Biological agents in the treatment of psoriasis.

Psoriasis is a chronic immune-mediated inflammatory disease, with an estimated prevalence of 1-3% worldwide. It is considered to be a multisystemic disorder, primarily affecting the skin and joints (psoriatic arthritis), and associated with other inflammatory conditions such as inflammatory bowel disease and coronary heart disease among others. Today, thanks to recent scientific advances that have allowed us to deepen our understanding of the pathogenesis of psoriasis, we count with an expanded therapeutic armamentarium that includes targeted therapy in the form of ''biologics''. These agents …

Pathogenesis and clinical features of psoriasis.

Psoriasis, a papulosquamous skin disease, was originally thought of as a disorder primarily of epidermal keratinocytes, but is now recognised as one of the commonest immune-mediated disorders. Tumour necrosis factor alpha, dendritic cells, and T-cells all contribute substantially to its pathogenesis. In early-onset psoriasis (beginning before age 40 years), carriage of HLA-Cw6 and environmental triggers, such as beta-haemolytic streptococcal infections, are major determinants of disease expression. Moreover, at least nine chromosomal psoriasis susceptibility loci have been identified. Several …

Genetics of psoriasis and psoriatic arthritis.

Psoriasis and psoriatic arthritis are linked diseases characterised by (distinct ?) immune-mediated pathogenetic mechanisms and by a genetic background interacting with environmental factors. Some candidate susceptibility genes have been studied extensively; they include HLA genes, genes within the HLA region and genes outside the HLA region; among them corneodesmosin and other genes of PSORS1 region, MICA and TNF-a polymorphisms. The main findings in the literature are discussed.

Lymphocytapheresis in the treatment of psoriasis vulgaris.

Psoriasis is a common autoimmune chronic inflammatory skin disease that affects approximately 2% of the world's population; fundamental for its immunopathogenic mechanism is secretion of type 1 (Th1) cytokines by T cells and their activation. Since cytapheresis has been widely applied to autoimmune disorders, emphasizing the recently reported results of granulocyte and monocyte adsorption apheresis in psoriasis, a small series of psoriasis vulgaris (PV) patients underwent lymphocytapheresis (LCA) with the aim to remove lymphocytes. Five patients were submitted to weekly LCA. The severity of th…

An Italian study on psoriasis and depression.

BACKGROUND: Psoriasis is often associated with the risk of physical disability, social discomfort and psychological disorders. OBJECTIVES: The aim of this study was to investigate the prevalence of depressive symptomatology among Italian patients with psoriasis vulgaris, in order to better evaluate the disease severity in this patient population. METHODS: Five thousand Italian patients with psoriasis were mailed the Center for Epidemiological Studies-Depression Scale (CES-D) questionnaire, a 20-item instrument developed to perform epidemiological studies of depressive symptomatology in the g…

Therapy of moderate and severe psoriasis.

OBJECTIVE AND METHODS: This health technology assessment (HTA) report synthesises systematically randomized controlled studies (RCT) on the therapy of moderate and severe psoriasis vulgaris which were published between 1999 and 2004; it includes some important clinical studies which have been published after 2004 and thus updates the English HTA report by Griffiths et al. [1]. The major objective is the evaluation of the medical effectiveness of different therapeutical approaches and the cost effectiveness with relevance for Germany. RESULTS: The major conclusions from the results of medical …

Mapping genes for asthma and psoriasis.

A property of susceptibility genes for complex diseases is their reduced penetrance, due to the influences of other genes, the environment, … ma and atopy, and the study of HCR on chromosome 6p21 at PSORS1, the major susceptibility locus for psoriasis. To implicate GPRA in asthma and atopy, we studied its isoform-specific expression in bronchial biopsies and other sites for allergic reactions. We also studied its expression in a mouse model of ovalbumin-induced hypersensitivity. To study the role …

Dobesilate in the treatment of plaque psoriasis.

Fibroblast growth factor (FGF)-mediated pathways participate in many of the cellular events implicated in the pathogenesis of psoriasis. Thus, targeting FGF signals may be potentially therapeutic in the treatment of psoriasis. We report for the first time on a 43-year-old man with chronic-type plaque psoriasis with a daily topical treatment of dobesilate, a new FGF inhibitor. As early as at day 14, the patient had cleared or achieved excellent improvement of psoriatic skin lesions. Topical dobesilate offers the potential for treatment of plaque psoriasis without atrophy or other local side eff…

Psoriasis of the dorsal surface of the tongue.

