Reduced inorganic phosphorus in the natural environment: significance, speciation and determination.

It is commonly assumed that phosphorus occurs almost exclusively in the environment as fully oxidized phosphate (primarily H(2)PO(4)(-) and HPO(4)(2-), where the oxidation state of phosphorus is +V). Recent developments in the field of microbiology and research on the origin of life have suggested a possibly significant role for reduced, inorganic forms of phosphorus in bacterial metabolism and as evolutionary precursors of biological phosphate compounds. Reduced inorganic forms of phosphorus include phosphorus acid (H(3)PO(3), P(+III)), hypophosphorus acid (H(3)PO(2), P(+I)) and various forms…

Renal handling of calcium and phosphorus in experimental renal hyperparathyroidism in dogs.

Twenty-four hour urinary excretion, fractional excretion and the filtered load of calcium and phosphorus were monitored as hyperparathyroidism evolved in a model of progressive canine renal failure. Thirteen beagles of both sexes aged four and a half months were used. Nine of them were subjected to a renal damaging schedule (neomycine, 60 mg/kg/48 h, IM, 32 weeks) in order to induce chronic renal failure leading to secondary hyperparathyroidism (2HPT group). The remaining four were kept as the control group. The experiment was conducted over 32 weeks. Blood and 24 h urine were collected every …

[Importance of the "adequate blood phosphorus" concept as a risk factor for hyperphosphatemia].

Hyperphosphatemia is an important risk factor of secondary hyperparathyroidism and extraosseous calcifications in chronic renal failure patients. In this study our hypothesis is that physicians misconception of adequate phosphatemia is a risk factor for hyperphosphatemia. In 1999 GEMOR sent a renal osteodystrophy inquiry to different hemodialysis centers in Argentina. It included 80 dialysis centers in 17 Argentinian provinces. The enquire had 33 questions about renal osteodystrophy. Here we report the section related to phosphorous metabolism. We obtained responses from 80 dialysis centers (4

The institute of medicine's "dietary reference intake" for phosphorus: a critical perspective.

Two questions relevant to the safety of phosphate additives have been raised in the literature: 1) Is the dietary ratio of calcium to phosphorus clinically significant in humans? Specifically, can a high-phosphorus, low-calcium diet cause hypocalcemia and/or secondary hyperparathyroidism in humans? 2) Does the growing use of phosphate-containing food additives increase the risk of low bone density and/or fracture in humans? In 1999, the Institute of Medicine published a monograph which addressed these two questions. This article critically reviews the findings and conclusions of the Institute …

[Influencing factors in the control of phosphorus in peritoneal dialysis. Therapeutic options].

UNLABELLED: Impaired phosphate excretion resulting in hyperphosphatemia is one of the earliest consequences of chronic renal failure. To control serum phosphate levels, we can use the following therapies: 1) Restriction of dietary phosphate (but on CAPD, obligatory protein losses via peritoneal fluid makes impractical any reduction of phosphate diet. 2) Reduction of phosphate absorption, using phosphate binders. 3) Peritoneal phosphate removal. OBJECTIVE: 1) To evaluate the factors affecting peritoneal phosphate removal such as plasma phosphate, peritoneal membrane transport type, peritoneal

[Radioactive phosphorus (32P); an old but not bad treatment for polycythemia vera].

OBJECTIVE: To determine if there is a place for radiophosphorus (32P) in the treatment of polycythaemia vera (PV) and essential thrombocytosis (ET). DESIGN: Retrospective. SETTING: Leyenburg Hospital, The Hague, the Netherlands. METHOD: Data on 144 patients with the diagnoses 'PV' or 'ET' from 1965 to 1994 were collected. Available data were insufficient in 19 of these. Regarding 125 patients, 80 with PV and 45 with ET, the survival and the frequency of acute leukaemia with various forms of treatment (32P, busulfan or combination of several treatment modalities) were studied. Moreover, in t…

Phosphorus and protein restriction and parathyroid function in chronic renal failure.

Phosphorus retention as a result of chronic renal failure (CRF) induces secondary hyperparathyroidism (HPT II) while supplemented low-phosphorus low-protein diets (LPD) prevent it. The aim of this study was to assess in seven patients with advanced CRF and biological HPT II the effects of a LPD providing daily 5 to 7 mg/kg phosphorus, 0.4 g/kg protein, 300 mg calcium (Ca) and supplemented with amino acids, ketoacids, CaCO3 and vitamin D2, on the relationship between ionized Ca (iCa) and PTH concentrations. Hyper- and hypocalcemia were induced by CaCl2 and Na2-EDTA infusion. After three months …

The effect of experimental cryptorchidism on the phosphorus NMR spectrum of the rat testis.

Magnetic resonance (MR) spectroscopy of the cryptorchid rat testis was used to test whether changes in the MR spectra of the rat testis might be a more sensitive indicator of changes in the metabolic status of germ cells in the testis rather than simply the cell types present. Testes of adult Wistar rats before and during 42 days of experimental cryptorchidism were investigated by in-vivo 31P MR spectroscopy. Results were compared to MR studies of the synchronized developing testis. The testicular phosphomonoester/ATP (PM/ATP) ratio was dependent only on the cell types present, and showed the

Technical aspects of phosphorus reduction and mineral adaptation in infant formulae.

Adapted milk-based formulae are complex products which simulate the protein and mineral composition of breast milk. The desired casein-to-whey protein ratio (40:60) in these formulae could be obtained by mixing cow's milk and whey. Unfortunately, this would yield formulae with too high a mineral content and a very unfavourable Ca:P ratio. The adjustment of the mineral content and the Ca:P ratio is the most sophisticated step in manufacture of such formulae. For this purpose, whey is demineralized with ion-exchange resins, electrodialysis or a combination of both processes. The demineralized wh

Effects of dietary aluminum and phosphorus on magnesium metabolism in dairy calves.

