Addressing gender-based violence in disaster contexts: institutional, professional, and social responses to the 2021 La Palma (Spain) volcanic eruption.

This study explores the institutional, professional, and social responses to the gender-based violence that occurred in the wake of the 2021 volcanic eruption in La Palma, Spain. Using a qualitative methodology, it analyses the perspectives of 46 stakeholders-political decision-makers, technical staff, and social activists-acquired through in-depth interviews. The findings reveal that gender-based violence was initially overlooked during the crisis as emergency actions prioritised general safety and housing. The absence of gender-sensitive planning increased the vulnerability of women, particu

A Conceptual Framework for Evaluating National Organ Donation and Transplantation Programs.

Conceptual frameworks are valuable resources that can be used to guide the planning, evaluation, and development of healthcare services. However, there are currently no comprehensive frameworks focused on organ donation and transplantation that identify the critical factors underlying a successful national program. To address this knowledge gap, we developed a conceptual framework that takes into account all major domains of influence, including political and societal aspects as well as clinical implementation. The framework was initially constructed based on a targeted review of the relevant

Six Questions for Well-Child Care Redesign.

In the United States, well-child care has the goal of providing comprehensive care to children by addressing developmental, behavioral, psychosocial, and health issues through visits at recommended intervals. The preventive care needs of families can outpace the capacity of clinics and practices to provide it, necessitating a redesign of our well-child care system that aligns the structure of preventive care delivery with the needs of families. Here we focus on 6 questions (the what, when, who, why, how, and where) for well-child care redesign for infants and young children. By addressing thes

Team-based Care for Cardiac Rehabilitation and Exercise Training in Heart Failure.

The estimated cost of treating patients with HF in the United States is expected to more than double by 2030.65 This forecast of the impact of HF in the United States should serve as a call to action. Despite well-documented benefits, participation in exercise training and CR programs by patients with HF remains low. In this article, standards and guidelines for exercise and CR in HF were reviewed. Although traditional CR had core components, it lacked care management specific for HF. Chronic stable HF patients can safely exercise; however, there are many unique needs that are not currently ad

Comment for conflict and health don't let sexual and reproductive health in humanitarian and fragile settings become a casualty of aid cuts and crisis fatigue.

In 2025, 300 million people require humanitarian assistance, with sexual and reproductive health (SRH) crises driving 60% of preventable maternal deaths in conflict-affected countries. Despite the proven life-saving impact of the Minimum Initial Service Package (MISP) for SRH in Crisis Situations, official development assistance for SRH declined by 27% between 2022 and 2023, threatening implementation in fragile settings. Analysis of MISP Readiness Assessments across 64 countries (2021-2024) reveals critical gaps: while 95% have national emergency policies, only 40% of these integrate essentia

Czechoslovakia's changing health care system.

Before World War II, Czechoslovakia was among the most developed European countries with an excellent health care system. After the Communist coup d'etat in 1948, the country was forced to adapt its existing health care system to the Soviet model. It was planned and managed by the government, financed by general tax money, operated in a highly centralized, bureaucratic fashion, and provided service at no direct charge at the time of service. In recent years, the health care system had been deteriorating as the health of the people had also been declining. Life expectancy, infant mortality rate

Child sexual abuse in the Roman Catholic Church in Germany: comparison of victim-impact data collected through church-sponsored and government-sponsored programs.

At around the same time in 2010, the German government and the Roman Catholic Church in Germany each set up a critical incident reporting system (CIRS) to gather information on child sexual abuse that had taken place within the Church. The objective in both cases was to solicit stories from victims and former victims and allow them to have input into a review process. The existence of these two similar systems, one initiated by the institution involved in the abuse and the other by an independent entity, provided an opportunity to compare the background and motivation of victims who chose to c

Context matters: using an Evidence to Decision (EtD) framework to develop and encourage uptake of opioid deprescribing guideline recommendations at the point-of-care.

OBJECTIVES: To describe the development and use of an Evidence to Decision (EtD) framework when formulating recommendations for the Evidence-Based Clinical Practice Guideline for Deprescribing Opioid Analgesics. STUDY DESIGN AND SETTING: Evidence was derived from an overview of systematic reviews and qualitative studies conducted with healthcare professionals and people who take opioids for pain. A multidisciplinary guideline development group conducted extensive EtD framework review and iterative refinement to ensure that guideline recommendations captured contextual factors relevant to the

Mass Incarceration and Racial Inequality.

Despite two decades of declining crime rates, the United States continues to incarcerate a historically and comparatively large segment of the population. Moreover, incarceration and other forms of criminal justice contact ranging from police stops to community supervision are disproportionately concentrated among African American and Latino men. Mass incarceration, and other ways in which the criminal justice system infiltrates the lives of families, has critical implications for inequality. Differential rates of incarceration damage the social and emotional development of children whose pare

Health policy and systems research and analysis in Nigeria: examining health policymakers' and researchers' capacity assets, needs and perspectives in south-east Nigeria.

BACKGROUND: Health policy and systems research and analysis (HPSR+A) has been noted as central to health systems strengthening, yet the capacity for HPSR+A is limited in low- and middle-income countries. Building the capacity of African institutions, rather than relying on training provided in northern countries, is a more sustainable way of building the field in the continent. Recognising that there is insufficient information on African capacity to produce and use HPSR+A to inform interventions in capacity development, the Consortium for Health Policy and Systems Analysis in Africa (2011-201

"Poppies are democracy!" A critical geopolitics of opium eradication and reintroduction in Turkey.

