Are the orally administered proton pump inhibitors equivalent? A comparison of lansoprazole, omeprazole, pantoprazole, and rabeprazole.

Four proton pump inhibitors (PPIs) are currently marketed in various parts of the world, and all of these (lansoprazole, omeprazole, pantoprazole, and rabeprazole) are available for prescription use in the United States. As a therapeutic group, the PPIs are highly useful for the relief of symptoms and healing of gastroesophageal reflux disease, gastric and duodenal ulcer disease, eradication of Helicobacter pylori infection, prevention and treatment of nonsteroidal anti-inflammatory drug (NSAID)-associated damage, management of hypersecretory states such as Zollinger-Ellison syndrome, and care

Gastro-oesophageal reflux associated with nocturnal gastric acid breakthrough on proton pump inhibitors.

BACKGROUND: Nocturnal gastric acid breakthrough, defined as intragastric pH < 4 for more than 1 h in the overnight period, is observed in up to 70% of normal subjects on proton pump inhibitors taken twice daily. The frequency of this breakthrough in patients with gastro-oesophageal reflux and accompanying oesophageal reflux during this period has not been studied. AIM: To examine the frequency of nocturnal break-through and accompanying oesophageal acid exposure in patients with gastro-oesophageal reflux treated with proton pump inhibitors twice daily. METHODS: Prolonged ambulatory pH record

Should Helicobacter pylori be eradicated before starting long-term proton pump inhibitors?

Symptomatic gastro-oesophageal reflux disease is a common disorder characterized by pathological exposure of the distal oesophagus to acid. The management requires the control of symptoms, prevention of relapse and complications. Proton pump inhibitors are without doubt the most effective agents in the management of gastro-oesophageal reflux disease. In Helicobacter pylori-negative individuals the efficacy of ranitidine, but more pronounced of omeprazole, on the nocturnal intragastric acidity, is less than in Helicobacter pylori-positive patients. Curing the Helicobacter pylori infection in ga

Treatment of peptic ulcer in the elderly. Proton pump inhibitors and histamine H2 receptor antagonists.

Some problems remain unresolved in the short and long term treatment of peptic ulcer in the elderly. These mainly concern the physiology and pathophysiology of the aging stomach, the pharmacokinetic and pharmacodynamic properties of antisecretory drugs, and the presence of different risk factors from those in patients under 60 years of age. The aim of this article is to provide a critical review of the present-day clinical and pharmacological knowledge concerning the use of antisecretory drugs [histamine H2 receptor antagonists (H2RAs) and proton pump inhibitors] in the treatment of peptic ulc

Effects of proton pump inhibitor on gastric mucosa hemodynamics and tissue oxygenation in anesthetized rats.

Proton pump inhibitors have been reported to have a cytoprotective action in addition to the anti-secretory action of acid. The precise mechanism, however, remains obscure. In this study, the effects of proton pump inhibitors (omeprazole and NC-1300) on gastric mucosa hemodynamics and tissue oxygenation were investigated using organ reflectance spectrophotometry in a hemorrhagic shock-reperfusion model involving anesthetized rats. Neither drug affected gastric mucosa hemodynamics nor tissue oxygenation in the basal state before hemorrhage. During the hemorrhagic shock state, however, these dru

[Endoscopic stage classification of peptic ulcer and characterization of the healing process by proton pump inhibitors].

Proton pump inhibitors are highly effective for gastric acid secretion and have been shown to be superior to histamine H2-receptor antagonists. The superiority of proton pump inhibitors over H2-receptor antagonists was more pronounced in duodenal ulcers. Omeprazole reduced the time required by H2-receptor antagonists the healing of duodenal ulcers by 2/3 to 1/2. On the other hand, unusual endoscopic findings, such as shallow white coat or protrusion of the ulcer floor, were noted in the healing stage of gastric ulcers with H2-receptor antagonists. Whereas these findings were rarely seen with c

[Action of triftazin on the physicochemical properties and the membrane proton pump of the synaptic vesicles in the rat brain].

The effects of trifluoperazine (TFP) on some membrane processes were studied in the isolated rat brain synaptic vesicles (SV). TFP (10(-5)-10(-4) M) was found to cause a sharp rise in the intensity of light scattering by SV suspension which was due both to an increased vesicle aggregation and to changes in the refraction index of the membrane. In addition, TFP blocked the ATP-dependent proton transport into the vesicles (K0.5 = 10(-6) M) with the concomitant stimulation of the ATPase activity which suggests an uncoupling effect caused by the permeation of this weak base through the membrane an

Different Routes of Proton Pumps Inhibitors Co-Administration have Significant Impact on Mycophenolate Acid (MPA) Serum Levels in Heart Transplant Recipients.

