Cross-resistance to both atracurium- and vecuronium-induced neuromuscular blockade in a critically ill patient.

A previously healthy 33-year-old woman received neuromuscular blocking agents during treatment of severe adult respiratory distress syndrome secondary to pneumococcal pneumonia and septic shock. Atracurium infusion rates were progressively increased, preceded by repeated loading doses up to a maximum of 3.57 mg/kg/hour, but produced inadequate neuromuscular blockade as assessed by clinical and ventilatory parameters as well as train-of-four (TOF) monitoring. Atracurium was discontinued and vecuronium infusions of 2.3 mg/kg/hour finally produced adequate paralysis for 7 days. Increasing vecuron

The First Case of Riboflavin Transporter Deficiency in sub-Saharan Africa.

This report describes the first case of a child with genetically confirmed Brown-Vialetto-van Laere syndrome in sub-Saharan Africa. This is an extremely rare clinical condition that presents with an auditory neuropathy, bulbar palsy, stridor, muscle weakness, and respiratory compromise that manifests with diaphragmatic and vocal cord paralysis. It is an autosomal recessive condition for which the genetic mutation has only recently been linked to a riboflavin transporter deficiency. We describe an 11-month-old affected male infant. He has required long-term respiratory support and a gastrostomy

[A case of emergency admission for CO2 narcosis in a patient with amyotrophic lateral sclerosis].

A 68-year-old man with severe dyspnea was admitted as an emergency case. He had no past history of any respiratory or neuromuscular diseases. Immediately after insufflation of oxygen, respiratory arrest occurred. The blood gas analysis showed hypoxemia and severe hypercapnia (PaO2; 32 mmHg, PaCO2; 127 mmHg). We diagnosed as CO2 narcosis, and he was treated with a respirator in the ICU. He showed nonflaccid bilateral diaphragmatic paralysis and muscle atrophy of the upper extremities. As the EMG showed giant spikes of neurogenic pattern, he was diagnosed as ALS. Weaning from the respirator fail

Respiratory dysfunction in Guillain-Barré Syndrome.

Guillain-Barré Syndrome is the leading cause of nontraumatic acute paralysis in industrialized countries. About 30% of patients have respiratory failure requiring intensive care unit (ICU) admission and invasive mechanical ventilation. Progressive weakness of both the inspiratory and the expiratory muscles is the mechanism leading to respiratory failure. Aspiration pneumonia and atelectasis are common consequences of the bulbar muscle weakness and ineffective cough. The classical signs of respiratory distress occur too late to serve as guidelines for management, and measurements of vital capac

Tracheotomy for infant botulism.

Botulism is a serious intoxication caused by ingestion of food containing preformed botulinus toxin and characterized by rapidly progressive bulbar paralysis, generalized weakness, and respiratory insufficiency. In 1976 a distinct clinical entity of infant botulism was recognized. The disease apparently results from intraintestinal toxin production which produces a defect in neuromuscular transmission by interfering with release of acetylcholine at cholinergic synapses. Five cases of infant botulism were identified at the Children's Hospital of Philadelphia between 1975 and 1977. Initial sympt

Patient Perspective: Words That Matter: Reflections on a Patient's Experiences With Guillain-Barré Syndrome.

Guillain-Barré syndrome (GBS) is a rare immune-mediated polyradiculoneuropathy. When severe, GBS can result in near-total paralysis and respiratory failure. Psychological symptoms are common but under-recognized complications of GBS and may develop because of physical disability and social isolation. In this article, we share the patient's perspective of the psychological effects of GBS and how words of encouragement, a listening ear, communication aids, and dedicated caregiving made the difference in the patient's physical and emotional recovery.

Systemic lupus erythematous with recurrent Guillain-Barré-like syndrome treated with intravenous immunoglobulins.

We report on an unusual case of a young African male with systemic lupus erythematosus who developed an acute, severe Guillain-Barré-like peripheral neuropathy associated with ascites. The illness appeared to respond dramatically to a short course of intravenous immunoglobulins. Eight months later he had recurrence of identical features. He required respiratory support. On this occasion there was no response to intravenous immunoglobulins or other measures and he died of sepsis after a protracted period of paralysis.

Nasopharyngeal teratoma involving the temporal bone.

Teratoma is the most common nasopharyngeal tumor in neonates. The tumor is usually limited to the oropharynx and is manifest by stridor and respiratory distress. Complete excision affects a cure. An unusual case of nasopharyngeal teratoma extending into the temporal bone, causing facial paralysis and conductive hearing loss, in addition to stridor, is presented. Stridor was relieved by transoral partial excision shortly after birth. Complete removal of the tumor by way of a subtemporal and infratemporal fossa approach was performed at 14 months of age. The defect was filled with a rectus abdom

[Tongue function in the rehabilitation of speech disorders, faulty breathing habits and deglutition disorders].