Historical scholarship in traditional geopolitics often relied on documents authored by states and by other influential actors. Although much work in the subfield of critical geopolitics thus far has addressed imbalances constructed in official, academic, and popular media due to a privileging of such narratives, priority might also be given to unearthing and bringing to light alternative geopolitical perspectives from otherwise marginalized populations. Utilizing the early-1970s case of the United States' first “war on drugs,” this article examines the geopolitics of opium-poppy eradication a

[Selection and destruction--treatment of "unworthy-to-live" children in the Third Reich and the role of child and adolescent psychiatry].

During the period of National Socialism in Germany, many "asocial", mentally retarded or disabled minors were persecuted. Several measures had been discussed theoretically before, but the National Socialists put the theoretical proposals into practice. As a result children and adolescents were separated, sterilized or killed. In concentration camps so-called "depraved" minors were selected to get special education. The object of this effort was to adapt minors to the ideology of national socialism. After passing the law to sterilize patients with "hereditary diseases" in 1933 about 375.000 peo

Community perceptions and practices on hepatic veno-occlusive disease in Tigray, Ethiopia: An explorative study challenging the attribution to Ageratum conyzoides.

BACKGROUND: Hepatic veno-occlusive disease (HVOD) is a rare but severe condition characterized by the blockage of microscopic veins in the liver due to endothelial damage, leading to sub-endothelial thickening, edema, and fibrosis. Globally the cause of HVOD is primarily associated with pyrrolizidine alkaloid (PA) ingestion, radiation therapy, and post-transplant reactions. Similarly, in the Tigray region of Ethiopia, the disease's outbreak has been linked with contaminated harvests containing PA-producing seed called "Ageratum conyzoides". However, the perception and current experience of com

Analysing the Efficiency of Health Systems: A Systematic Review of the Literature.

BACKGROUND: Efficiency refers the use of resources in ways that optimise desired outcomes. Health system efficiency is a priority concern for policy makers globally as countries aim to achieve universal health coverage, and face the additional challenge of an aging population. Efficiency analysis in the health sector has typically focused on the efficiency of healthcare facilities (hospitals, primary healthcare facilities), with few studies focusing on system level (national or sub-national) efficiency. We carried out a thematic review of literature that assessed the efficiency of health syste

Graduate Medical Education in Lebanon: Challenges, Support, and Adaptation Amid the Compounding Crises.

INTRODUCTION: Graduate Medical Education (GME) systems often face disruptions. In Lebanon, repeated crises over the past several years, including political instability, economic collapse, the COVID-19 pandemic, and military conflict, raise questions about institutional support, clinical residents' preparedness, and the resilience of the GME system. This study aims to examine clinical residents' perceived preparedness and competency at graduation, the challenges they faced during compounding national crises, and how training institutions supported them and adapted GME to maintain training and w

The emergence of urban scaling in Ancient Southwest Asia.

This study investigates the long-term emergence of settlement scaling in Ancient Southwest Asia by analysing house-size data from 1,852 buildings across 80 archaeological sites dating from the Late Epipalaeolithic to the Iron Age (c. 13,100-580 BCE). Using domestic architecture as a proxy for household productivity, we provide the first fully diachronic test of settlement scaling theory in the region where urbanisation first developed. Mixed-effects models reveal that scaling relationships were absent in the Neolithic, ambiguous in the Chalcolithic, and only fully emerged in the Bronze Age, wh

Understanding COVID-19 vaccine hesitancy among Black pregnant individuals: a mixed methods study of social media messenger and content type preferences.

INTRODUCTION: Pregnant individuals are at increased risk of developing severe illness from coronavirus disease 2019 (COVID-19). They also tend to be vaccine-hesitant, which is even more pronounced in certain racial and ethnic minority groups. The study objective was to determine whether social media advertisements promoting COVID-19 vaccination could influence vaccination likelihood among pregnant and recently pregnant participants who self-identified as Black or African American. METHODS: Participants were interviewed individually or in focus groups to explore their attitudes toward vaccinat

The Corporate Political Activity of the infant commercial milk formula industry in Mexico during the COVID-19 pandemic.

BACKGROUND: There is a need to protect, support, and promote breastfeeding for the best interests of children's and mothers' rights. Our study aimed to analyze the Corporate Political Activity (CPA) of the three main Commercial Milk Formula (CMF) industries in Mexico during the COVID-19 pandemic, as such activity may have impacted policymaking to the detriment of breastfeeding. CPA practices may indirectly lead to the early introduction of CMF; these have been a barrier to optimal breastfeeding in Mexico. METHODS: A qualitative study was carried out using multiple data collection techniques,

Building a maternal and child cohort amidst Lebanon's socioeconomic collapse: preliminary results and navigating research challenges.

The impact of conflict and crisis on maternal and child health underscores the need for reliable research in vulnerable populations. Lebanon, amidst ongoing economic collapse, political instability, and healthcare system strain, offers a case study for exploring these impacts, particularly on preterm babies and their development. This study aims to assess the feasibility of establishing a prospective cohort of mothers and their full-term and preterm babies in Lebanon, examining the association between social determinants, preterm birth, and developmental outcomes amidst the nation's multifacet
