Reconstruction of upper lip avulsion after dog bite: case report and review of literature.

IMPORTANCE: Upper lip avulsion after traumatic dog bite is a serious cause of facial disfigurement for which there is no consensus on management in the acute setting. OBJECTIVE: This review was prompted by a case at our institution and is intended to display the available evidence in the management of the patient after dog bite injury to the upper lip. Our main goals are to create a management algorithm using current evidence and to stimulate further clinical investigation to improve outcomes in patients with facial dog bite injuries. EVIDENCE REVIEW: A review of English literature was perfo

Cardiac Dysautonomia in Huntington's Disease.

Huntington's disease is a fatal, hereditary, neurodegenerative disorder best known for its clinical triad of progressive motor impairment, cognitive deficits and psychiatric disturbances. Although a disease of the central nervous system, mortality surveys indicate that heart disease is a leading cause of death. The nature of such cardiac abnormalities remains unknown. Clinical findings indicate a high prevalence of autonomic nervous system dysfunction - dysautonomia - which may be a result of pathology of the central autonomic network. Dysautonomia can have profound effects on cardiac health,

Neuropsychological profile of psychogenic jerky movement disorders: importance of evaluating non-credible cognitive performance and psychopathology.

BACKGROUND: Psychogenic movement disorders are disorders of movements that cannot be explained by a known neurological disorder and are assumed to be associated with psychiatric symptoms such as depression and anxiety. OBJECTIVE: To examine the neuropsychological profile of patients with psychogenic movement disorders. METHODS: We examined cognitive functioning using neuropsychological tests in 26 patients with clinically established psychogenic jerky movement disorders (PMD). We included 16 patients with Gilles de la Tourette syndrome (GTS) who served as a patient control group, in addition

Risks and predictors of current suicidality in HIV-infected heroin users in treatment in Yunnan, China: a controlled study.

OBJECTIVE: Suicide is an important public health problem in China. Elsewhere, injection drug use and HIV infection have independently been associated with suicidality, but research has often overlooked these high-risk groups in China. We determined the frequency and predictors of suicidal ideas in Chinese HIV-infected (HIV(+)) and HIV-uninfected (HIV(-)) heroin injection drug users (IDUs) in treatment and a control sample. We hypothesized that rates of suicidal ideas would be significantly higher among IDUs compared with controls and highest among HIV IDUs. METHOD: We assessed suicidal ideas

Quantifying over-activity in bipolar and schizophrenia patients in a human open field paradigm.

It has been suggested that a cardinal symptom of mania is over-activity and exaggerated goal-directed behavior. Nevertheless, few attempts have been made to quantify this behavior objectively in a laboratory environment. Having a methodology to assess over-activity reliably might be useful in distinguishing manic bipolar disorder (BD) from schizophrenia (SCZ) during highly activated states. In the current study, quantifiable measures of object interaction were assessed using a multivariate approach. Additionally, symptom correlates of over-activity were assessed. Patients admitted to an acute

A multiattribute decision model for bipolar disorder: identification of preferred mood-stabilizing medications.

OBJECTIVE: To develop a multiattribute decision model (MADM) to aid in the selection of preferred medications for the treatment of bipolar disorder. STUDY DESIGN: Data were obtained via a self-administered online survey among psychiatric pharmacist specialists. These survey data were used to construct a MADM based on multiattribute utility technology. METHODS: Anticonvulsant mood stabilizers, atypical antipsychotics, olanzapine-fluoxetine combination, and lithium carbonate were evaluated using a MADM. Attributes included in the model were effectiveness, safety and tolerability, cost, monitor

The impact of mental health and traumatic life experiences on antiretroviral treatment outcomes for people living with HIV/AIDS.

Potent antiretroviral therapy (ART) has transformed HIV from a death sentence to a chronic illness. Accordingly, the goal of HIV care has shifted from delaying death to achieving optimal health outcomes through ART treatment. ART treatment success hinges on medication adherence. Extensive research has demonstrated that the primary barriers to ART adherence include mental illness, especially depression and substance abuse, as well as histories of traumatic experiences such as childhood sexual and physical abuse. These psychosocial factors are highly prevalent in people living with HIV/AIDS (PLW

Historical note on Darwin's consideration of early-onset dementia in older persons, thirty-six years before Alzheimer's initial case report.

In February 1871, the great naturalist Charles Darwin received a letter from Dr. James-Crichton Browne, who was serving as Director of the largest lunatic asylum in England. Darwin had been introduced to Crichton-Browne 2 years earlier by Henry Maudsley, who believed that the young psychiatrist could provide Darwin with clinical examples of extreme emotional expression, to aid him in preparing to write Expression of the Emotions in Man and Animals (1872). This particular letter from Crichton-Browne contained the first and only reference to "premature dotage" or "senile decay" found anywhere in

The development of guidelines for the treatment of patients with mental disorders under particular consideration of rehabilitative aspects.

Inpatient psychotherapeutic treatment is quite extensive in Germany. Three treatment systems (psychosomatic/psychotherapeutic healthcare, psychiatric/psychotherapeutic healthcare and rehabilitation of patients with mental disorders) exist relatively independently from one another. They show large areas of overlap, however, with regard to various criteria. This is due to the fact that, as opposed to many somatic illnesses, a clear distinction between acute-medical and rehabilitative elements cannot be made in the treatment of mental disorders.Systematic treatment recommendations in the form of

Criteria for solvent-induced chronic toxic encephalopathy: a systematic review.