The tongue is as an organ of speech and articulation not as indispensable as the soft palate. The recovery of speech after extirpation of the tongue is usually satisfactory, and the intelligibility of patients with microglossia compares favourably with the speech of an untreated cleft palate patient. Hypernasality and nasal escape of air are typical also for the congenital short palate (in the absence of a cleft); syndromes with a congenital short palate are often misdiagnosed as suprabulbar paralysis. The function and involuntary usage of the tongue e.g. in swallowing, influence the growth an

[Experimental treatment of respiratory failure caused by omethoate poisoning in rats].

OBJECTIVE: To examine the therapeutic effect of combined use of pralidoxime-Cl and atropine with artificial ventilation on respiratory muscle paralysis caused by omethoate poisoning in rats. METHODS: Rats were administered with same doses of 2LD(50) omethoate and then treated with atropine (10 mg/kg) to resist effectively chlolinergic symptoms. When the rats had slow respiratory frequency and breathed with difficulty, the trachea was intubated and artificial ventilation was carried out (except for group A). The rats in group B were continuously treated with atropine. The doses of pralidoxime-

[Respiratory distress due to traumatic lesions in newborn infants and in adults].

1. The clinical features of the condition we presently term resipratory distress syndrome of neonates, was described by Jörg in 1832 and 1835. 2. In many, however not in all cases in which this syndrome is clinically observed, band-like ("membranous") and also massive depositions of fibrin occur in bronchioli, alveolar ducts and alveoli. They develop as the consequences of acapillary exudation process. 3. More frequently and more regularly, often combined with fibrin precipitation, intraalveolar, intrabronchiolar and intrabronchial hemorrhages arise in such cases. Peribronchial, peribronchiola

Rhabdomyosarcoma Mimicking Otomastoiditis in a Preschool Child with Facial Paralysis.

Rhabdomyosarcoma of the temporal bone is an uncommon but important malignant mimic of pediatric otomastoiditis. We describe a 4-year-old boy who presented with persistent right-sided otorrhea after an upper respiratory tract infection, followed by an external auditory canal mass and progressive right peripheral facial nerve paralysis. Temporal bone computed tomography showed soft-tissue opacification of the right external auditory canal, tympanic cavity, mastoid antrum, and mastoid air cells. Magnetic resonance imaging demonstrated an irregular, heterogeneously enhancing lesion involving the s

Herpes Viral Origin of the Parsonage-Turner Syndrome: Highlighting of Serological Immune Anti-Herpes Deficiency Cured by Anti-Herpes Therapy.

In 2012, a 50 year-old athletic male presented with weakness, pain and unilateral phrenic paralysis, followed by bilateral phrenic paralysis with deep dyspnea. In 2013, the Parsonage-Turner syndrome was diagnosed. When the patient was seen in September 2014 for the first time, he was facing phrenic neuromuscular failure, which led to the hypothesis of neurotropic herpes viruses. A control of the global serological anti-Herpes immunity to analyze his antibody (Ab) levels confirmed herpes immune genetic deficiency. An appropriate herpes chemotherapy treatment was proposed. Immediately, a spectac

Battery ingestion leading to bilateral vocal cord paresis.

IMPORTANCE: Disk battery ingestion is common in the pediatric population, with over 50,000 ingestions reported annually. In the upper aerodigestive tract, consequences of such ingestions vary widely from superficial mucosal ulcerations to death from erosion through vital structures. This report describes a battery ingestion complication, vocal cord paralysis, to our knowledge not previously described in the otolaryngology literature. OBSERVATIONS: We describe a patient who presented with biphasic stridor and drooling after upper esophageal disk battery ingestion. The battery was removed 5 hou

Coral snake bites (Micrurus spp.) in Brazil: a review of literature reports.

CONTEXT: In the Americas, the main representatives of the family Elapidae are coral snakes of the genus Micrurus, of which 33 species are in Brazil. They are the smallest cause of venomous snakebite in Brazil. We analyzed literature reports of coral snake bites in Brazil from 1867 to 2014, and provide a brief review of case series and reports of coral snake bites in the Americas in general. METHODS: Only reports with clinical descriptions of envenomation were included. The variables recorded included identification of the offending snake, patient's age, sex, bite site, clinical manifestations

Recovery of Vocal Cord Motion Among Pediatric Patients.

OBJECTIVES: Patients with vocal cord paralysis can experience feeding, respiratory, and vocal problems leading to disability and decreased quality of life. Current evidence suggests waiting a period of 12 months for spontaneous recovery before permanent interventions. This study aims to determine the time to recover spontaneously and vocal cord movement in a pediatric population and create a model for evidence-based patient counseling. STUDY DESIGN: Retrospective longitudinal cohort study. METHODS: The report is a single institution longitudinal study on vocal cord paralysis recovery. Patien

Contact laser surgery in treatment of vocal fold paralysis.

Vocal fold paralysis is a pathological condition characterised by varying degrees of respiratory distress in relation to the degree of glottic stenosis. Dyspnoea may be present even when resting and may even require emergency tracheotomy. Frequently, the patient arrives for attention after the onset of exertional dyspnoea associated with a certain degree of dysphonia. The causes may be central or peripheral, more commonly iatrogenic following thyroid or tracheal surgery or secondary to injury. The aim of all surgical techniques used in the treatment of vocal fold paralysis is to restore a lume
