The treatment of orthopedic implant-associated infections (IAIs) presents significant challenges due to increasing drug resistance. This study reports a virulence-selective trap-capture-kill antibacterial system for effectively treating persistent infections while promoting local bone tissue regeneration by regulating disordered immune and metabolic microenvironments caused by IAIs. Here, this work fabricates a nanostructured antibacterial system (F-14-TNA) based on a TiO2 nanotube array modified with a cationic pentacyclic coplanar backbone, fascaplysin derivative 14 (F-14). The modified surf

Periprosthetic joint infection (PJI) remains a severe complication after arthroplasty, with rates expected to rise given the ageing population and their increasing rates of comorbidity. Management is challenging due to the interplay between surgical strategies, antibiotic treatment, and patient-specific factors. Traditionally, prolonged courses of antibiotics were used; however, extending therapy may not improve outcomes, and raises risks such as toxicity and antimicrobial resistance. Recent evidence suggests that shorter antibiotic regimens and early transition to oral antibiotics can be equa

Biofilm eradication remains a significant challenge in clinical practice and contributes substantially to healthcare costs. Medical devices serve as a common substrate for biofilm formation and are implicated in a considerable proportion of invasive candidiasis cases. Rezafungin, a next-generation echinocandin, exhibits broad-spectrum fungicidal activity against Candida spp. and in vitro studies have confirmed its efficacy against Candida biofilms. A literature review up to July 2025 was conducted, including PubMed, Embase, Web of Science, and gray literature, focusing on the real-world use of

SUMMARYImplant-associated infections (IAIs) pose serious threats to patients and can be associated with significant morbidity and mortality. These infections may be difficult to diagnose due, in part, to biofilm formation on device surfaces, and because even when microbes are found, their clinical significance may be unclear. Despite recent advances in laboratory testing, IAIs remain a diagnostic challenge. From a therapeutic standpoint, many IAIs currently require device removal and prolonged courses of antimicrobial therapy to effect a cure. Therefore, making an accurate diagnosis, defining

Currently, despite advancements in diagnostic and therapeutic modalities, osteomyelitis and prosthetic joint infection (PJI) continue to pose significant challenges for orthopaedic surgeons. These challenges are primarily attributed to the high degree of heterogeneity exhibited by these disorders, which are influenced by a combination of environmental and host factors. Recent research efforts have delved into the pathogenesis of osteomyelitis and PJI by investigating single nucleotide polymorphisms (SNPs). This review comprehensively summarizes the current evidence regarding the associations b

We report an unusual case of an elderly man presenting with formation and rupture of an epidermal inclusion cyst at the level of an implantable cardiac defibrillator (ICD) pocket. The lesion appeared 2 years after ICD implantation, mimicking a decubitus or a pocket infection. Surgical revision showed no signs of infection of the pocket, but the pedunculated lesion was rooted in the subcutaneous tissue, whit an implant base external to the ICD pocket, which was removed and analyzed histologically, confirming the diagnosis of epidermal inclusion cyst. The pathophysiological mechanism of cyst for

BACKGROUND: Periprosthetic joint infection (PJI) is the most serious complication following total joint arthroplasty (TJA) and has a significant impact on patients and the national healthcare system. To date, the diagnosis of PJI is still confronted with dilemmas. The present study investigated the validity of sonication fluid culture (SFC) for removing implants in the diagnosis of PJI after joint replacement.
METHODS: From database establishment to December 2020, relevant literature was retrieved from the PubMed, Web of Science, Embase and Cochrane Library databases. Two reviewers independen

BACKGROUND: The use of double gloving has become a standard practice for joint replacement surgeons. However, since there are limited data on how gloves are contaminated during both primary and revision arthroplasty, no precise protocol exists to direct surgeons on when, or if, to change their gloves. The goals of this preliminary study were to evaluate the contamination of gloves during total joint arthroplasties (TJAs).
METHODS: We included 25 infected cases and 10 primaries, which were performed at the same institution using the same surgical protocol from 3 fellowship trained surgeons. Sa

PURPOSE: Cardiac implantable electronic device (CIED) is used increasingly in recent years. In this study, it is aimed to evaluate the frequency of CIED-associated infections in the cardiology clinic of our hospital in the last 10 years, the causative microorganisms by determining their risk factors and clinical characteristics.
MATERIALS AND METHODS: 225 patients who underwent CIED implantation in our hospital's cardiology clinic between March 2009 and December 2019 were retrospectively identified. The patients were grouped as with and without CIED-associated infection.
RESULTS: In a period

BACKGROUND: Fungal periprosthetic joint infections are rare. Acremonium osteoarticular infections are scarcely reported. Variable susceptibility to antifungal agents have been reported and optimal pharmacotherapy has yet to be established. Here we illustrate an Acremonium osteoarticular infection involving a prosthetic joint and present an antifungal regimen that had led to treatment success.
CASE PRESENTATION: A 60-year-old female with a body mass index (BMI) of 40 had left total knee arthroplasty done in 2012 with a cementless implant for knee osteoarthritis. In 2019, the patient had asympt

Periprosthetic joint infection (PJI) remains a devastating complication after total joint arthroplasty. Bacteria involved in these infections are notorious for adhering to foreign implanted surfaces and generating a biofilm matrix. These biofilms protect the bacteria from antibiotic treatment and the immune system making eradication difficult. Current treatment strategies including debridement, antibiotics, and implant retention, and one- and two-stage revisions still present a relatively high overall failure rate. One of the main shortcomings that has been associated with this high failure ra

BACKGROUND AND HYPOTHESIS: The treatment of periprosthetic joint infection is complicated by the presence of residual biofilm, which resists eradication owing to bacterial adherence to orthopedic implants. The purpose of this study was to compare Bactisure (Zimmer Biomet, Warsaw, IN, USA), povidone-iodine (Betadine), and chlorhexidine gluconate solution (Irrisept; Irrimax, Gainesville, FL, USA) in reducing biofilm formation of Staphylococcus aureus, Staphylococcus epidermidis, and Cutibacterium acnes inoculated on cobalt-chrome, titanium, and stainless steel disks, representing metals commonly

Periprosthetic joint infection (PJI) is the most difficult complication following total joint arthroplasty. Most of the etiological strains, accounting for over 98% of PJI, are bacterial species, with Staphylococcusaureus and Coagulase-negative staphylococci present in between 50% and 60% of all PJIs. Fungi, though rare, can also cause PJI in 1%-2% of cases and can be challenging to manage. The management of this uncommon but complex condition is challenging due to the absence of a consistent algorithm. Diagnosis of fungal PJI is difficult as isolation of the organisms by traditional culture m

Prosthetic joint infection is a feared complication following elective total joint arthroplasty of the hip and knee. Infection following arthroplasty leads to high morbidity and even mortality and is a major burden to the health care system. Although many advances are being made in the treatment of prosthetic joint infection, with the increasing volume of total joint arthroplasty anticipated over the next decade, a significant emphasis should be placed on prevention. Several preoperative, intraoperative, and postoperative strategies may be employed to help reduce the risk. (Journal of Surgical
