Continuity of Care at the Edges of America: Rethinking Health for Migrant and Seasonal Agricultural Workers in the U.S. Midwest.

Migrant and seasonal agricultural workers are essential to the food systems of the United States (U.S.), yet they experience some of the most persistent health inequities in the nation. In the U.S. Midwest, particularly Iowa and Nebraska, workers arriving from Mexico and Central America confront structural, linguistic, geographic, and policy barriers that disrupt continuity of care and limit access to preventive services. Despite carrying disproportionate burdens of chronic diseases such as diabetes, hypertension, and musculoskeletal conditions, migrant and seasonal workers face fragmented hea

A need to integrate healthcare services for HIV and non-communicable diseases: An Indian perspective.

With the decline in HIV mortality, a concomitant increase in morbidity and death not directly related to HIV has been witnessed. Consequently, many countries especially low- and middle-income countries (LMICs) are now facing the dual burden of HIV and non-communicable diseases (NCDs). 2.3 million people living with HIV in India are at a higher risk of developing NCDs due to ageing, which can be attributed to the additional impact of long-standing HIV infection and the side-effects of antiretroviral therapy. This has led to a rise in demand for a combined health system response for managing HIV

The Catholic Appeal to Health: A Pastoral Analysis on the Catholic Church's View of Health Inequity in Health Outcomes and Diseases in the Philippines.

Health inequities in the Philippines remain a critical challenge, with marginalized populations, particularly those in poor rural communities, disproportionately affected by both communicable and non-communicable diseases. These groups face higher risks of illnesses such as tuberculosis, dengue, and respiratory infections due to inadequate sanitation, poor nutrition, and limited access to healthcare services. At the same time, the growing prevalence of chronic conditions like hypertension and diabetes further burdens low-income populations who struggle to afford preventive care, treatment, and

A pox on all our houses: a missing component in the global mpox response is equity.

The global health community has had more than a decade to develop pandemic preparedness programmes and apply lessons learnt to new disease outbreaks. Mpox has now been declared a global public health emergency, but the response appears to be missing important elements of equity, focusing instead on diagnosis and surveillance. This approach leaves vulnerable populations in countries grappling with the outbreak without the preventive and treatment services they need. Based on our experiences managing the current mpox outbreak in Burundi, we outline some limitations of ongoing response strategies

"Service with open arms": enhancing community healthcare experiences for individuals with a history of incarceration.

BACKGROUND: The criminal justice-involved population has a higher disease burden than the general population and a high risk of death post-incarceration. However, this group underutilizes healthcare, especially preventive and primary care services. Sixteen in-person, semi-structured interviews were conducted with formerly incarcerated individuals in Milwaukee to explore health impacts of incarceration, barriers and facilitators to healthcare access, and what ideal health service provision would look like following incarceration. Interviews were transcribed, coded, and analyzed using an immersi

Health Behaviours and Experiences With Health Care Systems Among Thai Immigrants: A Scoping Review.

The health behaviours and health care experiences of Thai immigrants are relevant in the context of growing global migration, as they navigate health care systems across their origin and host countries. While numerous studies have examined these issues, existing evidence remains fragmented across countries, study designs, and population subgroups, limiting understanding of how Thai immigrants manage their health and engage with health care systems. This scoping review aimed to identify, categorise, and summarise existing knowledge on Thai immigrants' health behaviours and health care experienc

Improving public health through access to and utilization of medication assisted treatment.

Providing access to and utilization of medication assisted treatment (MAT) for the treatment of opioid abuse and dependence provides an important opportunity to improve public health. Access to health services comprising MAT in the community is fundamental to achieve broad service coverage. The type and placement of the health services comprising MAT and integration with primary medical care including human immunodeficiency virus (HIV) prevention, care and treatment services are optimal for addressing both substance abuse and co-occurring infectious diseases. As an HIV prevention intervention,

Evaluating the role of youth-onset diabetes mellitus in the context of food insecurity and preventive care: a repeated cross-sectional study using propensity score weighted machine learning analysis.

BACKGROUND: Youth-onset diabetes mellitus (DM) is an increasing public health concern, especially among adolescents facing socioeconomic challenges. Food insecurity (FI), a key social determinant of health, has been linked to adverse metabolic outcomes, but its association with youth-onset DM and the role of preventive care remain unclear. METHODS: Using repeated cross-sectional data from the National Survey of Children's Health (2016-2021), we examined the association between FI status (food secure, mild FI and moderate-to-severe FI) and youth-onset DM among adolescents aged 10-17 years. Interventions by Health Care Professionals Who Provide Routine Child Health Care to Reduce Tobacco Smoke Exposure in Children: A Review and Meta-analysis.

