Acute myocardial infarction in a patient with anomalous left coronary artery origin and primary antiphospholipid syndrome.

Anomalous left main coronary artery (LMCA) originating from the right coronary sinus and running between the aorta and pulmonary trunk is a rare congenital condition. Although this disease is known to be associated with myocardial infarction and sudden death, the precise mechanism is uncertain. A 14-year-old male with this anomaly developed myocardial infarction during exercise complicated by primary antiphospholipid syndrome. He was admitted to hospital with persistent chest pain and sudden cardiac collapse that occurred while he was running. Cardiac catheterization demonstrated a narrowed se

Use of nitrates in patients with acute myocardial infarction.

The use of nitrates in treating acute myocardial infarction is reviewed; proposed mechanisms of action and pertinent pathophysiology are discussed. Oral and sublingual nitrates were first tested in acute myocardial infarction patients with mixed results. Later studies with sublingual nitroglycerin followed by phenylephrine infusion indicated that nitrates were effective in limiting myocardial ischemia and necrosis. I.V. nitroglycerin was then studied; beneficial results were documented by quantifying ECG changes and visualizing the areas of myocardial necrosis with radioisotopes. Mortality was

Heterogeneous Pulmonary Perfusion on Dual-Energy Computed Tomography: Faulty Postprocessing or New Artifact?

Iodine uptake in the lungs and intrathoracic lesions on postcontrast dual-energy computed tomography is used for evaluation of pulmonary embolism-related perfusion defects and pulmonary infarctions. It has been applied in characterization and treatment response assessment of lung and mediastinal abnormalities. We report a new imaging artifact or faulty image postprocessing in a commercially available rapid kV switching technique of dual-energy computed tomography, which can confound its clinical utility for evaluation of iodine uptake.

Cutaneous phycomycosis (mucormycosis) with fatal pulmonary dissemination.

Phycomycosis of the skin manifested as a spontaneous solitary ulcer of the leg in a patient with uremia. The infection later spread to the lungs and produced fatal pulmonary infarction. Histopathologic study of the lesions revealed broad nonseptate fungal hyphae that had invaded vascular channels, with areas of necrosis and acute inflammation. Primary cutaneous phycomycosis is rare. In the studied cases, dissemination of infection did not occur. The unusual feature of fatal pulmonary dissemination of primary cutaneous phycomycosis as observed in our patient has not been described in the letera…

Pulmonary dirofilariasis.

BACKGROUND: Pulmonary dirofilariasis is a rare entity caused by Dirofilaria immitis, the dog heartworm that is transmitted to humans by mosquitos. This filarial nematode enters the subcutaneous tissue, travels to the right ventricle where it dies, and then embolizes the pulmonary vasculature, causing a small pulmonary infarction, which subsequently appears as a solitary nodule. Although these nodules are usually identified incidentally by chest radiography in asymptomatic patients, the lesion is generally presumed to be neoplastic. Diagnosis is made by surgical excision. METHODS: We reviewed …

Spinal cord infarction as a rare complication of fat embolism syndrome following bilateral intramedullary nailing of femur fractures.

Fat embolism syndrome (FES) is a rare and potentially fatal complication occurring most often after long bone or pelvic fractures and orthopaedic procedures. It can consist of pulmonary, central nervous system and cutaneous manifestations. The exact pathophysiology of emboli reaching the arterial circulation is poorly understood.1 It is suggested that this may occur by either 'paradoxical' embolism or microembolism.2 3 Its true incidence is unknown but increases in the presence of multiple closed fractures. It can be a diagnostic dilemma for clinicians and if suspected diffusion-weighted MRI i

Bilateral papillary muscle infarction in a chagasic patient.

We report a case of a 59-year-old patient diagnosed with chagasic cardiomyopathy, who manifested sudden heart failure while hospitalized, evolving to death due to cardiogenic and septic shock. Anatomical-pathological studies revealed infarction of the papillary muscles together with histological changes compatible with 48 to 72 hours of evolution. Pulmonary edema was considered the cause of death, probably related to mitral regurgitation of ischemic nature. The cause of the papillary muscle infarction was not elucidated by study of the coronary tree, which presented no signs of recent thrombos

[Cerebral infarct following trauma].

Vascular occlusions due to cerebral trauma have always been regarded as great rarities. However, we have found hypo-dense foci of vascular distribution in 3.5% of 3500 CT examinations for trauma during the late phase. Lesions in the vascular territory of the posterior cerebral artery are usually the result of supratentorial pressure rise from epidural and subdural haematomas, leading to compression of the vessels against the edge of the tentorium. Typical infarcts in the territory of the medial and anterior cerebral arteries were found only rarely by CT after cerebral trauma. Infarcts at the w

[A rare cause of acute circulatory failure with pulmonary edema: catecholergic cardiomyopathy of pheochromocytoma].

We report the case of a 37 years old woman with pheochromocytoma of the adrenal gland, with acute circulatory failure and pulmonary oedema. Its originality lies, besides the rare circumstances of diagnosis, in the echographic aspect of the left ventricle with an apical myocardial infarction with both severe apical hypokinesia and compensating basal hyperkinesia. In the literature the most frequent aspect of catecholamine cardiomyopathy is a large uniform dilatation of the left ventricle with often wall hypertrophy. After surgical ablation of the left adrenal gland tumor, we observe quickly a c

Spontaneous Intramural Esophageal Hematoma Secondary to Thrombolysis in the Setting of Pulmonary Embolism.

