Pain, numbness, or both? Distinguishing the longitudinal course and predictors of positive, painful neuropathic features vs numbness after breast cancer surgery.

INTRODUCTION: Both positive (burning, stabbing, and allodynia) and negative (numbness) neuropathic symptoms may arise after surgery but likely contribute differently to patients' postoperative pain experience. Numbness has been identified as divergent from positive neuropathic symptoms and therefore excluded from some neuropathic assessment tools (Neuropathic Pain Scale for PostSurgical patients [NeuPPS]). OBJECTIVES: In this prospective longitudinal study of patients undergoing breast surgery, we aimed to delineate the time course of numbness and its coincidence with NeuPPS and to contrast t

Deficits in ascending and descending pain modulation pathways in patients with postherpetic neuralgia.

Postherpetic Neuralgia (PHN), develops after the resolution of the herpes zoster mucocutaneous eruption, is a debilitating chronic pain. However, there is a lack of knowledge regarding the underlying mechanisms associated with ascending and descending pain modulations in PHN patients. Here, we combined psychophysics with structural and functional magnetic resonance imaging (MRI) techniques to investigate the brain alternations in PHN patients. Psychophysical tests showed that compared with healthy controls, PHN patients had increased state and trait anxiety and depression. Structural MRI data …

Dissociated functional significance of decision-related activity in the primate dorsal stream.

During decision making, neurons in multiple brain regions exhibit responses that are correlated with decisions. However, it remains uncertain whether or not various forms of decision-related activity are causally related to decision making. Here we address this question by recording and reversibly inactivating the lateral intraparietal (LIP) and middle temporal (MT) areas of rhesus macaques performing a motion direction discrimination task. Neurons in area LIP exhibited firing rate patterns that directly resembled the evidence accumulation process posited to govern decision making, with strong

Gender differences in the neural response to acupuncture: clinical implications.

OBJECTIVE: To examine gender differences and similarities in the psychophysical and brain responses to acupuncture at GB34, a point that is frequently used to treat motor function issues in Traditional Chinese Medicine. METHODS: Functional MRI (fMRI) was used to measure brain activation in response to acupuncture at GB34 (on the right) in 19 healthy participants (9 male, 10 female). De qi sensations were rated to measure their psychophysical responses. RESULTS: Overall de qi scores did not differ by gender, although females reported greater intensity of aching (p=0.04). Acupuncture activated

Cone responses in Usher syndrome types 1 and 2 by microvolt electroretinography.

PURPOSE: Progressive decline of psychophysical cone-mediated measures has been reported in type 1 (USH1) and type 2 (USH2) Usher syndrome. Conventional cone electroretinogram (ERG) responses in USH demonstrate poor signal-to-noise ratio. We evaluated cone signals in USH1 and USH2 by recording microvolt level cycle-by-cycle (CxC) ERG. METHODS: Responses of molecularly genotyped USH1 (n = 18) and USH2 (n = 24) subjects (age range, 15-69 years) were compared with those of controls (n = 12). A subset of USH1 (n = 9) and USH2 (n = 9) subjects was examined two to four times over 2 to 8 years. Photo

Pain perception in idiopathic cervical dystonia (spasmodic torticollis).

Cervical spinal pain is frequently found in conjunction with idiopathic cervical dystonia (ICD), a focal dystonia characterized by sustained deviation of the head. Since the perception of noxious stimuli has never been studied in ICD, we performed a controlled study to obtain more insight into the psychophysics of dystonia-related muscle pain by evaluating pressure-induced pain levels. In nine ICD patients and five gender- and age-matched asymptomatic control subjects, pain-pressure thresholds (PPTs) were determined in the sternocleidomastoid and upper trapezius muscles, both at resting activi…

Animal Models of Tinnitus: A Review.

Animal models have significantly contributed to understanding the pathophysiology of chronic subjective tinnitus. They are useful because they control etiology, which in humans is heterogeneous; employ random group assignment; and often use methods not permissible in human studies. Animal models can be broadly categorized as either operant or reflexive, based on methodology. Operant methods use variants of established psychophysical procedures to reveal what an animal hears. Reflexive methods do the same using elicited behavior, for example, the acoustic startle reflex. All methods contrast th

How to measure olfactory bulb volume and olfactory sulcus depth?

In combination with psychophysical testing and electrophysiological studies, magnetic resonance imaging plays a role in the clinical evaluation of patients with an olfactory dysfunction. Quantitative measurements of olfactory bulb volume and of olfactory sulcus depth, and the morphological depiction of structural abnormalities, make synergistic contributions to the accurate radiological diagnosis of smell dysfunction. Moreover, the plasticity of our olfactory system can be demonstrated by temporal changes in OB volumetric measurements. In this paper, we provide an outline of how to measure olf

Temporal and speech processing deficits in auditory neuropathy.

Auditory neuropathy affects the normal synchronous activity in the auditory nerve, without affecting the amplification function in the inner ear. Patients with auditory neuropathy often complain that they can hear sounds, but cannot understand speech. Here we report psychophysical tests indicating that these patients' poor speech recognition is due to a severe impairment in their temporal processing abilities. We also simulate this temporal processing impairment in normally hearing listeners and produce similar speech recognition deficits. This study demonstrates the importance of neural synch

Alpha-2-adrenoceptor sensitivity in heroin addicts with and without previous attention deficit disorder/hyperactivity and conduct disorder.

