Low back pain and sciatica are common causes of medical consultation. The medical history and the physical examination are essential in their management. The primary care physician initiates treatment and relies on his physical examination to identify "red flags" that require further investigations. Regarding sciatica, it is essential to identify 3 clinical entities that require prompt referral to the emergency department for prompt neurosurgical management. These include hyperalgic sciatica, sciatica with motor deficits and the cauda equina syndrome. Sciatica with sensory deficits do not requ

Cauda equina syndrome is an uncommon complication of ankylosing spondylitis (AS) characterized by the slow and insidious development of severe neurologic impairment. Imaging studies usually show a wide lumbar canal with dural ectasia. No medical or surgical treatment has been proven effective. We managed the care of a 66-year-old man who had longstanding AS and clinical features of cauda equina syndrome, including anal incontinence and buttock hypoesthesia. Magnetic resonance imaging demonstrated no cause for these symptoms other than AS. The patient was treated with infliximab, a monoclonal a

A 20-year-old woman with selective cauda equina hypertrophy presented with muscle weakness and severe pain in the lower extremities. Serial immunotherapy was not effective. We performed biopsy of the cauda equina, and laminectomy and duraplasty of the thoracolumbar region. Biopsy revealed marked infiltration of small lymphocytes and foamy macrophages in the endoneurium. Three years after decompression surgery, her symptoms have improved slightly without progression or relapse. This is the first case of selective cauda equina hypertrophy with idiopathic inflammation. We propose that this is a n

BACKGROUND: Symptomatic lumbar spinal stenosis produces gradually progressive back and leg pain with standing and walking, relieved by sitting or lying. One of the uncommon symptoms is involuntary intermittent penile erection due to spinal canal stenosis. This symptom is very rare and often forgotten when history is taken.
METHODS: In this case report, a patient suffering from intermittent priapism due to degenerative spinal canal stenosis and spondylolisthesis is described. On admission his symptoms were intermittent neurogenic claudication and involuntary erection provoked by walking a shor

Unintended incidental durotomy is not a rare complication of lumbar microsurgery and is usually recognized and treated immediately. The reconstruction process can be complicated further by unpredictable factors. To their knowledge, the authors report the first case of a symptomatic pneumorachis associated with the accidental awakening of a patient during reconstruction of an incidental durotomy following lumbar microdiscectomy. Incomplete cauda equina syndrome developed in the patient on awakening from surgery after reconstruction of an unintended incidental dural tear that occurred during lum

The authors report the case of a 40 year old patient with history of cauda equina syndrome and pyramidal signs due to intramedullary rupture of a congenital dermoid cyst of the conus without associated spinal dysraphism. Diagnosis was suggested at magnetic resonance imaging (MRI) and confirmed by histological examination and percutaneous aspiration of the intramedullary cavity.

We have made a study of a case of polyradiculoneuritis occurring in a diabetic. Clinically, symptoms were very similar to those in classical Guillain-Barré syndrome and respiratory complications necessitated a stay in a Respiratory Resuscitation Centre. Recuperation was slow but practically total. Ultrastructural changes noted in the roots and the peripheral nerves of patients suffering from Guillain-Barré syndrome are similar to those occurring in experimental allergic neuropathies. These changes are specific as the myelin takes on a fishnet appearance and, more important, the myelin is attac

Guillain Barré syndrome (GBS) is a post-infectious acute autoimmune polyradiculopathy. Cerebrospinal fluid (CSF) total protein level and plasma neutrophil/lymphocyte ratio (NLR) are related with autoimmune response. We aimed to reach a prognostic indicator for GBS by using electrophysiological findings, protein level of CSF, and plasma NLR based on Medical Research Council (MRC) sum score data. Cases who met diagnostic criteria of GBS and followed at least six months were enrolled in the study. Nerve conduction study (NCS) and lumbar puncture were performed one week after symptom onset. Routin…

Penetrating spinal injuries with foreign bodies are exceedingly rare. To date, pathological problems due to glass fragments in the spinal canal have rarely been reported. In this report, the case presenting with a back laceration, leg pain, and leg weakness was found to have glass frag-ments in the spinal canal at the L2-L3 level by lumbar computed tomography and magnetic resonance imaging. After L2 total laminectomy and retrieval of the glass fragments, the dura was re-paired. The patient was discharged from the hospital after complete neurological recovery. In cases of spinal canal injuries

BACKGROUND: Acute cauda equina syndrome is an uncommon but significant neurologic presentation due to a variety of underlying diseases. Anatomical compression of nerve roots, usually by a lumbar disk hernia is a common cause in the general population, while inflammatory, neoplastic, and ischemic causes have also been recognized. Among human immunodeficiency virus (HIV) infected patients with acquired immunodeficiency syndrome, infectious causes are encountered more frequently, the most prevalent of which are: cytomegalovirus, herpes simplex virus 1/2, varicella zoster virus, and Mycobacterium

Lidocaine, as an anesthetic substance, is often used for surface and spinal anesthesia. However, studies have shown that lidocaine may induce transient neurological symptoms and cauda equina syndrome. In the present study the effects of the ginsenoside Rg1 (Rg1) on lidocaine‑induced apoptosis were assessed in Jurkat cells using flow cytometry and terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling (TUNEL). The data showed that incubation with Rg1 provides protection against lidocaine‑induced apoptosis in cultured Jurkat cells. In order to investigate the effect of Rg1 on the

BACKGROUND: Rapid rate magnetic stimulation of human sacral nerve roots can reduce stress incontinence.
OBJECTIVE: To test stimulation for urgency incontinence due to lumbosacral plexopathy.
METHODS: The authors studied patients with intractable neurogenic urinary dysfunction due to lumbosacral nerve injuries. Eligible patients were randomized to receive either real or sham repetitive lumbosacral magnetic stimulation (rLMS; 15 Hz with a total of 1500 pulses/session) for 10 sessions. Evaluation was performed before starting treatment, immediately after the 5th and 10th treatment session, and
