[Correspondence between primary and secondary healthcare providers about patients with cancer; how can it be improved?].

OBJECTIVE: To explore the correspondence between primary and secondary healthcare providers about patients with lung, breast or colorectal cancer. DESIGN: Qualitative research. METHOD: We collected the medical files of 50 patients with lung, breast or colorectal cancer by purposive sampling and selected the correspondence-related items from them. These concerned referral letters from primary to secondary caregivers and letters from specialists. A qualitative content analysis of these documents was performed. In addition, 4 general practitioners, 4 oncologists and 1 nurse specialist were inte

Time to End Physician Sexual Abuse of Patients: Calling the U.S. Medical Community to Action.

Despite the strict prohibition against all forms of sexual relations between physicians and their patients, some physicians cross this bright line and abuse their patients sexually. The true extent of sexual abuse of patients by physicians in the U.S. health care system is unknown. An analysis of National Practitioner Data Bank reports of adverse disciplinary actions taken by state medical boards, peer-review sanctions by institutions, and malpractice payments shows that a very small number of physicians have faced "reportable" consequences for this unethical behavior. However, physician self-

A balanced approach to falls prevention: Application in the real world.

BACKGROUND: Falls place a heavy burden on the health system, impacting on an individual's quality of life, often resulting in a fear of falling, reduction in independence and at times admission to residential care. This study aimed to determine health professionals' confidence in discussing falls prevention strategies, topics discussed and the barriers and enablers to falls prevention discussions with clients aged 60 years and over. METHOD: A cross-sectional self-complete online survey was undertaken with a sample of health professionals (n = 191) who had engaged in the services of the Stay O

Role of Genetic Testing in Inherited Cardiovascular Disease: A Review.

IMPORTANCE: Genetic testing is a valuable tool for managing inherited cardiovascular disease in patients and families, including hypertrophic, dilated, and arrhythmogenic cardiomyopathies and inherited arrhythmias. By identifying the molecular etiology of disease, genetic testing can improve diagnostic accuracy and refine family management. However, unique features associated with genetic testing affect the interpretation and application of results and differentiate it from traditional laboratory-based diagnostics. Clinicians and patients must have accurate and realistic expectations about the

Sexual Violation of Patients by Physicians: A Mixed-Methods, Exploratory Analysis of 101 Cases.

A mixed-method, exploratory design was used to examine 101 cases of sexual violations in medicine. The study involved content analysis of cases to characterize the physicians, patient-victims, the practice setting, kinds of sexual violations, and consequences to the perpetrator. In each case, a criminal law framework was used to examine how motives, means, and opportunity combined to generate sexual misconduct. Finally, cross-case analysis was performed to identify clusters of causal factors that explain specific kinds of sexual misconduct. Most cases involved a combination of five factors: ma

Therapeutic Alliance in Internet-Delivered Cognitive Behaviour Therapy for Depression or Generalized Anxiety.

UNLABELLED: There has been limited research on therapeutic alliance in the context of therapist-assisted Internet-delivered cognitive behaviour therapy (ICBT) when delivered in clinical practice. The present study investigated therapeutic alliance in ICBT delivered to patients seeking treatment for symptoms of depression (n = 83) or generalized anxiety (n = 112) as part of an open dissemination trial. ICBT was provided by 27 registered therapists or 28 graduate students working in six geographically dispersed clinics; therapist-assistance was delivered primarily through secure messages and occ

A system of evidenced-based techniques and collaborative clinical interventions with a chronically ill patient.

Although evidence-based treatments (EBTs) exist for both bulimia nervosa and binge eating disorder, there is far less evidence underpinning the treatment of anorexia nervosa (AN). Furthermore, there is no clearly defined standardized approach to patients who have not responded to treatment over an extended period of time. Chronic eating disorder patients in particular might need long-term engagement with treatment providers offering a wide range of interventions. This case study highlights how an experienced private practitioner systematically employed a variety of EBT techniques for a patient

Performance and determinants of routine immunization coverage within the context of intensive polio eradication activities in Uttar Pradesh, India: Social Mobilization Network (SM Net) and Core Group Polio Project (CGPP).

