A novel planning solution for semi-autonomous aspiration of Baker's cysts.

BACKGROUND: A Baker's cyst is a pathological structure located near a kneepit, which causes discomfort and reduces mobility of the knee. It is commonly treated with aspiration, which often requires MRI scanning and US guidance. The aim of this study was to propose a novel planning solution for semi-autonomous aspiration of the Baker's cyst using only MRI imaging. METHODS: The proposed method requires minimal user input and offers automatic cyst segmentation with collision-free path planning for the assumed robotic structure with four degrees of freedom. RESULTS: The prepared software was tes

Pseudotumor in a well-fixed metal-on-polyethylene uncemented hip arthroplasty.

The incidence of pseudotumor formation has been reported to be 1% in patients with metal-on-metal resurfacing arthroplasties. This complication is not exclusive to these patients. We report a case of pseudotumor formation secondary to femoral head-neck corrosion after a metal-on-polyethylene uncemented total hip arthroplasty.

Intramuscular dissection of Baker's cysts: report on three cases.

Baker's cysts are fluid distensions of the gastrocnemius-semimembranosus bursa and are the most common cystic lesion around the knee. Typically cysts enlarge along intermuscular planes around the knee. We report three cases in which the expanding cyst did not respect these planes and dissected along an intramuscular route as confirmed by MR imaging. Such behaviour by Baker's cysts is hitherto unreported in the literature. Possible mechanisms to account for this phenomenon are discussed.

[Prevalence and characteristics of complications of Baker cysts by MRI].

OBJECTIVE: To evaluate the prevalence and describe the findings of complicated Baker cysts observed in a large series of knee MR studies. MATERIALS AND METHODS: A total of 145 Baker cysts were detected out of a series of 382 (38%) consecutive patients with knee MR studies performed. Cysts were classified as simple (smooth walls, homogeneous content, signal intensity similar to that of the synovial fluid and with no changes in the surrounding tissues) or complicated (changes of wall, changes in the intensity of the content signal, presence of free bodies or changes in the surrounding tissues).

Recurrent diffuse tenosynovial giant cell tumor of the posterior knee: a rare presentation with synovial herniation (Baker's cyst) in a young patient.

INTRODUCTION: Tenosynovial giant cell tumor (TGCT), previously referred to as pigmented villonodular synovitis (PVNS), is a rare, locally aggressive proliferative disorder of the synovium. It typically affects large joints, most commonly the knee. Posterior compartment involvement in the knee is less common compared to anterior or suprapatellar involvement, and it presents unique diagnostic and therapeutic challenges due to the complex anatomy and proximity to neurovascular structures. While popliteal (Baker's) cysts may coexist with intra-articular TGCT, they are considered synovial herniatio

Recurrence of cystic adventitial disease in an interposed vein graft.

A case of cystic adventitial disease (CAD) of the popliteal artery with intermittent claudication in the left calf is reported. This patient was first treated by total excision of the cyst and the involved artery followed by graft interposition with an autogenous saphenous vein. Recurrence of CAD in the interposed vein graft was noted after 6 months, and excision of the cyst with the involved graft and graft interposition with an autogenous saphenous vein was again required. We consider that the definitive evidence from early recurrence in the interposed vein graft shown in this case will clos

A complex assessment of patients with knee osteoarthritis and Baker's cyst: observational study.

In our observational study, we performed the clinical and functional examination, analyzed imagistic and histological findings and evaluated the correlation between previous aspects in patients with primary knee osteoarthritis (PKOA) and Baker's cyst (BC). The correlations were made to better understanding of BC in patients with PKOA and optimal choosing for treatment. Seventy patients with painful PKOA (ACR criteria) and BC were assessed. We evaluated knee pain using a 100 mm VAS and functional status using the pain, stiffness and functional subscales of WOMAC index. All patients were imagist

[Lesions in the popliteal fossa with neurologic symptomatology--case reports, diagnostics and treatment].

