Increasing trends in mortality and costs of infectious diseases in Korea: trends in mortality and costs of infectious diseases.

OBJECTIVES: In an era when the average life expectancy and overall mortality rate have improved, Korea remains at risk for infectious disease outbreaks that place substantial burdens on the healthcare system. This study investigated trends in mortality and the economic burden of infectious diseases. METHODS: Healthcare data from the Health Insurance Review and Assessment Service (2009-2019) and the Korean Statistics Information Service (1997-2019) were used. We selected 10 infectious disease groups (intestinal infections, tuberculosis, vaccine- preventable diseases, sepsis, viral hepatitis, H

The Effect of Immigration on Adverse Perinatal Outcomes: Analysis of Experiences at a Turkish Tertiary Hospital.

INTRODUCTION: In literature, it is well documented that migration is associated with adverse perinatal outcomes in many countries over the world. But in Turkey, health care providers and obstetricians had to face the effects of migration for the first time after civil war in Syria. Hence, this situation motivated us to conduct the current research in Turkey. Also we aimed to evaluate the effect of immigration on adverse perinatal outcomes, comparing the obstetric results of a native population and an immigrant population, and focusing on relevant indicators of perinatal health. METHODS: Infor

CKD surveillance using administrative data: impact on the health care system.

The US Renal Data System Coordinating Center identifies patients with chronic kidney disease (CKD) in administrative data sets and determines the overall size of the population with a diagnosis of CKD, with such major comorbid conditions as diabetes and congestive heart failure, and associated expenditures. A 2-year observation period is used to define individuals with at least 1 International Classification of Diseases, Ninth Revision, Clinical Modification diagnosis code from inpatient claims or at least 2 from outpatient claims or physician and supplier service claims for kidney disease and

Fear, fatalism and breast cancer screening in low-income African-American women: the role of clinicians and the health care system.

BACKGROUND: African-American women have the highest breast cancer death rates of all racial/ethnic groups in the US. Reasons for these disparities are multi-factorial, but include lower mammogram utilization among this population. Cultural attitudes and beliefs, such as fear and fatalism, have not been fully explored as potential barriers to mammography among African-American women. OBJECTIVE: To explore the reasons for fear associated with breast cancer screening among low-income African-American women. METHODS: We conducted four focus groups (n = 29) among a sample of African-American wome

Five-year relative survival for human papillomavirus-associated cancer sites.

BACKGROUND: Human papillomavirus (HPV) vaccines can potentially prevent greater than 90% of cervical and anal cancers as well as a substantial proportion of vulvar, vaginal, penile, and oropharyngeal cancers caused by certain HPV types. Because more than 38,000 HPV-associated cancers are diagnosed annually in the United States, current studies are needed to understand how relative survival varies for each of these cancers by certain demographic characteristics, such as race and age. METHODS: The authors examined high-quality data from 27 population-based cancer registries covering approximate

Impact of pneumococcal conjugate vaccines on nasopharyngeal carriage and invasive disease among unvaccinated people: review of evidence on indirect effects.

BACKGROUND: Invasive disease due to Streptococcus pneumoniae remains an important worldwide cause of morbidity and mortality, particularly in young children and the elderly. The development and use of pneumococcal conjugate vaccines (PCVs) have had a dramatic impact on rates of vaccine-type invasive pneumococcal disease (IPD) not only in the pediatric population targeted for vaccination but in non-vaccinated age-groups as well. This indirect effect is directly mediated by a reduction of vaccine-type nasopharyngeal carriage and thus transmission by vaccinated children. Current PCV licensing pro

Genome-Wide Association Study and Rare Variant Association Studies of Strabismus in the All of Us Research Program.

OBJECTIVE: Despite significant evidence of a genetic contribution to strabismus, precise genetic mechanisms have not been identified. There are distinct population differences in the prevalence of strabismus and its subtypes. This study aimed to explore the genetic contributions to strabismus in different ancestral groups. DESIGN: Case-control. PARTICIPANTS: The All of Us Research Program includes genotypic and phenotypic data from a diverse population of adults (age ≥18 years at time of enrollment) across the United States. Among participants with whole-genome sequences available, strabismu

Effect of Technology-Enhanced Screening in Addition to Standard Targeted Clinician Education on the Duration of Untreated Psychosis: A Cluster Randomized Clinical Trial.

IMPORTANCE: Reducing the duration of untreated psychosis (DUP) is essential to improving outcomes for people with first-episode psychosis (FEP). Current US approaches are insufficient to reduce DUP to international standards of less than 90 days. OBJECTIVE: To determine whether population-based electronic screening in addition to standard targeted clinician education increases early detection of psychosis and decreases DUP, compared with clinician education alone. DESIGN, SETTING, AND PARTICIPANTS: This cluster randomized clinical trial included individuals aged 12 to 30 years presenting for

Trauma patients at risk for venous thromboembolism who undergo routine duplex ultrasound screening experience fewer pulmonary emboli: A prospective randomized trial.

BACKGROUND: Although guidelines are established for the prevention and management of venous thromboembolism (VTE) in trauma, no consensus exists regarding protocols for the diagnostic approach. We hypothesized that at-risk trauma patients who undergo duplex ultrasound (DUS) surveillance for lower extremity deep venous thrombosis (DVT) will have a lower rate of symptomatic or fatal pulmonary embolism (PE) than those who do not undergo routine surveillance. METHODS: Prospective, randomized trial between March 2017 and September 2019 of trauma patients admitted to a single, level 1 trauma center

Health reform and the US Virgin Islands: high-need-limited impact.

