Group B streptococcal vertebral osteomyelitis is rare in adults. Osteomyelitis due to this organism is in general related to contiguous infections, recent surgery, or peripheral vascular disease. All reported cases of group B streptococcal vertebral osteomyelitis, however, have had no association with these predisposing factors and have usually been presumed to be of hematogenous origin, though bacteremia has never been demonstrated. Here we describe a 45-year-old intravenous drug abuser who had vertebral osteomyelitis and bacteremia. We conclude that the vertebral osteomyelitis in this patien…

The authors present two cases of resultant hallux hammer toe secondary to the definitive treatment of hallux sesamoidal osteomyelitis. Pseudomonas osteomyelitis developed in both cases following puncture wounds to the first metatarsophalangeal joint complex. The authors also review the literature on pseudomonas osteomyelitis secondary to puncture wounds and the development of hallux hammer toe after removal of the involved sesamoid bones.

Experimental studies of osteomyelitis have been hampered by methodologic problems. For inducing experimental anaerobic osteomyelitis, the medullary cavity of the proximal tibial metaphysis of five New Zealand white rabbits was excavated and filled with a polyvinyl alcohol sponge. On the right side, 1 ml of a suspension containing Bacteroides fragilis (10(7) colony-forming units/ml) was injected, while on the left side, Ringer's solution was used as a control substance. Within five weeks, all animals developed clinical signs of osteomyelitis. The findings were verified by roentgenograms and bon…

Recently, pyogenic vertebral osteomyelitis has been documented following transcolonic passage of bullets lodging in or near the vertebral column. It is becoming more recognized that foreign body missiles are not as sterile as was once thought--especially after passage through the gastrointestinal tract--and that this condition contributes to the incidence of infection in such bone injuries. In this article the problem of deciding on and implementing proper initial treatment and management of vertebral injuries caused by foreign body missiles is illustrated and discussed.

Serratia plymuthica was isolated from the sinus tract and from the infected bone in a case of chronic osteomyelitis following an accident of a motorcyclist. This is the second case in which S. plymuthica was a significant pathogen. Previously, a case of S. plymuthica sepsis associated with infection of a central venous catheter was described.

A patient with Salmonella group D (Salmonella enteritidis) acute osteomyelitis of an operated femur is described. The patient failed to respond to chloramphenicol therapy and aztreonam therapy. The local infection that complicated the presence of a foreign body was eradicated only with the removal of the foreign body, an internal fixation plate.

Coagulase-negative staphylococci (CONS) are occasionally cultured from foci of osteomyelitis in otherwise healthy individuals and are usually regarded as contaminants. The present report describes two children with subacute osteomyelitis of the distal tibia in which pure cultures of CONS were obtained from bone. In one patient, the infection was multifocal, and CONS were isolated from two anatomical sites at two different times. In the other patient, the infection was unifocal, and CONS were cultured from two separate specimens obtained from the same site. In both patients, the symptoms progre…

Sixty-eight patients with clinically suspected chronic osteomyelitis were studied with [111In]chloride. Fifty-four images were categorized as true positive; seven were categorized as true negative. There were four false-positive studies, two of which were associated with healing cancellous bone grafts. There were three false-negative studies in patients previously treated with long-term antibiotic therapy. Images in eight noninfected healing fractures 3 to 8 mo old were normal. Three patients with infected total hip prostheses had positive images. Two patients with loose prostheses had negativ…

The CT findings in 20 patients with chronic osteomyelitis, mostly of post-traumatic origin, were compared with conventional radiographs and operative findings. CT diagnosed rather more sequestra than the conventional method. An important advantage of CT is the ability to demonstrate lesions in the medulla and infections in the soft tissues, eg. abscesses. CT should be reserved for cases where conventional methods with tomography leave unsolved problems, and in the present of fistulae.

Based on nine patients 4 to 26 years of age, we describe the clinical characteristics of chronic recurrent multifocal osteomyelitis. This disorder was characterized by an insidious onset of fever, local swelling and pain in affected bones, and radiologic findings suggesting osteomyelitis. The lesions were mainly localized to the clavicles and the metaphyses of tubular bones. The clinical course was characterized by intermittent periods of exacerbation and improvement over several years. Six of the patients had recurrent pustulosis palmaris and plantaris, which closely parallelled the exacerbat…

50 patients suffering from chronic posttraumatic osteomyelitis were examined. In 66% an open fracture, caused either by traffic or industrial accidents was the primary reason for hospitalization. The most frequent pathogenic bacterium was staphylococcus aureus: 80% of the patients suffered from staphylococcal infections, mostly, however, in mixed culture with pseudomonas aeruginosa or clebsiella. Whereas the peripheral leukocyte count and the serum electrophoresis showed abnormal values only in a small percentage of the patients, the erythrocyte sedimentation rate proved to be a reliable index…

Proximal humerus osteomyelitis after injection of influenza vaccination is a rare but potentially debilitating complication. An otherwise healthy 55-year-old woman presented with acute osteomyelitis after influenza vaccination. This case report describes successful treatment with intravenous antibiotics and subsequent shoulder arthroscopic d?bridement, manipulation under anesthesia, and capsular release for secondary adhesive capsulitis. Consideration of proximal humerus osteomyelitis as a potential risk after flu vaccination is an important tool for orthopaedic specialists and primary care pr…