In 1985, a WHO Working Group presented diagnostic criteria and a classification for solvent-induced chronic toxic encephalopathy (CTE). In the same year, the "Workshop on neurobehavioral effects of solvents" in Raleigh, N.C., USA introduced a somewhat different classification for CTE. The objective of this review is to study the diagnostic procedures that are used to establish the diagnosis of CTE, and the extent to which the diagnostic criteria and classification of the WHO, and the classification of the Raleigh Working Group, are applied. A systematic search of studies on CTE was performed,

Neuropsychiatric disturbance is associated with executive dysfunction in HIV-1 infection.

Prominent apathy and/or irritability are frequently observed among individuals infected with the human immunodeficiency virus (HIV). Although these symptoms often occur as part of a mood disorder, compelling evidence suggests that they may occur independently of depression in neurologic disease/disorder. The current study examined the prevalence of both apathy and irritability among a sample of HIV-infected individuals and explored the degree to which these neuropsychiatric (NP) phenomena were associated with performance on neuro-cognitive measures thought to be sensitive to the potential CNS

Bipolarity in children.

Childhood and adolescent bipolar disorder have been less studied than adult onset bipolar illness. However, case reports of mania in childhood can be found as early as the mid 19th century. Historically, several factors have made the accurate diagnosis of bipolar disorder in childhood difficult: clinical bias against the diagnosis of mania in children; low base rate of disorder; symptom overlap between bipolar disorder and other more prevalent childhood-onset psychiatric disorders; and developmental constraints and variability in clinical presentation. The epidemiology of juvenile-onset bipola

Chromosomal localization of 15 ion channel genes.

Several human Mendelian diseases, including the long-QT syndrome, malignant hyperthermia, and episodic ataxia/myokymia syndrome, have recently been demonstrated to be due to mutations in ion channel genes. Systematic mapping of ion channel genes may therefore reveal candidates for other heritable disorders. In this study, the GenBank and dbEST databases were used to identify members of several ion channel families (voltage-gated calcium and sodium, cardiac chloride, and all classes of potassium channels). Genes and ESTs without prior map localization were identified based on GDB and OWL databa

The glutamatergic basis of human alcoholism.

OBJECTIVE: Although alcoholism is one of the most common psychiatric diagnoses, understanding of its pathophysiology remains poor. Accumulating evidence suggests that neurophysiological and pathological effects of ethanol are mediated to a considerable extent through the glutamatergic system. This article reviews the evidence of ethanol's effects on glutamatergic transmission and proposes a glutamatergic basis for alcoholism. METHOD: The information was derived from original research. The authors located more than 100 articles from psychiatry and neuroscience journals that related ethanol to

Suicide as an outcome for medical disorders.

The association between suicide and medical disorder has not received as much attention as the association between suicide and psychiatric disorder. We identified by statistical overview medical disorders with an altered suicide risk. We found reports on the mortality of 63 medical disorders (ICD9 001-289, 320-999) said to have an altered suicide risk. English-language reports were located on MEDLINE with the search terms "disease name with mortality and follow-up"; and from the reference lists of these reports. We abstracted 235 reports of mortality studies of medical disorders with 2 years o

Comparison of Resting-State EEG and Synchronization Between Young Adults with Down Syndrome and Controls in Bipolar Montage.

The qEEG findings of subjects with Down syndrome (DS) have not been described in the context of bipolar montage. Resting-state EEG (rsEEG) with a bipolar montage was performed in 22 young adults (26.0 ± 1.2 years) with DS but without psychiatric or neurological pathology and matched control subjects of the same sex and age, and the results were conventionally and numerically analyzed. Channels were grouped into frontal, parieto-occipital, and temporal lobes. For every channel, the power spectrum was calculated and used to compute the area for the delta, theta, alpha and beta bands and was log-

Acoustic features and social cognition in schizophrenia and schizotypal personality disorder.

INTRODUCTION: Although prior studies have observed speech alterations in schizophrenia, minimal research has focused on this in schizotypal personality disorder (SPD), despite impairments observed in social communication and function. SPD is a disorder within the schizophrenia spectrum, but individuals are typically antipsychotic naïve and lack a history of psychiatric hospitalizations. The current study examined acoustic features extracted from an open-ended interview in individuals with schizophrenia spectrum disorders and healthy controls. METHODS: Twenty-seven individuals with SPD, 36 ind

Design and Antischizophrenic Studies of 1,3-disubstituted Chalcone Derivatives: In-silico Molecular Docking Approach.

Schizophrenia is a psychiatric disorder that affects a person's ability to think, feel and behave clearly. The pathophysiology of the disease stems from the overexpression of dopamine neurotransmission and deficiency of glutamate activity at glutamate synapse in the brain. Considering the significant global burden of the disease, lack of complete efficacy using the current medications and variety of adverse effects associated with their use, and the huge opportunity created by Computer-Aided Drug Design, it is therefore important and possible to come up with drug leads that will have improved

Perspectives of general psychiatric inpatient care for persons with anorexia nervosa: an integrative literature review.

OBJECTIVES: Persons diagnosed with anorexia nervosa (AN) may receive care in general psychiatric inpatient care (GPIC) for several reasons including severity of their condition, comorbidities and lack of access to specialised inpatient care. However, scant research has explored how this specific setting may impact persons with AN, either positively or negatively. Additionally, there is limited evidence regarding the most effective form of care for AN within GPIC. This integrative literature review provides a comprehensive overview of research focusing on care for AN in GPIC settings, shedding