Growth hormone (GH) and beta-endorphin (beta-EP) responses to clonidine stimulation were examined in 18 male heroin addicts, 9 with and 9 without previous histories of attention deficit disorder with hyperactivity (ADD-H) and conduct disorder (CD). Ten psychophysically healthy volunteers were used as controls. ADD-H/CD addicts had blunted GH and beta-EP responses as compared to controls while those of non-ADD-H/CD addicts were normal. This suggests that postsynaptic adrenoceptor sensitivity is decreased and, possibly, that presynaptic noradrenaline secretion is increased in ADD-H/CD patients w

Maximum acceptable frequencies for females performing a drilling task in different wrist postures.

This paper presents a study on the effect of wrist posture; flexion, extension, ulnar deviation, and radial deviation on maximum acceptable frequencies (MAF) for a drilling task using a psychophysical approach. Twelve females were selected from a college population to serve as subjects and an adjustable workstation was used to simulate a drilling task. The results revealed that flexion, extension, and radial deviation all had a significant effect on MAF but, ulnar deviation did not. The postures have been ranked in order of possible risk of contributing to CTD injury, with neutral having the l

The application of visually evoked cerebral potentials in ophthalmological diagnosis.

A short survey is presented of what can be done with electrophysiological techniques in ophthalmological diagnosis. It is stressed that more investigation has to be done to elucidate the discrepancies between electrophysiological and psychophysical disturbances. These discrepancies can be used for the differential diagnosis. More, though still too little, is known about differences between a disturbance of latency time and that of amplitude. It seems that latencies are more disturbed in active pathological processes like demyelination, compression of nerve fibres and oedema, whereas isolated r

Vision threshold profiles in X-linked retinitis pigmentosa.

Absolute thresholds for blue-green and red stimuli were measured along the horizontal and vertical meridans in two patients with X-linked retinitis pigmentosa. From these data, we deduced that cones mediate detection of both stimuli in the central 10 degrees, there is a ring scotoma in the mid-periphery, and in the far periphery rods mediate detection of both stimuli. Conclusions drawn from the psychophysical measures about rod and cone function across the retina are in point-by-point agreement with the data of a recent electron microscopic study by Szamier et al. on the retina of a 24-year-ol

Occult retinitis pigmentosa simulating central nervous system disease.

The case histories of 3 elderly patients with senile pigmentary dystrophy of the retina are presented. Recognition of this occult form of retinitis pigmentosa was obscured by the late onset of symptoms, lack of subjective night blindness, subtle fundus abnormalities, and absence of family history of eye disease. Unexplained visual field loss prompted invasive neurologic studies for suspected brain tumor in 2 patients and long-term anticoagulation administration in the third, who was believed to have primary cerebrovascular disease. Retinitis pigmentosa should be considered when evaluating olde

A Turing Test for artificial nets devoted to vision.

In this work we argue that, despite recent claims about successful modeling of the visual brain using deep nets, the problem is far from being solved, particularly for low-level vision. Open issues include where should we read from in ANNs to check behavior? What should be the read-out? Is this ad-hoc read-out considered part of the brain model or not? In order to understand vision-ANNs, should we use artificial psychophysics or artificial physiology? Anyhow, should artificial tests literally match the experiments done with humans? These questions suggest a clear need for biologically sensible…

Food Supplements and Well-Being: A Pilot Investigation in the General Practitioner Office of the Veneto Region.

BACKGROUND: The use of food supplements (FS) is rapidly increasing, particularly in Italy, which leads the European market. This trend is driven by various factors, including the pursuit of physical well-being, the influence of advertising, and concerns about disease prevention. This exploratory pilot descriptive study aimed to characterize FS use among patients attending general practitioner (GP) offices and examine potential patterns with psychophysical well-being. METHODS: Two questionnaires were administered to participants: one on FS use and another on physical and mental health (SF-12 q

Limited observed efficacy of olfactory training in severe long-term post-COVID-19 syndrome: a prospective cohort study.

PURPOSE: Persistent chemosensory dysfunction is a debilitating symptom of Post-COVID-19 Syndrome (PCS). While olfactory training (OT) is effective in early post-viral smell loss, its efficacy in chronic, multisystemic PCS remains unclear. This study aimed to characterize a severely affected cohort and to evaluate changes in olfactory function during a structured OT intervention. METHODS: In this prospective study, 29 patients with persistent PCS lasting > 12 weeks (mean duration approximately 30 months) were recruited from a specialized post-COVID outpatient clinic. All patients underwent str

Changes in the Epidemiology of Pediatric Olfactory Dysfunction Before and After COVID-19 Pandemic.

BACKGROUND: The COVID-19 pandemic may have altered the epidemiology and clinical features of pediatric olfactory dysfunction, but data focusing on children remain limited, particularly regarding etiological changes and qualitative symptoms such as parosmia. This study investigated temporal changes in pediatric presentations at a tertiary smell clinic. METHODOLOGY/PRINCIPAL: We retrospectively reviewed patients younger than 18 years who visited our clinic for olfactory dysfunction between January 2009 and December 2024. Patients were divided into pre-pandemic (January 2009-March 2020) and post