IMPORTANCE: Reducing child exposure to tobacco smoke is a public health priority. Guidelines recommend that health care professionals in child health settings should address tobacco smoke exposure (TSE) in children. OBJECTIVE: To determine the effectiveness of interventions delivered by health care professionals who provide routine child health care in reducing TSE in children. DATA SOURCES: A secondary analysis of 57 trials included in a 2014 Cochrane review and a subsequent extended search was performed. Controlled trials (published through June 2015) of interventions that focused on reduc

Conquering the intolerable burden of malaria: what's new, what's needed: a summary.

Each year, up to three million deaths due to malaria and close to five billion episodes of clinical illness possibly meriting antimalarial therapy occur throughout the world, with Africa having more than 90% of this burden. Almost 3% of disability adjusted life years are due to malaria mortality globally, 10% in Africa. New information is presented in this supplement on malaria-related perinatal mortality, occurrence of human immunodeficiency virus in pregnancy, undernutrition, and neurologic, cognitive, and developmental sequelae. The entomologic determinants of transmission and uses of model

Reaching Men and Women Through a Decentralized Mobile Service Delivery Model for HIV Prevention and Pre-Exposure Prophylaxis Services in South Africa.

INTRODUCTION: Structural and service delivery barriers such as long waiting times, health care provider attitudes, and distance from services prevent access to HIV prevention and pre-exposure prophylaxis (PrEP) services, particularly for young people. The World Health Organization recommends differentiated service delivery models for HIV prevention and treatment services, including through mobile outreach. Mobile health clinics offer an opportunity to overcome barriers to service access by bringing services closer to places of work or study, improving convenience, minimizing stigma, and expand

Fetal alcohol spectrum disorder: a systematic review of the cost of and savings from prevention in the United States and Canada.

BACKGROUND AND AIMS: Fetal alcohol spectrum disorder (FASD) is a preventable condition that imposes a significant financial burden on societies. Funding of FASD prevention is a small portion of the total expenditures associated with FASD. This paper aimed to review the literature on the costs of and savings from prevention of FASD and present a model for the United States and Canada of projected savings based on expansion of existing evidence-based prevention models. METHODS: A systematic review of published literature on the cost of FASD prevention was conducted and experts in the field were

Commentary: restarting NTD programme activities after the Ebola outbreak in Liberia.

It is widely known that the recent Ebola Virus Disease (EVD) in West Africa caused a serious disruption to the national health system, with many of ongoing disease focused programmes, such as mass drug administration (MDA) for onchocerciasis (ONC), lymphatic filariasis (LF) and schistosomiasis (SCH), being suspended or scaled-down. As these MDA programmes attempt to restart post-EVD it is important to understand the challenges that may be encountered. This commentary addresses the opinions of the major health sectors involved, as well as those of community members, regarding logistic needs and

Scrub typhus in Bhutan: a synthesis of data from 2009 to 2014.

Scrub typhus is an acute, febrile illness, caused by the bacterium Orientia tsutsugamushi, that affects millions annually in the endemic Asia-Pacific region. In untreated cases, the case-fatality rates range from 6% to 35%. In Bhutan, there was a probable outbreak in Gedu in 2009, which resulted in heightened awareness of the disease. Nevertheless, information on scrub typhus in Bhutan is limited and scattered and the epidemiology has yet to be established. To report the current picture of scrub typhus in Bhutan, this review gathered data from scholarly databases, surveillance reports, the Ann

Breast self-examination: how committed are women? How committed are we?

Breast self-examination (BSE), increasingly promoted as a good health habit, is gaining recognition by surgeons as a significant factor in tumour size and staging at diagnosis. Clinically-based reports leave unanswered important behavioural questions and pay scant attention to the quality of the teaching or of the practice of BSE. This paper argues that if BSE is to be used as the coarse-screening modality of which it seems to be capable, we must apply to it the same criteria as to other screening modalities. Since BSE ipso facto must be undertaken by the women herself, it is a screening in wh

Principles and lessons from the smallpox eradication programme.

The eradication of smallpox required a unique, fully collaborative international effort on the part of WHO and Member States. In the course of the programme, many lessons were learned in its organization, execution and evaluation which have implications for other international activities. Most important among these was the need to establish measurable objectives and to evaluate progress and performance in terms of these; to establish procedures for quality control both of vaccines and performance; to recruit the best possible personnel and support them; and to assure an on-going programme of p
