Minimally Invasive Endodontic Treatment through the Labial/Buccal Cervical Approach - Protocol and Case Series.

INTRODUCTION: Traditional access cavity preparation involves removing the roof of the pulp chamber and smoothing the dentin bulges at the root canal orifice, thereby creating straight-line access. However, this may damage more healthy dental tissue and reduce the tooth's fracture resistance. This case series presents a novel minimally invasive endodontic protocol for 1 maxillary canine and 4 mandibular premolars, which required root canal therapy, due to labial/buccal cervical decay that caused pulpitis or periapical periodontitis. METHODS: Our treatment protocol includes the following steps:

Sine Qua Non: Dentigerous Cyst.

Dentigerous cysts, also known as follicular cysts, are among the most common developmental cysts of the gnathic bones. The majority of cases are clinically asymptomatic and discovered incidentally on panographic radiographs during routine dental care. The cyst appears as a radiolucency, classically unilocular, associated with the crown of an unerupted or impacted tooth. Usually diagnosed in the 2nd-3rd decade, third molars of the mandible are the most commonly affected teeth. Histologically, dentigerous cysts demonstrate a fibrous or fibromyxoid connective tissue wall lined by squamous epithel

Distinctive microbiota distribution from healthy oral to post-treatment apical periodontitis.

Post-treatment apical periodontitis (PoAP) occurs when root canal treatment has not adequately eliminated bacterial invasion and infection. Yet little is known about the bacterial composition and changes related to the etiology and pathogenesis of PoAP. In this study, clinical samples classified as root apex (HARD) and periapical granulation tissues (SOFT) were separately collected from 10 patients with PoAP. The microbiota of each sample was characterized by 16S rRNA gene sequencing, and the obtained dataset was coanalyzed with 20 NCBI sequence read archive (SRA) datasets of healthy oral (HO)

Regenerative endodontic treatment of an immature tooth with a necrotic pulp and apical periodontitis using platelet-rich plasma (PRP) and mineral trioxide aggregate (MTA): a case report.

AIM: To report the successful clinical and radiographic outcome of a regenerative endodontic treatment. SUMMARY: A 16-year-old male patient presented with a discoloured, maxillary left lateral incisor with a necrotic pulp. Radiographic examination revealed an incompletely developed root with an open apex. Under local anaesthesia and rubber dam isolation, an access cavity was prepared and the necrotic pulpal remnants were removed. The canal was disinfected without mechanical instrumentation with 5.25% NaOCl solution and dried with sterile paper points. A triple antibiotic (metronidazole, cipro

Stabilizing bony plate in guided endodontic surgery using an innovative approach.

Endodontic microsurgery in tandem with advanced radiographic techniques has led to the emergence of guided surgeries. Preservation of the cortical bone to enhance the healing and stabilization of tissues surrounding the tooth of concern can now be facilitated by bone cement used in the field of orthopedics. This case report details a guided endodontic surgery technique in 17 years old where a traumatic infliction leads to a phoenix abscess. The technique elaborated emphasizes on the three-dimensional printing of a surgical template with the help of cone-beam computed tomography, followed using

Radicular Cyst Associated with Type 3B Dens Invaginatus in a Permanent Maxillary Lateral Incisor.

Dens invaginatus (DI) is a developmental abnormality of the tooth resulting in the invagination of the enamel organ into the dental papilla before the calcification of dental tissues. DI is one of the common dental deformities, but its association with the radicular cyst (RC) is still very uncommon. Oehler's Type 3B DI has a deep invagination that extends apically communicating with the apical area. This allows the entry of irritants predisposing for dental caries, periapical lesions, and pulp pathology if there is a communication with pulp. The sequelae of undiagnosed and untreated coronal in

Very Large Inflammatory Odontogenic Cyst with Origin on a Single Long Time Traumatized Lower Incisor.

One of the consequences of traumatic injuries is the chance of aseptic pulp necrosis to occur which in time may became infected and give origin to periapical pathosis. Although the apical granulomas and cysts are a common condition, there appearance as an extremely large radiolucent image is a rare finding. Differential diagnosis with other radiographic-like pathologies, such as keratocystic odontogenic tumour or unicystic ameloblastoma, is mandatory. The purpose of this paper is to report a very large radicular cyst caused by a single mandibular incisor traumatized long back, in a 60-year-old

Localized vertical maxillary ridge preservation using bone cores and a rotated palatal flap.

The aim of vertical augmentation of the alveolar ridge is to restore resorbed alveolar ridges. This technique is critical to the placement of dental implants in a favorable position and the enhancement of restoration esthetics. The present report describes a technique for surgical preservation of the anterior maxillary process using maxillary bone from the surgical site and raising a soft tissue rotated palatal flap. Maxillary lateral incisor extraction and periapical surgery of the central incisors were first carried out. Two bone cores were harvested from the neighboring buccal vestibular re

A review of the wide applications of microfluidic devices in peri-implantitis, periodontitis, and endodontics.

Periodontitis (PD) is a chronic inflammatory disease driven by a dysbiotic oral biofilm, leading to the progressive destruction of the tooth-supporting tissues. Peri-implantitis (PI) involves a similar plaque-induced inflammatory process that results in the loss of bone around dental implants. Endodontic diseases are primarily infections of the dental pulp and periapical tissues. The effective management of these conditions depends on accurate diagnosis and the removal of microbial biofilms from root canals, periodontal pockets, and implant surfaces. However, the development of advanced regene…

Molecular docking and MD simulations predicted quercetin as a potent human interleukin-1 beta (hIL1β) inhibitor for improved endodontic disease management.

IL1β, a pro-inflammatory cytokine, is a key mediator in the inflammatory processes linked to endodontic disorders. Studies have shown that IL1β production is elevated in symptomatic periapical lesions, highlighting its involvement in inflammation and lesion development. Elevated levels of IL1β correlate with larger lesion sizes and increased inflammation in periapical tissues. Given its role in inflammation, IL1β represents a potential therapeutic target for endodontic diseases, including the use of IL1β inhibitors. The present study used molecular docking and MD simulations to identify small …