The metabolism of Mg was studied in young dairy calves fed two levels of added Al (0 and .20% Al) and two levels of added P (0 and .22% P) for 7 wk. The four treatments were 1) normal P-low Al, 2) low P-low Al, 3) normal P-high Al and 4) low P-high Al. The basal diet (low P-low Al) contained, by analysis, .132% P, .021% Al and .17% Mg. Added Al did not affect (P greater than .10) serum Mg. An Al x P interaction on bone Mg was detected (P less than .01). Magnesium was reduced in tibia shaft (.34 vs .44%) and in tibia joint (.43 vs .53%) in calves fed high Al in the presence of normal dietary P,

Calcium acetate, an effective phosphorus binder in patients with renal failure.

Calcium salts are increasingly used as phosphorus binders in patients with chronic renal failure. Calcium carbonate is the principal salt presently utilized, however, other calcium salts may be more effective and safer phosphorus binders. Theoretical calculations, in vitro experiments, and in vivo studies in normal subjects have shown calcium acetate to be a more effective phosphorus binder than other calcium salts. This salt has not previously been studied in patients with chronic renal failure. We used a one-meal gastrointestinal balance technique to measure phosphorus absorption, calcium ab…

Dietary calcium and phosphorus and seizure susceptibility of magnesium deficient rats.

Convulsions are characteristic of magnesium deficiency and hypocalcemia. In this study, weanling rats were fed magnesium deficient diets with varying concentrations of calcium and phosphorus. Diets were either normal (Mg =) or low (Mg-) in magnesium and were either low (Ca- or P-), normal (Ca = or P =) or high (Ca+ or P+) in calcium or phosphorus. After consuming the diets for 17 days, the rats were tested for audiogenic seizures and blood was then drawn for serum mineral analyses. Rats fed Mg-Ca = P =, Mg-Ca = P-, Mg-Ca+P = or Mg-Ca+P+ diets had high incidences of seizures. Those fed Mg-Ca-P …

The influence of dietary calcium and phosphorus on tibial dyschondroplasia in broiler chickens.

The incidence and severity of tibial dyschondroplasia in broiler chickens were increased by feeding rations containing high levels of phosphorus relative to the level of calcium. Tibial dyschondroplasia was not eliminated by feeding a ration containing 1.5% calcium and 0.5% available phosphorus. Before typical dyschondroplasia developed, chickens fed a high level of phosphorus had uniformly thickened growth plates at 2 weeks of age. The majority of this thickening was due to an increase in the zone of proliferation, identical to that which occurs in calcium-deficiency rickets.

Effects of induced total-body hyperthermia on phosphorus metabolism in humans.

The effects of total-body hyperthermia on phosphorus homeostasis are controversial. To evaluate the problem, 10 clearance studies were performed in seven patients undergoing total-body hyperthermia as an adjunct to the treatment of solid malignant tumors. Total-body hyperthermia was associated with significant reduction in plasma phosphorus concentration from a baseline value of 3.51 +/- 0.18 to 0.6 +/- 0.1 mg/dl (p less than 0.001), returning to baseline following cessation of total-body hyperthermia. The clearance of phosphorus increased from 15.2 +/- 2.5 to 26.1 +/- 3.1 ml per minute (p les…

Calcium and phosphorus studies: interpretation of results and strategies for further testing.

Persistent hypercalcemia is most often due to tumor. Other causes are best eliminated by clinical methods combined with simple tests. Hypocalcemia is most often a result of hypoalbuminemia. Hyperphosphatemia induced by phosphate infusion or enema may cause profound hypocalcemia. Biochemical profiling may uncover severe hypophosphatemia necessitating phosphate administration. Markedly elevated serum alkaline phosphatase in the absence of hepatic disease is most likely to reflect either Paget's disease of bone or metastatic prostate cancer.

Systemic mastocytosis in a patient with polycythemia vera treated with radioactive phosphorus.

Systemic mastocytosis occurred as a fatal event in a patient with long-standing polycythemia vera. The patient had been treated over the course of 21 yr with radioactive phosphorus. Possible relationships between mastocytosis and polycythemia vera, and also between mastocytosis and treatment with ionizing radiation, are discussed. Histopathologic and electron microscopic findings are illustrated. Difficulties in establishing the diagnosis of mast cell disease in this setting are also described.

[Metabolism and effects of phosphorus in eutrophic infants dehydrated from diarrhea].

To verify whether there is a state of phosphate depletion in dehydrated infants with diarrhea and whether phosphate administration affects recovery from metabolic acidosis, two groups of infants were studied. All were males, from 1 to 7 months old, and in good nutritional state. The control group was treated with a standard regimen of I.V. fluids initially, followed by feedings of a diluted milk formula starting on the 2nd day of treatment. The study group, in addition to this standard regimen, received sodium phosphate (Na2HPO4/Na2HPO4, 4:1, pH 7.4), initially by the I.V. route and thereafter

[Isoprenaline, propranolol and in vivo phosphorus metabolism of the rat salivary gland].

Chronic treatment with isoprenaline causes hypersecretion and a marked enlargement of submaxillary glands. Isoprenaline, given to normal rats or treated animals with hypertrophied salivary glands, increases the incorporation of 32P in tissue inorganic and organic phosphates and phospholipids. Propranolol given alone is inactive on phosphorus metabolism, but inhibits the effect of isoprenaline. The phospholipid effect is probably related to the increase of membranes in hypertrophied acinar cells; a correlation with the hypersecretion induced by isoprenaline needs further study. The results show…