Psoriasis is primarily an inherited inflammatory skin disease, it is characterized by erythemato-squamous lesions that usually involve elbows, knees and the scalp. Oral manifestations are rare in psoriasis, infact, oral psoriasis involves 2% of psoriatic patients and usually it is observed with the onset of cutaneous lesions and progresses with them. Differential diagnosis should be done for Reiter's syndrome, leukoplakia and geographic tongue. The authors describe a case of tongue psoriasis without cutaneous lesions.

[Changes in lipid peroxidation in psoriasis].

Changes of the lipid peroxidation processes (LP) in the skin (corneal layer of the epidermis) and in blood, as observed in 61 patients with local psoriasis at the progressing stage and in 55 patients with local psoriasis at remission, were investigated. The quantity of diene conjugates, ketodienes, and of conjugate trienes in the heptanoic and isopropanol phases was decreasing both in the affected and intact skins versus the controls. The test based on thiobarbituric acid (TBA) was used to establish that the content of the TBA-positive products was also decreasing. The examined values did not …

[Psoriasis vulgaris. An indication for lasers?].

Treatment of psoriasis is often complicated and remains a challenge. Along with the many new immunomodulatory approaches, various laser systems have been employed in an attempt to treat chronic plaque psoriasis. These include CO(2), Er:YAG, pulsed dye, 1302 nm Nd:YAG and excimer lasers. After reviewing and critically evaluating the literature, we conclude that the 308 nm excimer laser has proven its efficacy, not only in comparison with other laser systems, but also when compared with conventional 311 nm narrow-band UVB treatment. It is an efficient and promising therapeutic option for treatin…

Infliximab for psoriasis and psoriatic arthritis.

Tumor Necrosis Factor alpha (TNF) as proinflammatory cytokine plays in the pathogenesis of many diseases an important role. In psoriasis and in psoriatic arthritis TNF is up-regulated in the skin lesion and in the synovitis. Recent trials showed that the blockade of TNF with the chimeric antibody infliximab is able to improve both, the skin lesions and the synovitis of the joints. In psoriasis in 82% of patients treated with infliximab achieved an over 75% response in the PASI index. In Psoriatic arthritis the skin improvement was correlating with the reduction of synovitis and in a small MRI …

Psoriasis of the scalp. Diagnosis and management.

Psoriasis of the scalp is a frequently occurring condition affecting approximately 2% of the Western population. The sharply demarcated erythematosquamous lesions with silver-white scaling characterize scalp psoriasis. Quality of life can be seriously reduced by this condition and therefore long term treatment is needed in most patients. Coal tar shampoos, containing 2 to 10% coal tar solution, are effective in scalp psoriasis. However, no double-blind studies are available to support such an assumption. Salicylic acid 5 to 10% has a pronounced keratolytic effect. Salicylic acid should be form…

Atopy and contact sensitization in psoriasis.

The allergen-specific IgE antibody was determined in 20 men and 120 women with psoriasis and the results were correlated with a history of current and previous allergic disease. Allergic disease was reported in 21% of the patients, but a positive RAST test was obtained in 44%. In chronic plaque-type psoriasis a positive RAST test was significantly more common (58%) than in active psoriasis (22%). Grass pollen and house dust mite were the most prevalent sensitizing allergens, with frequencies of 64% and 53%, respectively in the sensitized subjects. Sensitization increased with age and polysensi…

Diagnosis: psoriasis or not? What are the clues?

Psoriasis is a common inflammatory, hyperproliferative skin disorder that affects 1% to 2% of the population of Western Europe and the United States. Because the clinical presentation of psoriasis is varied, many times the definitive diagnosis depends on the histological examination. However, the histological changes of psoriasis are as varied as the clinical presentations. Therefore, a combination of histopathologic features must be present for the diagnosis of psoriasis to be made. In this article, we review the clues for the histopathologic diagnosis of each type and stage of psoriasis, suc…

Coexistence of superficial pemphigus and psoriasis.

We describe a case of the coexistence of psoriasis and pemphigus erythematosus. The coexistence of these two diseases may be more than coincidental: the effects of one immune-mediated skin disease may influence the occurrence of another. Possible mechanisms include antigen modification, altered regulation of T-cell activity, and plasminogen activator abnormalities.
