Hemopytsis is commonly encountered in the daily practice of the pulmonary physician. Younger patients with normal chest x-rays frequently have acute or chronic bronchial disease accounting for their complaint. Occasionally parasitic disease is described as an unusual cause for a patient presenting with hemoptysis. Although pulmonary paragonimiasis is unusual in this country, because of the rapid growth in travel as well as immigration, physicians will need to be aware of this disorder.

A variety of pulmonary diseases have diffuse cystic abnormalities within the lungs, including emphysema, cystic bronchiectasis, desquamative interstitial pneumonia, lymphocytic interstitial pneumonia, usual interstitial pneumonia, lymphangioleiomyomatosis, Langerhans histiocytosis, cystic metastasis, Wegener granulomatosis, Pneumocystis carinii pneumonia, pulmonary paragonimiasis, and septic pulmonary emboli. CT can easily detect cystic abnormalities as common features in many diseases. However, some characteristic pattern of them can facilitate specific diagnoses.

Status of metacercarial infections of Paragonimus westermani was observed in freshwater crabs, which were purchased at 3 markets in its peak season of 1990. All of 85 crabs were Eriocheir japonicus. No other species of Eriocheir were found. When crushed muscle and viscera was examined individually, the infection rate was 11.8%; and mean number of metacercariae was 2.1 per infected crab. Unless adequately cooked, freshwater crabs are still potential sources of human paragonimiasis.

A sandwich-ELISA was developed using IgG from rabbits infected with Schistosoma japonicum as capture antibody and a monoclonal antibody as detecting reagent for circulating Sj 31/32 in patient sera. The assay detected circulating antigen in 36 of 53 sera from Chinese schistosomiasis japonica patients, but not in sera from healthy persons or patients with paragonimiasis, trichinosis or cysticercosis. This is the first demonstration that Sj 31/32 proteins may be used for circulating antigen-based serodiagnosis of schistosomiasis.

OBJECTIVE: To detect circulating antigen in the patients with schistosomiasis.
METHODS: Polyclonal antibody(PcAb)-monoclonal antibody(McAb) sandwich ELISA.
RESULTS: Sera from 150 patients with schistosomiasis were detected, the average sensitivity was 84.7%(72.0%-91.0%). No false positive reaction was detected in 40 normal controls and none but 2 patients(2/10) with paragonimiasis showed cross reactions among 74 cases with other parasitic infections.
CONCLUSION: PcAb-McAb sandwich ELISA is a relatively ideal method for circulatling antigen detection.

Three species of the families Viviparidae and Pleuroceridae, the first intermediate host of paragonimiasis, metagonimiasis and echinostomiasis were studied cytologically. The observed diploid chromosome number was as follows: Semisulcospira libertina 36, S. dolichostoma 34, and Viviparus rivularis 64. The mitotic chromosome complement of S. libertina has nine metacentric pairs and nine submetacentric pairs, and S. dolichostoma has three metacentric pairs and 14 submetacentric pairs of chromosomes. Viviparus rivularis showed two metacentric pairs and 30 submetacentric pairs of chromosomes.