Intramural hematoma of the esophagus (IHE) represents a rare condition on the spectrum of esophageal injuries. The most common symptoms are hematemesis, epigastric pain or retrosternal chest pains, odynophagia, and dysphagia. Early recognition of IHE is important as it may mimic other diseases such as myocardial infarction, pulmonary embolism, Mallory-Weiss tears, Boerhaave's syndrome, ruptured aortic aneurysms, and aortic dissection. Computed tomography is the preferred investigation method, and treatment is usually conservative. We herein present 2 cases of IHE associated with catheter-direc

[Right ventricular assessment with echocardiography].

Right ventricular (RV) function is essential in cardio--pulmonary physiology and physiopathology. RV dysfunction has prognostic impact in inferior myocardial infarction, significant valvulopathies, congenital cardiopathies, arterial pulmonary hypertension and in patients suffering from acute or chronic heart failure. RV analysis relies on non invasive (echocardiography-Doppler, isotopic technology, cardiac magnetic resonance imaging) and/or invasive approaches (right cardiac catheterization). Neglected a short time ago, RV assessment has regained interest with tissular Doppler imaging, strain

Cardiovascular magnetic resonance of anomalous coronary arteries.

Cardiovascular magnetic resonance of anomalous coronary arteries is a class I indication. The term anomalous coronary artery encompasses those with an abnormal origin (from the incorrect sinus, too-high or too-low from the correct sinus, or from the pulmonary artery) and/or number of ostia. Their clinical significance results from the increased risk of myocardial infarction and sudden cardiac death associated with those traversing an interarterial course between the aorta and main pulmonary artery/right ventricular outflow tract. In this article, we review the role and practice of cardiovascul

Necrotizing aspiration pneumonia.

Infectious disease is the most common etiology of a cavitary lung process, which can occur with necrotizing pneumonia, lung abscess and septic pulmonary embolism. Other infectious causes of pulmonary cavitation include cavitating fungal or mycobacterial granulomas and superinfection of preexisting spaces. Noninfectious causes of cavitation that should be considered in the differential diagnosis are infarction from thromboembolic disease or vasculitis, necrotic primary or metastatic tumor, rheumatoid nodules and air-filled congenital cysts. The clinical history combined with radiographic featur

Persistent left superior vena cava and right superior vena cava drainage into the left atrium without arterial hypoxemia.

A 46 year old patient who presented with an acute myocardial infarction was discovered to have a systemic venous communication with the left heart during attempted insertion of a pulmonary flotation catheter. There was no evidence of cyanosis or systemic arterial desaturation. A right superior vena cava that emptied into the right superior pulmonary vein and a persistent left superior vena cava draining into the coronary sinus were confirmed pathologically after death related to a brain abscess. The embryology, physiology and noninvasive diagnostic approach to this unique venous anomaly are di

Bilateral Acute Renal Infarction Due to Paradoxical Embolism in a Patient with Eisenmenger Syndrome and a Ventricular Septal Defect.

A 52-year-old man who was diagnosed with Eisenmenger syndrome due to a muscular-type ventricular septal defect 30 years previously, visited our emergency room after experiencing six hours of severe left flank pain and vomiting. On laboratory examination, azotemia and microscopic haematuria were identified. Contrast-enhanced computed tomography also revealed pulmonary embolism (PE) and bilateral acute renal infarction. The flank pain resolved after heparin was administered for anti-coagulation and aspiration thrombectomy was performed. The patient was discharged on warfarin as anticoagulant the

[A case of emergency surgery for acute mitral regurgitation due to complete papillary muscle rupture as a severe form of reperfusion injury: report of a case].

Complete rupture of a papillary muscle following acute myocardial infarction is a severe complication associate with acute left ventricular failure and pulmonary edema. Since the introduction of acute percutaneous transluminal coronary angioplasty (PTCA) in the management of acute myocardial infarction, the frequency of this complication has further decreased. We described a patient who had acute mitral regurgitation due to complete rupture of papillary muscle rupture after successful coronary intervention. Transesophageal echocardiography demonstrated severe mitral regurgitation and the ruptu

Sarcoidosis with fever and a splenic infarct due to CMV or lymphoma?

We present a case of an adult female with a past history of pulmonary sarcoidosis who presented with fever, night sweats, profound fatigue, and LUQ abdominal pain. Sarcoidosis is an afebrile disorder (excluding Lofgren's syndrome, Heerfordt's syndrome or neurosarcoidosis). Therefore, the presence of fever with sarcoidosis should suggest infection, usually viral, or lymphoma. Sarcoidosis-lymphoma syndrome describes the evolution of a lymphoma in long standing sarcoidosis. Fever aside, possible lymphoma is suggested by otherwise unexplained fever, pleural unilateral effusion, highly elevated ESR

[Abnormal origin of the left coronary artery from the pulmonary artery trunk; a fortuitous discovery in a 52 year-old man].

Anomalous origin of the left coronary artery from the pulmonary artery is a rare congenital abnormality but with a very poor prognosis at birth. Myocardial ischaemia, infarction and sudden death are common. Cases diagnosed in adulthood are exceptionally rare. The authors describe the case of an asymptomatic 52 year old man with this congenital abnormality diagnosed after routine electrocardiography had shown sequellae of anterior myocardial infarction. This case would seem to be one of the rarest cases diagnosed fortuitously at this age. In view of the good clinical tolerance of the disease an
