BACKGROUND: Esophageal replacement (ER) in pediatric patients is complex, and while gastric or colonic interpositions are common, they carry long-term complications. Supercharged jejunal interposition (JI) offers better function but is technically challenging due to variable vasculature. We describe a protocol using computed tomography angiography (CTA) and 3D reconstruction to optimize surgical planning and vessel selection.
METHODS: A retrospective review of patients undergoing supercharged JI for ER was conducted. Preoperative CTA of the chest and neck assessed the internal thoracic arteri

BACKGROUND: The management of a high voltage electrical injury and lower limb salvage remains a challenging task for plastic surgeons. Reconstruction with flaps is often the only alternative to limb amputation. The purpose of this study was to present a cross flow-through pedicle free latissimus dorsi muscle flap for the salvage of severely traumatized lower limbs perfused by one remaining vessel (a single vessel lower limb) in high voltage electrical injuries.
METHODS: In this retrospective study, between 2000 and 2014, six men underwent cross-leg free Latissimus dorsi muscle flap operations

In this report, we present a scalp defect reconstruction with lateral arm free flap. We highlight the difficulty in obtaining a recipient vein and the venous drainage managed through an open end of the donor vein. A 52-year-old woman presented with a pressure sore on the left scalp. A lateral arm free flap was transferred to cover this 8×6 cm defect. The arterial anastomosis was successful, but no recipient vein could be identified within the wound bed. Instead, we used a donor venous end for the direct open venous drainage. In order to keep this exposed venous end patent, we applied heparin-s

Tufted angioma (TA) is a rare benign vascular neoplasm characterized histopathologically by the proliferation of endothelial cells arranged in lobules in the dermis and subcutaneous fat. To date, about 200 cases have been reported, most of which are of Japanese ethnicity. TA predominantly affects children and young adults, developing in 80% of patients younger than 10 years. A white 72-year-old renal transplant recipient presented with 2 asymptomatic dusky red papules on his right leg. The lesions appeared 5 years after the start of immunosuppressive treatment. Histopathologic examination show

BACKGROUND: Lymphedemas due to local lymphatic blocks can be treated by microsurgical transplantation or transposition of lymphatic vessels. Here, the anastomoses are usually made end-to-end between lymphatics, but occasionally appropriate lymphatic recipient vessels are missing. In such cases, reconstructing lymph drainage by connection to a lymph node could be another technical option. The purpose of this study was to examine the patency rate of such lympho-lymphonodular anastomoses in an experimental animal model.
METHODS: Male Sprague-Dawley rats were anesthetized, and the retroperitoneum…

Ultrasonography (US) plays a major diagnostic role in the pre- and post-transplant evaluation of recipient and donor. In most cases, US remains the only necessary imaging modality. After pediatric kidney transplantation, US can ensure immediate bedside diagnosis of vessel patency and possible postoperative non-vascular complications. Criteria for US diagnosis of kidney vessel thrombosis and stenosis in the transplant will be presented. Non-vascular complications after kidney transplantation include hydronephrosis, hematoma, lymphocele, and abscess. US can detect suggestive, but nevertheless no

BACKGROUND: Most organs are maintained lifelong by resident stem/progenitor cells. During development and regeneration, lineage-specific stem/progenitor cells can contribute to the growth or maintenance of different organs, whereas fully differentiated mature cells have less regenerative potential. However, it is unclear whether vascular endothelial cells (ECs) are also replenished by stem/progenitor cells with EC-repopulating potential residing in blood vessels. It has been reported recently that some EC populations possess higher clonal proliferative potential and vessel-forming capacity com

BACKGROUND: For successful microsurgical reconstruction using free tissue transfer, healthy recipient vessels must be obtained from outside the zone of injury. Securing an appropriate length pedicle length is also essential, and various techniques for lengthening a vascular pedicle have been developed. Herein, we present our experience using the descending branch (DB) of the lateral circumflex femoral vessels (LCFVs) with a thigh flap as an extender graft for consecutive second flap.
METHODS: We reconstructed the complex and vessel-depleted defects of nine patients. The mean age was 47.6 year…

BACKGROUND: Simultaneous reconstruction of two separate defects by two free flaps is time consuming and often requires two donor sites. The anterior and lateral aspect of the thigh is an ideal donor site for free tissue harvest without incurring significant morbidity. In this article, the authors describe their recently developed technique that allows for harvesting two independent fasciocutaneous free flaps from the same descending branch of lateral circumflex femoral vessel as a new clinical application of the versatile anterolateral thigh flap.
METHODS: A total of eight flaps were develope

This paper describes a technique used to treat an end-stage kidney disease patient, a kidney transplant recipient, who was diagnosed with an advanced aneurysmal degeneration of the brachial artery. The current report presents a novel, staged technique of surgical vascular repair with a previously arterialized autologous great saphenous vein (GSV), along with a 12-month follow-up observation period. The patient was treated with an arterialised great saphenous vein bypass that was not primarily suitable for use as a conduit due to its small diameter. To achieve a satisfactory vessel size, an art

A new surgical technique has been devised for the prevention of wound infection in cadaveric renal transplant recipients with severely infected bladders; the ascending colon is mobilized through a transperitoneal approach. The peritoneum covering the iliac vessels are retracted after blunt dissection, thereby forming a pouch. After completion of the vascular anastomoses, the ureter is brought into the peritoneal cavity through a small hole made in the peritoneum. The ascending colon is refixed to the abdominal wall so that the graft is separated from the bladder. No local or systemic infection

Calciphylaxis is a rare disease characterized by calcification of small- to medium-sized blood vessels in the dermis and subcutaneous fat, resulting in cutaneous necrosis. Although most commonly shown in patients with end-stage kidney disease, it has also been reported in patients with other diseases, including alcoholic cirrhosis and malignancies. Here, we report an unusual case of calciphylaxis in an orthotopic liver transplant recipient with acute kidney injury. The patient, a 43-year-old white female with a history of type 2 diabetes mellitus, alcoholic cirrhosis, and normal kidney functio

A living-related small bowel transplantation (SBT) was performed in two pediatric patients with short bowel syndrome. In both cases, the donor was the patient's mother. The distal ileum (100 cm, 120 cm) was harvested and the ileocolic vessels, ileocecal valve, and terminal ileum were left intact. The two donors were discharged from the hospital on postoperative days 15 and 6, respectively. Recipient 1 was a 2 year 6 month-old boy with short bowel syndrome who underwent SBT due to loss of venous access. The graft vein was anastomosed to the recipient's infrarenal inferior vena cava.
