Optical Coherence Tomography Angiography of Combined Central Retinal Artery and Vein Occlusion.

Optical coherence tomography angiography (OCTA) is a new, noninvasive technology that enables detailed evaluation of flow in the retinal and choroidal vasculature. The authors believe this to be the first report to describe the optical coherence tomography angiography findings associated with combined central retinal artery occlusion (CRAO) and central retinal vein occlusion (CRVO).

Stroke Risk and Risk Factors in Patients With Central Retinal Artery Occlusion.

PURPOSE: Central retinal artery occlusion (CRAO) is mechanistically similar to a stroke. Current guidelines recommend a standardized and systematic evaluation of risk factors for patients who have had a stroke. This study evaluates the yield of this evaluation in patients with CRAO and frequency of stroke in this population. DESIGN: Cohort study. METHODS: We evaluated the diagnostic yield of an expedited inpatient evaluation of cerebrovascular risk factors in a cohort of patients presenting with an acute CRAO within the period from 2009 to 2017 at an academic hospital. Vital signs and labora…

Branch retinal artery occlusion post-penetrating globe injury with intraocular foreign body.

Intraocular foreign body (IOFB) in cases of penetrating eye injury accounts for an important indication of vitreoretinal intervention following ocular trauma. Vascular occlusion as a complication of IOFB is rare. Here we present a case of a 34-year-old male with post-traumatic cataract and an intraocular metallic foreign body (IOFB) lodged in the superficial layers of the retina inferotemporal to the disc, causing an inferotemporal branch retinal artery occlusion. The case was managed by lensectomy with pars plana vitrectomy and IOFB removal followed by a second procedure of secondary IOL impl…

Triamcinolone emboli leading to central retinal artery occlusion: a multimodal imaging study.

A 19-year-old man presented with unilateral sudden onset vision loss following an intra-articular triamcinolone injection in the right temporomandibular joint. At fundus examination emboli of triamcinolone were visible in multiple retinal arteries. Choroidal ischaemia and occlusion of the central retinal artery and its branches were documented at fluorescein angiography. Optical coherence tomography revealed significant thickening of the inner retinal layers. Optical coherence tomography angiography clearly demonstrated an abrupt cut-off of flow in a vessel below the optic disc. Lack of perfus…

Iatrogenic central retinal artery occlusion after carotid body tumor embolization and excision.

Objective: To report a case of iatrogenic central retinal artery occlusion after embolization and surgical resection of carotid body paraganglioma. Methods: Case report Results: One adult female patient presented with persistent unilateral visual loss after embolization with Embosphere® and Contour® microparticles of carotid body tumor. Fluorescein angiography revealed intraluminal microspheres in the central retinal artery ramifications. OCT revealed intraretinal spherical, hyporeflective particles with posterior shadowing. Conclusions: Central retinal artery occlusion should be assessed as a…

Vision Loss After Central Retinal Artery Occlusion Secondary to Orbital Sarcoid Mass.

The authors describe the first report in the literature of central retinal artery occlusion as the presenting manifestation of sarcoidosis. A 33-year-old man with asthma, headache, and 6 days of intermittent, transient vision loss in the OS presented with persistent vision loss in the OS. Ophthalmic examination was consistent with diagnosis of central retinal artery occlusion in the OS. Vascular imaging with CT angiography revealed an incidental finding of an intraconal mass surrounding the left optic nerve and hilar lymphadenopathy. Broncho scopic lymph node biopsy demonstrated noncaseating g…

The spectrum of superficial and deep capillary ischemia in retinal artery occlusion.

PURPOSE: To describe the spectrum of retinal capillary ischemia, including superficial and deep capillary ischemia, as identified with spect … superficial and deep capillary ischemia in each eye. RESULTS: Of the 40 eyes, 15 eyes had central retinal artery occlusion (CRAO), 22 eyes had branch retinal artery occlusion (BRAO), and 3 eyes had cilioretinal artery occlusion. During the acute phase, SD OCT showed the following 3 distinct patterns, related to retinal ischemia occurring at varying levels with …

Seronegative granulomatous polyangiitis with central retinal artery occlusion as the initial manifestation.

PURPOSE: To report a case of central retinal artery occlusion in a patient with autopsy-verified granulomatous polyangiitis (GP) with negative cytoplasmic antineutrophil cytoplasmic antibody. METHODS: Case report. RESULTS: A 61-year-old Hispanic man with history of tuberculosis was admitted to the hospital with worsening dyspnea. Two weeks later, he experienced sudden vision loss due to central retinal artery occlusion in his right eye. A CT of the lung revealed multiple opacities. He developed renal failure during his hospital stay. A serum cytoplasmic antineutrophil cytoplasmic antibody te…

Unusual presentation of a multiple sclerosis case involving central retinal artery occlusion.

The term intermediate uveitis (IU) refers to a subgroup of uveitis in which the vitreous is the site of greatest inflammation. Patients with multiple sclerosis (MS) have a greater frequency of IU compared with the general population. The IU associated with MS is characterized by the presence of pars planitis (occasionally accompanied by anterior uveitis) and the presence of peripheral retinal vasculitis in the form of periphlebitis (venous sheathing) in 6-26% of patients. We present a patient with an unusual initial presentation of MS involving central retinal artery occlusion (CRAO) in the ri…

Central retinal artery occlusion caused by fat embolism following endoscopic sinus surgery.

Endoscopic sinus surgery (ESS) can lead to a variety of ophthalmic complications. Central retinal arterial occlusion (CRAO) is one such complication, usually due to orbital compartment syndrome. We report a case of CRAO following endoscopic sinus surgery as a result of fat embolism.

Macular branch retinal artery occlusion as the first manifestation of ocular toxoplasmosis.

BACKGROUND: Ocular toxoplasmosis can cause a variety of retinal vascular changes including branch retinal arterial occlusion, which is a rare complication of the disease. METHODS: An unusual toxoplasma chorioretinitis presenting a macular branch retinal arterial occlusion adjacent to the active chorioretinitis lesion is reported. RESULTS: The patient received an appropriate steroid and antibiotic treatment, and the retinitis lesion resolved over a 6-week period. At 2 months after diagnosis, visual acuity in her left eye was 20/30 and there was a hyperpigmented scar at the site where active r
