Steroid therapy has been used as a treatment for idiopathic retroperitoneal fibrosis for the past ten years and more. In 14 out of a series of 19 patients, this treatment was well-tolerated and successful. It acts quickly. The results of medical treatment compare favorably with those of surgical ureterolysis. In the authors' view, steroid therapy is therefore the treatment of choice for this complaint, and surgical management should be confined to the relatively rare setbacks of medical treatment, and to patients who do not tolerate steroids.

Eleven of 157 patients undergoing surgery for abdominal aortic aneurysm, with features of both aneurysm and periaortic retroperitoneal fibrosis, are presented. In a series of patients with idiopathic retroperitoneal fibrosis followed for up to 22 years, 21 were treated after ureterolysis of 36 ureters by wrapping the ureters in omentum. Recurrent fibrosis was noted in 2 patients involving 2 ureters. Earlier in the same period 3 patients with 4 ureters were treated by intraperitoneal placement after ureterolysis: in these, recurrence was noted in 2 patients involving 2 ureters. A 20-year experience with idiopathic retroperitoneal fibrosis (RPF) is presented (6 cases). Its gynecologic manifestations with the simulation of pelvic malignancy is discussed. Three of the patients presented with pelvic masses, one of these a para-vaginal mass. In one patient, RPF followed or was concurrent with invasive squamous cell carcinoma of the cervix, of which this is the first reported case. A new method of surgical treatment for retroperitoneal fibrosis is demonstrated on 3 patients. 31 months postoperatively the results are excellent. The operation is easy to perform, without complications and recurrences.

Idiopathic retroperitoneal fibrosis may rarely invade the ureteral wall, thereby rendering the attempt to perform classical ureterolysis futile. Possibilities for surgical correction include resection with end-to-end anastomosis, utilization of a spiral strip of ureter, small bowel substitution, iatrogenic nephroptosis or longitudinal incision of the involved ureter down to the mucosa. Erdheim-Chester disease is a rare multisystemic non-Langerhans histiocytosis with about 500 reported cases. Typical features include retroperitoneal and perirenal fibrosis (hairy kidney), periaortitis with a coated aorta, osteosclerosis of the lower limbs, and sometimes exophthalmia or diabetes insipidus. Retroperitoneal fibrosis is an uncommon disease characterized by inflammatory fibrosis typically surrounding abdominal aorta and iliac arteries. The glucose analogue F18-fluorodeoxyglucose can be used to image inflammatory cell activity non-invasively by PET. In this report we investigated the usefulness of the FDG PET/CT in the disease activity and therapy response evaluation of retroperitoneal fibrosis.