The lesions arising in the popliteal fossa are varied. We report two different lesions related to the peroneal nerve that presented with neurologic symptoms. Both were initially treated at surgical department with insufficient diagnostics and finally required a neurosurgical operation. We emphasize performing complete diagnostics that may avoid unnecessary burden for the patient and healthcare personnel. Patients with proven intimate relation of the lesion to nerve structures in the popliteal fossa should be referred to neurosurgical department.

MRI of posterior knee masses.

A wide variety of masses may be visualized in the posterior knee. Some are encountered incidentally on MRI obtained for seemingly unrelated symptoms; others are palpable and present for MRI characterization. We describe the appearance, significance and pathophysiology of some of the most common lesions found in the popliteal fossa as well as several of the less common entities we have encountered. Thorough understanding of the appearance and location of the more specific masses should augment precise diagnosis in a majority of cases. In turn, this will eliminate unnecessary additional investig

["Pseudothrombosis" of the lower leg, induced by a ruptured Baker's cyst].

A sudden onset of unilateral swelling and tenderness on pressure of the calf with no history of trauma is suggestive of vein thrombosis. Baker cysts, especially if ruptured, mimic the clinical picture of deep vein leg thrombosis. The examination method of choice is duplex sonography. We report on a 30-year-old man who was admitted to our hospital with signs of deep vein leg thrombosis. Phlebography revealed normal venous circulation. Examination by duplex sonography showed a ruptured Baker cyst. No intra-articular lesions of the knee joint were revealed by arthroscopy. Histological examination

Post-Traumatic Hemosiderotic Synovitis of the Knee Mimicking Pigmented Villonodular Synovitis on Magnetic Resonance Imaging (MRI) in a Child: A Case Report.

BACKGROUND Hemosiderotic synovitis (HS) is a rare proliferative synovial disorder with incompletely understood pathophysiology. It mainly affects the knee joint. It can be confused with pigmented villonodular synovitis (PVNS), both clinically and radiologically. HS has not been previously reported in children, and this rare incidence may lead to difficulties in early clinical and radiological diagnosis, possibly affecting the patient's outcome. CASE REPORT A 13-year-old boy presented with progressive right knee pain, swelling, and limitation of movement 2 months after a traumatic injury in a s

Is it worth diagnosing and treating distal deep vein thrombosis? No.

The standard diagnostic approach to suspected deep vein thrombosis (DVT) is serial lower limb compression ultrasound (CUS) of proximal veins. Although it only assesses the proximal veins, withholding anticoagulant treatment in patients with a negative CUS on day 1 and after 1 week has been proved safe. In particular, studies evaluating CUS limited to the proximal veins showed a good safety profile with a pooled estimate of the 3-month thromboembolic rate of 0.6% (95% CI, 0.4-0.9%) in non-anticoagulated patients. However, performing two lower limbs CUS is cumbersome and expensive. Recently, stu

Ultrasonographic scan in knee pain in athletes.

Fifty-two knees were examined using real-time high-definition ultrasonography with a 7.5 MHz probe. The extra-articular structures were easily visualized and diagnosis of patellar tendon lesions and Baker's cysts formulated. While the meniscal cartilages were shown as a homogeneous triangular structure between the femoral condyle and the tibial plateau, no lesions were detected. Deeper intra-articular structures, such as the cruciate ligaments, were not shown by the scan, thus their evaluation was not possible. Given its low cost, wide availability, non-invasiveness and patients' acceptability

Candida arthritis in adult patients who are not intravenous drug addicts: report of three cases and review of the literature.

Candida species are an uncommon cause of infectious arthritis, although the frequency has increased during recent years. Three cases of septic arthritis caused by Candida species are reported, and the literature is reviewed. The first patient developed a popliteal cyst infected by Candida albicans after multiple intravenous antibiotic treatments. The second patient had acute myelogenous leukemia and experienced knee arthritis after chemotherapy, and the third suffered oligoarthritis after a second heart transplantation. All patients received treatment with a standard dose of intravenous amphot
