Neuroendocrine Dysfunction in a Young Athlete With Concussion: A Case Report.

An 18-year-old female ringette and basketball player presented to our sport concussion clinic 27 months after concussion with fatigue, headache, exercise intolerance, polyuria, nocturia, and difficulties concentrating. Her history was remarkable for 4 previous concussions. Her neurologic examination was normal. Neuroendocrine screen including thyroid function, morning cortisol, glucose, and insulin-like growth factor-1 (screening test for growth hormone deficiency) were normal. Further testing for growth hormone deficiency with an insulin hypoglycemia test revealed severe growth hormone defici

Improvements in balance control in individuals with PCS detected following vestibular training: A case study.

Concussed individuals have been found to experience balance deficits in the anterior-posterior (AP) direction as indicated by greater Center of Pressure (COP) displacement and velocity. One possible reason for this change in balance control could be due to damage to the lateral vestibulospinal tract which sends signals to control posterior muscles, specifically ankle extensors leading to compensatory torques about the ankle. The purpose of the study was to quantify balance assessments in individuals experiencing persistent post-concussion symptoms (PCS) to determine balance control changes fol

An examination of the expected symptoms of postconcussion syndrome in a nonclinical sample.

OBJECTIVES: This study examined the robustness of the postconcussion syndrome (PCS) expectation-as-etiology hypothesis using a variation on the typical study design and different symptom assessment methods. METHODS: Participants were 51 university students with no history of brain injury or neurological disease. Participants reported expected PCS symptoms after being exposed to an established vignette in which they were asked to imagine that they had had a car accident (expectation condition). One week later, they reported their current symptoms (control condition). Symptoms were elicited in

Pathology of blast-related brain injury.

Blasts are responsible for about two-thirds of the combat injuries in Operation Iraqi Freedom and Operation Enduring Freedom, which include at least 1,200 traumatic brain injuries. Blasts inflict damage to the brain directly and by causing injuries to other organs, resulting in air emboli, hypoxia, and shock. Direct injuries to the brain result from rapid shifts in air pressure (primary blast injury), from impacts with munitions fragments and other objects propelled by the explosion (secondary blast injury), and from collisions with objects and rapid acceleration of individuals propelled by th

[Mild head trauma: complications and acousticovestibular sequelae].

Head injury with fracture of the petrous bone is frequent and directly related to the development of human activity. Head trauma is associated with nearly 75% of traffic accidents; 5% of them with petrous bone fracture. Based on a clinical case, the acute injuries and secondary acoustic or vestibular sequelae after low-energy cranio-cerebral trauma are discussed here. The subject was a victim of a low-energy head trauma with brief loss of consciousness and translabyrinthine fracture of the petrous bone. This fracture caused invalidating equilibrium disorders related to a perilymphatic fistula.

Serum calcitonin gene-related peptide in patients with persistent post-concussion symptoms, including headache: a cohort study.

BACKGROUND: Calcitonin gene-related peptide (CGRP) plays an important role in migraine pathophysiology, and post-traumatic headache (PTH) frequently presents with migraine-like features. Despite several clinical similarities, few studies have explored CGRP in PTH and concussion. This study investigates serum CGRP levels in patients with persistent post-concussion symptoms (PPCS), including PTH. METHODS: This cohort study was based on serum samples from individuals aged 18-30 years with PPCS who participated in a previously published randomized controlled trial of a non-pharmacological interve

Gastrointestinal Symptoms After Sport-Related Concussion in Irish Athletes.

Background/Objectives: Sport-related concussion (SRC) elicits multi-systemic symptoms, including nausea, fatigue, and cognitive changes. Gastrointestinal (GI) symptoms are not well captured in current concussion assessments and may be under-recognised in clinical follow-up. GI disturbances may influence intake tolerance and day-to-day fuelling during post-SRC recovery. This study investigated the prevalence and severity of self-reported GI symptoms in Irish athletes after their most recent SRC, examined sex-based patterns, and evaluated the rationale for integrating GI symptom checks into stan

The Relationship Between Sex, Fatigue, and Post-Concussion Symptoms Differs Between Those With Mild Traumatic Brain Injury Versus Trauma Control and Healthy Control Participants.