BACKGROUND Antiproliferative drugs including mycophenolate mofetil (MMF) are widely accepted part of an immunosuppressive therapy following heart transplantation. Proton pump inhibitors (PPIs) are routinely administered after cardiac surgery procedures including transplantation. They may also have impact on mycophenolate acid (MPA) serum levels. MATERIAL AND METHODS There were 30 consecutive patients (28 male and 2 female patients) with a mean age of 45±12 years who were enrolled into this study. MPA serum levels were studied; PPIs were intravenously and orally administered. RESULTS The mean M

Effects of Barium Excess on the Performance of Multilayer Microtubular Proton Ceramic Electrochemical Hydrogen Pumps with BaxCe0.7Zr0.1Y0.1Yb0.1O3-δ (x = 1.05, 1.10, 1.15).

A series of Ba-excess BaxCe0.7Zr0.1Y0.1Yb0.1O3-δ (BxCZYYb, x = 1.05, 1.10, 1.15) was employed as electrolytes. Multilayer microtubular protonic ceramic electrochemical hydrogen pumps (PCEHPs) with a current collector/anode/electrolyte/cathode/current collector architecture were fabricated by a triple-layer one-step co-spinning and co-sintering method. Their hydrogen separation performance was systematically investigated over the temperature range of 200-400 °C. The results show that the hydrogen pump employing the B1.05CZYYb electrolyte delivers the optimal performance. At 250 °C and a feed H2

Beyond the Induction Phase: Should We Keep the Proton Pump Inhibitor in Pediatric Eosinophilic Esophagitis?

BACKGROUND: Proton pump inhibitors (PPIs) are a common and effective management strategy for eosinophilic esophagitis (EoE) leading to successful remission in most cases. However, there is uncertainty regarding the ideal follow-up pharmacological treatment to maintain remission. We performed a cost-effectiveness and quality-adjusted life year (QALY) analysis. OBJECTIVES: To compare various approaches of pharmacologic treatments for EoE. METHODS: A Markov model was constructed to evaluate the cost-effectiveness and QALY of maintenance therapies for eosinophilic esophagitis, including PPI, top

Self-medication with proton pump inhibitors among physicians in Bosnia and Herzegovina: a cross-sectional study.

BACKGROUND: Proton pump inhibitors (PPIs) are widely used for the treatment of acid-related disorders, but inappropriate or prolonged use carries potential health risks. Physicians, due to their access to medication and clinical knowledge, may be prone to self-medicating with PPIs without appropriate oversight. OBJECTIVE: To assess the prevalence and patterns of personal PPI use and self-medication among practicing physicians in Bosnia and Herzegovina, and to identify demographic and professional predictors of such behavior. METHODS: A cross-sectional, questionnaire-based survey was conducte

Impact of an IV proton pump inhibitor restriction policy on prescribing patterns and estimated drug cost.

INTRODUCTION: Intravenous (IV) proton pump inhibitors (PPIs) are commonly used in hospitalized patients, although a substantial proportion of this use may be inappropriate, particularly when enteral therapy is feasible. Restriction policies may help optimize prescribing and reduce unnecessary drug costs. This study evaluated the impact of an IV PPI restriction policy on prescribing patterns and estimated direct drug cost at a regional secondary care hospital in Saudi Arabia. METHODS: This retrospective before-and-after study was conducted from April 2023 to April 2024. The pre-restriction per

Vonoprazan vs Proton Pump Inhibitors for Gastroprotection in Anticoagulated Patients: A Real-World Cohort Study.

Introduction Oral anticoagulation is associated with an increased risk of GI bleeding. Proton pump inhibitors (PPIs) are commonly used for gastroprotection but have pharmacologic limitations. This study examined whether vonoprazan is associated with different one-year bleeding and clinical outcomes compared with PPIs among anticoagulated adults. Methods We conducted a retrospective cohort study using the TriNetX database to identify adults receiving oral anticoagulants who initiated vonoprazan or a PPI between 2015 and 2025. Propensity score matching (1:1) was performed to balance demographics

Formulation and Characterization of Multiparticulate System for Proton Pump Inhibitor using Combination Therapy for Peptic Ulcer.

INTRODUCTION: The purpose of this study was to develop sustained-release enteric-coated granules of Amla extract and esomeprazole magnesium for the treatment of peptic ulcer. It is well known that Amla possesses anti-ulcer activity due to the phenolic compound gallic acid, which inhibits a gastric H+/K+ ATPase pump. Therefore, enteric-coated Amla extract granules were combined with esomeprazole magnesium granules to enhance the synergistic effect, mitigate adverse effects associated with esomeprazole magnesium, and improve overall anti-ulcer activity, while the enteric-coated multiparticulate