BACKGROUND: Studies that have looked at the effect of polio eradication efforts in India on routine immunization programs have provided mixed findings. One polio eradication project, funded by US Agency for International Development (USAID) and carried out by the CORE Group Polio Project (CGPP) in the state of Uttar Pradesh of India, has included the strengthening of routine immunization systems as a core part of its polio eradication strategy. This paper explores the performance of routine immunization services in the CGPP intervention areas concurrent with intensive polio eradication activit

Psychotherapy for social phobia: how do alliance and cognitive process interact to produce outcome?

The purpose of this study was to ascertain whether and how alliance and specific cognitive process may interact to influence outcome. Eighty social phobic patients were randomized to 10-week residential cognitive or interpersonal therapy, conducted mostly in groups. They completed process and outcome measures weekly. The ratings were analyzed with mixed models. It was found that initial patient-rated alliance predicted the course of social anxiety throughout therapy and that this effect was indirect through the cognitive process. However, this indirect effect did not interact with treatment. T

A comparison of consumer and provider preferences for research on homeless veterans.

Despite the dramatic growth of homelessness research, there have been no systematic assessments of consumer and provider preferences regarding the content of this research. Therefore, 87 clients and 28 staff of a homeless veterans program were administered a 15-item questionnaire requesting identification of the 5 "most" and 5 "least" important research topics. Staff and clients differed significantly on 6 items considered most important and 4 items considered least important. Clients wanted more research that focused on material needs, whereas staff preferences were more broadly distributed.

Formal long-term care networks. Forming a partnership with gerontological nurses.

Faced with overwhelming demands on their time, physical reserves, and emotional energy, contemporary families are depending more and more on formal networks to help them with their elder care responsibilities. In the past, these interactions worked in a somewhat complimentary fashion with each caregiver having identifiable roles and responsibilities. Now, reforms in health and human services are pushing and pulling families into complex caregiving partnerships. These situations have the very real potential for creating stress, strain, tension and conflict among caregivers, as well as opportuni

False allegations of sexual misconduct: clinical and institutional considerations.

That some therapists engage in sexual misconduct with patients has been unequivocally established. In recent years, however, some patients' allegations of sexual misconduct have been determined to be false. This paper describes four cases in which hospitalized psychiatric patients made false allegations. Such allegations may result from patients' seeking to gain monetarily or in other ways or from a desire for retaliation or revenge against a clinician who they believe has scorned, abandoned, or otherwise mistreated them. In other cases, especially among patients with a history of severe traum

Interactions between peers with and without severe handicaps: dynamics of teacher intervention.

The impact of teachers' verbal interventions upon the dyadic social interactions between 40 elementary school children with severe mental retardation and their regular-education peers was investigated. All interactions occurred in the context of a structured program designed to facilitate social interactions. Half of the dyads were further instructed in specific cooperative play behavior. Students' and teachers' behaviors were coded. Results showed that the intervention initially positively affected various social play behavior; however, these effects diminished or reversed themselves as the i

Accountability: a right of the rape victim.

Accountability by the professional is especially important in a crisis situation. Rape is experienced by victims as a life threatening situation and it thus triggers an acute stress reaction which requires them to go through a long-term reorganization process. As a crisis situation, it makes them hypersensitive to the attitudes of those people to whom they turn for help and assistance. Clients--in this case rape victims--benefit when professionals are careful to be accountable to them. The victim counselors defined and redefined their role to the victims through their work with the victim as w

Problems of internalization: a button is a button is-not.

Analysts hope to help the patient internalize a relationship with the analyst that contrasts with the original archaic object relation. In this paper, the author describes particular difficulties in working with a patient whose defenses and anxieties were bulimic, her movement toward internalization inevitably undone. Several issues are considered: how does the nonsymbolizing patient come to internalize the analyst's understanding, and when this does not hold, what is the nature of the patient's subsequent methods of dispersal? When the patient can maintain connection to the analyst as a good

Tips for managing children with attention deficit hyperactivity disorder in the dental setting.

It has been estimated that between 3% and 7% of children suffer from attention deficit hyperactivity disorder (ADHD). It is probable, therefore, that these children will present in any dental office. ADHD is characterized by inattention, overactivity and impulsivity. Such symptoms can be quite severe and often present challenges in the dental setting. ADHD symptoms are often managed by medication; and it is important for the clinician to be familiar with these medications to determine when to schedule dental appointments. In dealing with ADHD children, adjunctive behavioral strategies are ofte
