Patient Outcomes in Mandibular Distraction Based on Timing of Feeding Initiation.

OBJECTIVE: The aim of this study is to compare the feeding outcomes between patients who underwent mandibular distraction osteogenesis (MDO) surgery; one subset received feeding throughout the distraction period, whereas the other subset was not fed until after distraction completion. The objective is to assess whether initiating feeding during the distraction phase can lead to enhanced postoperative care for patients with airway obstruction secondary to micrognathia or Pierre Robin sequence (PRS). STUDY DESIGN: Retrospective chart review (2000-2024). SETTING: Single academic institution. M

Long-term survival in TARP syndrome and confirmation of RBM10 as the disease-causing gene.

TARP syndrome, comprising Talipes equinovarus, atrial septal defect (ASD), Robin sequence (micrognathia, glossoptosis, and cleft palate), and persistence of the left superior vena cava, is an X-linked condition with pre- or postnatal lethality in affected males. Based on linkage studies and massively parallel sequencing of X-chromosome exons in two families, the disease-causing gene was identified as RBM10. We identified a maternally inherited frameshift mutation in an unrelated patient, confirming RBM10 as the disease gene. This is the first reported individual with TARP syndrome who survived

Bilateral complex odontomas: a rare complication of external mandibular distraction in the neonate.

BACKGROUND: The authors present an unusual complication of external mandibular distraction in a child with bilateral complex odontomas in the posterior mandible. CLINICAL REPORT: The patient is a 4-year-old girl with a history of micrognathia, glossoptosis, and respiratory distress (Pierre-Robin sequence) treated with neonatal external mandibular distraction. She was referred to our service with bilateral amorphic radio-opaque masses in the body of the mandible. CONCLUSIONS: The etiology of odontomas is uncertain, but different theories exist, such as local trauma, infection, family history,

Airway management in patients with isolated Pierre Robin sequence during the first year of life.

Managing the airway of patients with Pierre Robin sequence is diagnostically and therapeutically challenging. Like many other pathologies, Pierre Robin sequence is best managed with a multidisciplinary team. Providing a comprehensive evaluation is important to detect patients who may have silent events during activities of early life. Children with airway obstruction should have a complete assessment to anatomically define the site of airway obstruction. Therefore, management options can be targeted to the specific deficiency. The present article outlines a treatment protocol for patients who

De novo interstitial direct duplication 1(q23.1q31.1) in a fetus with Pierre Robin sequence and camptodactyly.

Interstitial duplications of chromosomal region 1q are rarely seen. We report the first prenatal diagnosis of pure partial trisomy 1q. The fetus was karyotyped for polyhydramnios, micrognathia, and flexion of fingers of both hands. Conventional and molecular cytogenetics showed a de novo direct duplication of the chromosomal region 1q23.1q31.1 leading to a partial trisomy 1q. At autopsy, the fetus showed Pierre Robin sequence (PRS) and camptodactyly. The main histological finding was a decreased number of motoneurons with apoptotic features in the anterior horn of the spinal cord. A literature

c. 595-596 insC of FOXC2 underlies lymphedema, distichiasis, ptosis, ankyloglossia, and Robin sequence in a Thai patient.

Lymphedema-distichiasis syndrome is a rare primary lymphedema inherited as an autosomal dominant disorder. The characteristic features consist of late onset-lymphedema and distichiasis together with other occasionally seen features including varicose vein, cleft palate, ptosis, and congenital heart diseases. FOXC2 is the gene found to be associated with this syndrome. We report here the first Thai patient who has characteristic features of this syndrome and the infrequently described features including ankyloglossia, and Robin sequence which consists of glossoptosis, cleft palate, and microgna

Management of the difficult pediatric airway with Shikani Optical Stylet.

The airways of children with many craniofacial and mandibulofacial malformations often present unique challenges to the anesthesiologists. We report the application of Shikani Optical Stylet (SOS) in facilitating the tracheal intubation of four children with history of difficult airway management. The SOS combines the benefits of the lightwand and fiberoptic bronchoscope.

Epidemiology of cleft lip and palate in Singapore--a 10-year hospital-based study.

During the study period from January 1985 to December 1994, there were 1105 new cleft cases seen in the Department of Plastic Surgery, Singapore General Hospital. These included newborn as well as unoperated children and adult cases. During the same period, the total number of recorded births in Singapore was 474,542. Out of the 1105 new cleft cases seen, 984 were Singaporeans. The incidence of this hospital-based study of cleft population in Singapore was 2.07 per 1000 livebirths. Chinese had the highest incidence of 1.64 per 1000 as compared to Malay, Indian and other races. The most common

Dislocation of the hip in birth defects.

Several heritable disorders--although rare--are reviewed that are directly or indirectly related to dislocation of the hip. With the ever increasing list of clinical syndromes and our growing understanding of genetics, the physician who first sees these patients should broaden his views to identify these conditions and establish the diagnosis. The effectiveness of treatment often depends upon such knowledge.

Orthodontic Appliance-Related Mucosal Ulcerations in Newborns and Infants With Craniofacial Disorders.

OBJECTIVES: In craniofacial disorders (CD) like cleft lip and/or palate (CL/P), Robin sequence (RS) or Down syndrome (DS), an early orthodontic intervention with different palatal plate devices is often applied. However, there are no data on complications such as mucosal ulcerations (MU).To determine the frequency and location of MU and evaluate associations with potential risk factors such as CD type, sex, appliance type, cleft location, underlying syndrome, and anatomical cleft morphology. MATERIALS AND METHODS: In a retrospective analysis, we searched our electronic patient records of newb

Cleft palate morphology, genetic etiology, and risk of mortality in infants with Robin sequence.

Robin sequence (RS) has many genetic and nongenetic causes, including isolated Robin sequence (iRS), Stickler syndrome (SS), and other syndromes (SyndRS). The purpose of this study was to determine if the presence and type of cleft palate varies between etiologic groups. A secondary endpoint was to determine the relationship of etiologic group, cleft type, and mortality. Retrospective chart review of patients with RS at two high-volume craniofacial centers. 295 patients with RS identified. CP was identified in 97% with iRS, 95% with SS, and 70% of those with SyndRS (p < .0001). U-shaped CP was
