may be secondary to increased nucleic acid turnover in accelerated bone remodeling. It is unclear whether all PD patients should be evaluated for the presence of PD and gout in the same patient. In this report, I present a gout patient who later diagnosed as having PD and discuss the relationship between gout and PD.

In South America the incidence of Paget's disease of bone (PDB) is low and more than half of the cases published in the last 30 years come from Brazil and Argentina. The aim of this study was to describe the clinical and epidemiological characteristics of PDB in two institutions in Pernambuco. PDB patients in Recife, Brazil were studied retrospectively. A total of 108 cases were diagnosed from 1984 to 2005. Over 90% of the patients were of European descent. The average age was 66.2 years and 49.1% were male. The polyostotic form was the more common. Pain and bone deformity were significantly m
