Carotid body arterial chemoreceptors are essential for a normal hypoxic ventilatory response (HVR) and ventilatory acclimatization to hypoxia (VAH). However, recent results show that O(2)-sensing in the brain is involved in these responses also. O(2)-sensing in the rostral ventrolateral medulla, the posterior hypothalamus, the pre-Bötzinger complex and the nucleus tractus solitarius contribute to the acute HVR. At sea level oxygen is toxic to man when breathed for more than twenty-four hours at a percentage greater than about forty percent. Pulmonary pathology is the first manifestation in subjects with previously normal lungs. In patients with pre-existing lung disease the results are often additive. There is, however, great variation in response from subject to subject and between patients. Queen's Belfast and Harvard University Medical School have been the sites of seminal investigations. There has been considerable controversy surrounding the optimal inspired oxygen concentration for resuscitation of term and preterm infants. We have developed a rat pup model to quantify both physiologic and biochemical parameters associated with normoxic vs. hyperoxic resuscitation. We have confirmed existing human data that hyperoxic resuscitation of rat pups is associated with a significant delay in onset of spontaneous respiratory efforts. Conventional treatment of respiratory failure involves positive pressure ventilation with high concentrations of inspired oxygen. Pulse oximeters are now commonplace in modern medical practice, but we still need to be aware of their limitations. We present here a case of a 62-yr-old gentleman who underwent general anaesthesia for a recurrent parietal meningioma. He had received multiple general anaesthetics in the past. Persistent low pulse oximetry readings in the perioperative period, without any suggestion of respiratory compromise, led us to investigate him further and to discover a new, but benign, haemoglobinopathy.

This article reviews the applications of hyperbaric oxygen (HBO) as an adjunctive treatment of certain infectious processes. Infections for which HBO has been studied and is recommended by the Undersea and Hyperbaric Medicine Society include necrotizing fasciitis, gas gangrene, chronic refractory osteomyelitis (including malignant otitis externa), mucormycosis, intracranial abscesses, and diabetic foot ulcers that have concomitant infections. Life Cycle Assessment (LCA) was used to compare energy consumption and pollutant emissions of two oxygen biofuels, ethanol and methyl ester, which were mixed with gasoline and diesel oil at levels of 10% and 30% of the biofuel. As is well known, the origins and development of hyperbaric medicine are closely tied to the history of diving medicine. Our HBO2 studies stemming from diving medicine date back to 1972. We concentrated our early basic research on dysbaric osteonecrosis. There are now good indications that HBO2 is helpful in a variety of orthopedic conditions. However, hyperbaric medicine in orthopedics is still relatively new and some aspects of it remain controversial.