OBJECTIVES: There are 4.1 million residents living in the US territories, which is more than the combined population of many US states, yet the territories and their citizens are often overlooked from a policy perspective, because most individual territories are relatively small, geographically isolated, and have been treated differently than the states historically. This tendency to fall beneath the radar is clear in the realm of health policy, especially in the area of insurance coverage. This article provides an initial assessment of the potential impact of health reform on the US Virgin Is

Mortality Due to Aortic Dissection in Adults With Primary Hypertension: A Nationwide Analysis Over Two Decades.

OBJECTIVE: Hypertension is a key risk factor for aortic dissection (AD). AD, if left untreated, carries significant mortality rates. Our aim is to analyse trends in mortality due to AD in adults with primary hypertension in the United States (US). METHODS: We used the CDC WONDER database to extract mortality data for patients with primary hypertension who died due to AD. Age-adjusted mortality rates (AAMRs) and crude mortality rates (CMRs) were extracted per 100 000 persons. Annual percentage changes (APCs) and average APCs (AAPCs) in AAMRs and CMRs were calculated using Joinpoint regression.

Locations of emergency medical services attended non-fatal opioid-involved overdoses, 888 counties, USA, 2022-2024.

BACKGROUND: Emergency medical service (EMS) encounters for non-fatal opioid overdose in the USA increased from 2018 to 2022, and persons treated for an overdose by EMS increasingly refuse EMS transport to emergency departments after an overdose. Understanding where non-fatal overdoses occur can inform overdose prevention and response efforts. METHODS: EMS data from biospatial by ImageTrend were queried from 888 counties reporting consistently during January 2022 to December 2024, representing approximately 30.6% of the US population. Frequencies and proportions of non-fatal opioid-involved ov

Development, Early Verification and Application of a North American Pediatric PBPK Model, Comparison With the North European Pediatric Population.

Physiologically based pharmacokinetic (PBPK) models are increasingly used in pediatric drug development and matching such models to the target population is an important consideration. A US pediatric PBPK model was developed incorporating four sub-groups describing physiology and enzymatic/transporter levels for White, African American, Hispanic/Latino and Asian subjects. Virtual populations could then be created reflecting the demographics of the clinical study design. Ten different drugs (14 studies) were used to verify the pediatric PBPK model performance. Predicted PK parameters showed rea

Cancer Screening Rates, Disparities by Immigration Status, and Predictors of Cancer Screening Among Black Women With HIV.

INTRODUCTION: People with HIV have a higher incidence and mortality from screenable cancers than the general population. Among women, the 4 most common cancers are breast, colorectal, lung, and cervical cancers. This study examined breast, colorectal, lung, and cervical cancer screening rates among Black women with HIV and disparities that exist by immigration status. METHODS: With a sample of 604 women, we examined cancer screening rates for breast, colorectal, lung, and cervical cancers and examined the rates by country of origin. Generalized linear mixed models were used to examine potenti

Penile Human Papillomavirus Prevalence and Genotype Co-Detection Patterns in US Men: A Nationally Representative Analysis, 2013-2016.

Human papillomavirus (HPV) surveillance often treats genotypes as independent events. However, shared host, behavioral, biological, and assay-related factors may produce non-independent genotype co-detection patterns. We quantified the burden and observed co-detection patterns of penile HPV in a nationally representative sample of US men to establish a descriptive reference for future surveillance. We analyzed men aged 18-59 years participating in the National Health and Nutrition Examination Survey (NHANES) 2013-2016. Penile swabs were assayed using the Roche Linear Array. We estimated the po

Frailty in chronic myeloid leukemia: evidence from 2016-2018 Nationwide Inpatient Sample of the US.

BACKGROUND: Frailty is a marker of poor prognosis in older adults with hematologic malignancies and contributes to the severe vulnerability of the aging population to adverse health outcomes. This study aimed to determine the association between frailty and outcomes in hospitalized patients with chronic myeloid leukemia (CML). METHODS: The International Classification of Diseases (ICD-10) identified data on hospitalized patients 20 years or older admitted with CML between 2016 and 2018 in the US National Inpatient Sample (NIS) database. The cohort was further divided into groups of patients w

Racial and ethnic disparities in the management of acute pain in US emergency departments: Meta-analysis and systematic review.

OBJECTIVE: This review aims to quantify the effect of minority status on analgesia use for acute pain management in US Emergency Department (ED) settings. METHODS: We used the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) methodology to perform a review of studies from 1990 to 2018 comparing racial and ethnic differences in the administration of analgesia for acute pain. Studies were included if they measured analgesia use in white patients compared to a racial minority in the ED and studies were excluded if they focused primarily on chronic pain, case reports an

Epidemiology and location of rugby injuries treated in US emergency departments from 2004 to 2013.

Rugby participation in the US is increasing, and with its inclusion in the 2016 Summer Olympics, the increased participation rates are expected to continue. Naturally, as participation increases, so too do rugby-related injuries. The difference in injury patterns with regard to age and gender illustrates differences in how the game is being played. Understanding what accounts for these emerging injury patterns will help guide future injury prevention efforts. This study provides an update on injury rates for the growing population of rugby players in the US, especially young players. Our resul

Dietary patterns associated with dental caries in adults in the United States.

OBJECTIVES: Dental caries experience, which affects 91% of US adults, is a consequence of a carious process influenced by diet. Although individual foods have been implicated, we hypothesized that dietary patterns might be important predictors of caries presence. METHODS: We analysed data from 4467 people ≥18 years old participating in the 2013-2014 National Health and Nutrition Examination Survey, a nationally representative sample of the US population. Data from 24-hour dietary recalls were classified into standard food categories and reduced to three dietary patterns using principal compon