OBJECTIVE: Sex, fatigue and subjective sleep disruption have been shown to be associated with poor subjective recovery after mild traumatic brain injury (mTBI). This study aimed to determine whether the relationship between these factors and post-concussion symptom (PCS) reporting are unique to individuals who have incurred an mTBI. Setting, Participants, Design and Main Measures: In this observational study, a group of 208 pre-morbidly healthy adults was assessed with measures of fatigue (Multi-dimensional Fatigue Inventory), subjective sleep disturbance (Pittsburgh Sleep Quality Index), depr

Recovery, interrupted: persisting symptoms after concussion and the power of iatrogenesis.

Persisting symptoms after concussion (PSaC) affect up to 30% of children, adolescents, and young adults beyond 1 month post-injury, posing challenges in clinical care. This retrospective study examined 54 patients referred for neuropsychological evaluation due to PSaC, exploring factors contributing to symptom persistence. Results showed that 75.9% of patients experienced symptoms attributable to non-concussion factors, with 90.7% lacking evidence-based care post-injury. Pre-existing conditions, including migraines, anxiety, depression, and other psychiatric disorders, correlated with symptom

Influence of Preexisting Conditions and Concussion History on Postconcussion Symptom Severity and Recovery Time in Collegiate Athletes.

Mental health conditions and concussion history reported by a collegiate athlete may contribute to prolonged recovery and symptom severity after concussion. This work examined the potential associations among concussion history, preexisting conditions, and sex relative to initial symptom severity and recovery duration following sport-related concussion (SRC) in a cohort of Division 1 National Collegiate Athletic Association athletes. This prospective cohort study analyzed symptom severity, recovery, and return-to-play (RTP) times reported post-SRC using data collected as part of the Pac-12 Con

Can Exposure Therapy Be Effective for Persistent Post-Concussion Symptoms? A Nonconcurrent Multiple Baseline Design Across 4 Cases.

OBJECTIVE: After a concussion, some patients develop persistent post-concussion symptoms, which interferes with functioning in daily life. A biopsychosocial explanation for the development and continuation of persistent post-concussion symptoms is the fear avoidance model (FAM). This study aimed to investigate the effectiveness and feasibility of an individual 14-week exposure therapy for patients with persistent symptoms after concussion. PARTICIPANTS: Four participants from a medical psychology outpatient clinic participated in the study. Their age ranged between 51 and 68 years old ( M = 6

Long-term factor structure of the Rivermead Post-Concussion Symptom Questionnaire in mild traumatic brain injury and normative sample.

BACKGROUND: Previous studies of the Rivermead Post-Concussive Questionnaire(RPQ)'s factor structure were conducted within 1 year post-injury. Post-concussive symptoms may persist, and are common in the general population, so determining if the factor structure in mild-TBI and controls differ is important. This study examined factor structure of the RPQ in adults 4 years post-mild-TBI and in age-/gender-matched controls. METHOD: A total of 232 adults 4 years post-mild-TBI and 232 age-/gender-matched controls completed the RPQ. RESULTS: Both samples produced a three-factor solution (57.98% and

The Sport Concussion Assessment Tool (SCAT2) for evaluating civilian mild traumatic brain injury. A pilot normative study.

Self-report measures, particularly symptom inventories, are critical tools for identifying patients with persistent post-concussion symptoms and their follow-up. Unlike in military or sports-related assessment, in general civilian settings pre-injury levels of concussion-like symptoms are lacking. Normative data are available in adolescent and college populations, but no reference data exist to guide clinical adult explorations. The purpose of this study was to use the second edition of the Sport Concussion Assessment Tool (SCAT2) to profile a cohort of 60 healthy community volunteers who had
